Learning curve for performing laparoscopic common bile duct exploration in biliary surgery 2.0 Era
ConclusionOur data suggest that the learning curve for complete competence in LC+LCBDE is approximately 60 cases, provided that proper training is available. The initial learning phase can be carried out safely and efficiently with acceptable results. (Source: Journal of Hepato-Biliary-Pancreatic Sciences)

Randomized phase III study of gemcitabine, cisplatin plus S ‐1 versus gemcitabine, cisplatin for advanced biliary tract cancer (KHBO1401‐ MITSUBA)
ConclusionsGCS is the first regimen which demonstrated survival benefits as well as higher RR over GC in a randomized phase III trial and could be the new first-line standard chemotherapy for advanced BTC. To exploit the advantage of its high RR, GCS is now tested in the neoadjuvant setting in a randomized phase III trial for potentially resectable BTC. Impact of preoperative muscle mass and quality on surgical outcomes in patients undergoing major hepatectomy for perihilar cholangiocarcinoma
ConclusionsPMI and PMD were closely associated with intraoperative blood loss and postoperative infectious complications. Additionally, PMI impacted long-term survival. These results suggest an importance of improving muscle mass and quality before surgery. (Source: Journal of Hepato-Biliary-Pancreatic Sciences)

Cefotaxime resistance in bile samples is an independent predictor of deep infectious complications after pancreaticoduodenectomy in patients with endoprosthesis
ConclusionsThe incidence of DIC is high after pancreaticoduodenectomy, with or without endoprosthesis, despite routine postoperative treatment. Cefotaxime resistance, the only independent predictor of DIC in patients with endoprosthesis, has increased over time. Hence, cefotaxime may no longer be an appropriate empirical treatment. Comparative analysis of opioid use between robotic and open pancreatoduodenectomy
ConclusionsRobotic PD patients required fewer inpatient OMEs than open PD while maintaining similar pain scores. A higher percentage of robotic PD patients tapered off of opioids prior to discharge than open surgery patients treated with a standardized opioid reduction protocol despite a shorter length of stay. These results provide a rationale for choosing robotic PD when feasible to minimize opioid use. (Source: Journal of Hepato-Biliary-Pancreatic Sciences)

Systematic review and meta ‐analysis of difficulty scoring systems for laparoscopic and robotic liver resections
ConclusionThe existing DSS were all effective in predicting difficulty of MILR. Present studies comparing between DSS have not establish a clear superior system, and the five main DSS have been found to be predictive of difficulty in LLR and 2 of these in RLR.
