Anticoagulation helps shrink giant venous lakes and arteriovenous fistulas in dural sinus malformation
Conclusions Anticoagulants may help to potentiate transarterial embolization in DSMs in babies by decreasing venous dilatation and reducing the remaining arteriovenous shunt, particularly the pial feeders. We did not observe recurrence of arteriovenous shunts after treatment, especially during anticoagulation treatment. Further studies are needed to support our findings. Effects of intravenous thrombolysis on stent retriever and aspiration thrombectomy outcomes: a systematic review and meta-analysis of the randomized controlled trials
Conclusions Our findings suggest that SR and ASP thrombectomy have comparable safety and efficacy profiles, regardless of prior IVT administration. Additionally, our results indicate that the addition of IVT may improve certain efficacy outcomes based on the employed first-line MT technique. (Source: Journal of NeuroInterventional Surgery)

Optimized density and locations of stroke centers for improved cost effectiveness of mechanical thrombectomy in patients with acute ischemic stroke
Conclusion The combination of optimization modeling and cost effectiveness analysis provides a powerful tool for configuring the extent and locations of CSCs (and TSCs). The most cost effective implementation of CSCs in Sweden entails 24/7 MT services at all seven university hospitals. (Source: Journal of NeuroInterventional Surgery)

Influence of vascular imaging acquisition at local stroke centers on workflows in the drip-n-ship model: a RACECAT post hoc analysis
Conclusion Among patients with a suspected LVO stroke initially evaluated at a LSC, vascular imaging acquisition might improve workflow times at thrombectomy-capable centers and reduce the rate of futile interhospital transfers. These results deserve further evaluation and should be replicated in other settings and geographies. (Source: Journal of NeuroInterventional Surgery)

Cost-utility of direct transfer to angiography suite (DTAS) bypassing conventional imaging for patients with acute ischemic stroke in Spain: results from the ANGIOCAT trial
Conclusions For patients with LVO admitted within 6 hours after symptom onset, the DTAS not only improves clinical outcome but also decreases the costs (dominant option) compared with the standard DTCT. Multicentric international randomized clinical trials are ongoing to determine the replicability of our findings. (Source: Journal of NeuroInterventional Surgery)

Neutrophil activation in patients treated with endovascular therapy is associated with unfavorable outcomes and mitigated by intravenous thrombolysis
Conclusions A peak in plasma MPO concentration occurs early after EVT and is associated with unfavorable outcomes. IVT abolished the post-EVT MPO peak and may modulate the association between NA and outcomes. (Source: Journal of NeuroInterventional Surgery)

Focused update to guidelines for endovascular therapy for emergent large vessel occlusion: large core and basilar artery occlusion patients
Background Endovascular therapy (EVT) dramatically improves clinical outcomes for patients with emergent large vessel occlusion (ELVO). Later this year, the Society of NeuroInterventional Surgery (SNIS) Standards and Guidelines Committee will update and supplement the existing SNIS guidelines on ‘Embolectomy for stroke with emergent large vessel occlusion (ELVO)’, ‘Indications for thrombectomy in acute ischemic stroke from emergent large vessel occlusion (ELVO)’, and ‘Current endovascular strategies for posterior circulation large vessel occlusion stroke’.1–3 However, given the imp...

New online tools to speed-up training of neurointerventionalists: an example in Central Asia
There is significant heterogeneity in access to endovascular stroke therapy (EVT) worldwide. Even within Europe, the rate of emergent large vessel occlusion patients treated with EVT ranges from <1% to >15%.1 In order to achieve a more uniform access rate that nears 15%, something like 500 new teams with EVT capabilities must be created in Europe. As difficult as this may look, at least this problem can be tackled by expanding a solid basis of existing regional stroke services and experiences. The situation is completely different in Central Asia, where most countries have very few EVT resources for stroke. The Uzbek...

Response to: Correspondence on 'Comparison of drug-coated balloon with conventional balloon for angioplasty in symptomatic intracranial atherosclerotic stenosis by Zhang et al
We appreciate the comments on the definitions of technical success and restenosis in our recent study1 provided by Zhang et al,2 which prompted us to write this response for clarification. In our article we defined technical success as ≤50% residual stenosis and restenosis as >50% stenosis, which have been previously reported in other studies.3 Also, as we know, stenosis >50% and absolute luminal loss >20% is used in a current clinical trial.4 Zhang et al mentioned that stent implantation could be considered a remedy for obvious residual stenosis after balloon dilation. We agree with this perspective but, as me...
