Burr Hole Endoscopic Mastoidectomy: A Morphometric Cadaveric Study
Conclusion We demonstrated the feasibility of an endoscopic mastoidectomy through a standardized postauricular burr hole. This approach reduces the incision size and the need for soft tissue dissection. Burr hole mastoidectomy is facilitated using angled scopes which are not reliant on 0-degree line-of-sight. Although the endoscopic approach afforded slightly less exposure, the location and burr hole size can be adjusted depending on the clinical indications. Analysis of Treatment Modalities for Advanced Stage Squamous Cell Carcinoma of the Maxillary Sinus: A National Cancer Database Study
Conclusion Multimodal treatment of advanced stage maxillary SCCa has a variable effect on outcomes by tumor stage. Our findings suggest that surgery plus neoadjuvant and surgery plus adjuvant treatment are associated with higher rates of survival. Increasing age, Charlson–Deyo score ≥ 1, and undifferentiated/anaplastic grade were associated with worse rates of survival. Further randomized controlled trials are required to quantify the therapeutic benefit of these treatments on survival and organ sparing in advanced stage disease. Outcome Measures for Medical and Surgical Treatment of Prolactinomas. Is the Role of Surgery Underestimated?
Conclusions Surgery for macroprolactinomas achieved greater tumor volume reduction and dose reduction or discontinuation of DAs. Prolactin levels achieved did not differ significantly between the groups. Over 60% of patients, undergoing surgery, were able to discontinue DAs. The current treatment paradigm may underestimate the role of surgery in the management of prolactinomas. Epidural Interlay Graft for Pituitary Surgery: A 4-Year Institutional Experience
Conclusions Simple sellar repair using an epidural interlay graft is highly effective in preventing postoperative CSF leaks, including cases of high-flow intraoperative leaks. More complex repair techniques are reserved for select high-risk cases or those with inadequate sellar bony ledges. With this technique, there has been no postoperative CSF leak at our institution in over 300 consecutive transsphenoidal pituitary surgeries.
