Thrombus radiomics in patients with anterior circulation acute ischemic stroke undergoing endovascular treatment
Conclusions
TR describing thrombus volume and texture were associated with more attempts to successful reperfusion. Compared with models using CV and MTM, TR had no added value for predicting procedural and functional outcome. (Source: Journal of NeuroInterventional Surgery)

Contrast conservation measures during the global iohexol contrast shortage crisis did not affect stroke thrombectomy outcomes
Conclusions
Modifying stroke workflow to adapt to the current global shortage in iohexol is feasible. Using diluted iohexol (50% contrast mixed with 50% heparinized saline) did not affect MT outcomes. (Source: Journal of NeuroInterventional Surgery)

Augmentation of perfusion with simultaneous vasodilator and inotropic agents in experimental acute middle cerebral artery occlusion: a pilot study
Conclusion
Results suggest that perfusion can be augmented in ischemic stroke with norepinephrine and hydralazine. Perfusion augmentation depends on degree of collateralization and territory in question, with some evidence of vascular steal. (Source: Journal of NeuroInterventional Surgery)

Thrombus imaging characteristics within acute ischemic stroke: similarities and interdependence
Conclusions
Thrombus imaging characteristics are significantly intercorrelated – strong correlations should be considered in future predictive modeling studies. Clustering analysis showed there are no distinct thrombus archetypes – novel treatments should consider this thrombus variability. (Source: Journal of NeuroInterventional Surgery)

Influence of time metrics on the treatment effect of intravenous alteplase prior to endovascular treatment in MR CLEAN-NO IV
Conclusion
In MR CLEAN-NO IV, the effect of IVT prior to EVT was not modified by OTR, OTN, DTG or NTG times. Our results do not support the use of these metrics to guide IVT treatment decisions prior to EVT in comprehensive stroke centres.

Stroke imaging modality for endovascular therapy in the extended window: systematic review and meta-analysis
Conclusions
Our study suggests that, in anterior circulation LVO, the rates of functional independence may be similar when patients are selected using ADVI or NCCT for EVT in the extended time window. A simplified triage protocol does not seem to increase mortality or sICH.

In-stent restenosis after vertebral artery origin stenosis stenting: a nomogram for risk assessment
Conclusions
A nomogram and a risk evaluation table were developed with good predictive ability for in-stent restenosis among patients with VAOS, which could serve as a practical approach for individualized risk evaluation. (Source: Journal of NeuroInterventional Surgery)

Gene expression profiles of ischemic stroke clots retrieved by mechanical thrombectomy are associated with disease etiology
Conclusions
Expression profiles of stroke clots were identified between stroke etiologies and reflected different biologic responses. Cryptogenic thrombi may be related to multiple etiologies. (Source: Journal of NeuroInterventional Surgery)

Persistent perfusion abnormalities at day 1 correspond to different clinical trajectories after stroke
Conclusion
ASL hypoperfusion within the infarct and the surrounding tissue was associated with poor outcome. A more comprehensive view of the mechanisms in the hypoperfused surrounding tissue could help to design new therapeutic approaches during and after reperfusion therapies. Direct to angiosuite strategy versus standard workflow triage for endovascular therapy: systematic review and meta-analysis
Conclusion
Patients undergoing the DTAS strategy had a significant reduction in door-to-puncture and door-to-reperfusion times. This resulted in an increased rate of early neurological and 90-day functional recovery without compromising safety in LVO patients undergoing endovascular thrombectomy. Predictors and outcome of early neurological deterioration after endovascular thrombectomy: a secondary analysis of the DIRECT-MT trial
Conclusions
Postoperative END is associated with adverse functional outcome. Several non-modifiable and modifiable factors can predict END and support future treatment decision-making to improve the potential utility of endovascular thrombectomy.

Preprocedural determination of an occlusion pathomechanism in endovascular treatment of acute stroke: a machine learning-based decision
Conclusions
An ML-PM could accurately determine an occlusion pathomechanism with common preprocedural findings. A decision flowchart consisting of the four most influential findings was clinically applicable and superior to single common preprocedural findings for determining an occlusion pathomechanism. Introduction
This online supplement is the first of three featuring previously accepted manuscripts focusing on ischemic stroke. The last decade has witnessed a revolution in the endovascular management of acute ischemic stroke. Device improvement, better-constructed prospective trials, and refinements in endovascular techniques have sparked this revolution. However, critical issues still need to be addressed. Specifically, the importance of pre-thrombectomy imaging, medium-sized vessel thromboses, patient access to capable treatment centers, management of intracranial atherosclerosis, and posterior circulation disease are subjects of ...

P183/344 Real-time quantitative radial force measurements of WEB devices
ConclusionThis study could play a significant role in WEB-assisted aneurysm embolization. In light of our findings, we can say that for aneurysms larger than their diameter, or if a spherical device is not planned to be used, it seems more appropriate to choose a device with a diameter of 2 mm larger than the aneurysm diameter.Disclosure of Interestnothing to disclose (Source: Journal of NeuroInterventional Surgery)
