Successful salvage of the left pulmonary artery in a neonate with isolated unilateral absence of the pulmonary artery
Publication date: Available online 10 January 2020Source: Journal of Cardiology CasesAuthor(s): Kota Kawada, Hirofumi Saiki, Manabu Kemmochi, Seiko Kuwata, Manabu Takanashi, Kagami Miyaji, Hideaki SenzakiAbstractIsolated unilateral absence of the pulmonary artery (UAPA) is a congenital anomaly where involution of the extrapulmonary PA is insufficient and the intrapulmonary PA is only fed by the ductus arteriosus. Affected lung disorder causes complications years after ductus closure; thus, early diagnosis is of importance to avoid these complications. Here, we present the case of a male infant who was admitted to the neona...

A case of takotsubo cardiomyopathy developing ventricular fibrillation after a pacemaker implantation
We report a case that developed potentially fatal complications after a pacemaker implantation.<Learning objective: This patient developed in-hospital cardiac arrest due to ventricular fibrillation from takotsubo cardiomyopathy after a pacemaker implantation, which is of special interest because complications leading to in-hospital cardiac arrest just after a pacemaker implantation are rare. We consider this case report as an educational case that caused potentially fatal complications even after pacemaker implantation.> (Source: Journal of Cardiology Cases)

Pregnancy with Fontan circulation: A report of case series in Japan
This report discusses pregnant women with Fontan circulation by focusing on affected Japanese women.<Learning objective: More women who have undergone the Fontan procedure reach childbearing ages due to the new surgical procedures and medications. We report five cases and all of them experienced obstetrical complications. Anticoagulation might be one of the causes of postpartum hemorrhage. Labor epidural analgesia could contribute to minimized dynamic circulatory changes during labor. This report can be the literature regarding pregnant women with Fontan circulation in Japan.> (Source: Journal of Cardiology Cases)

Strategy for estimating optimal heart rate in refractory heart failure with relative sinus bradycardia: A case report
We report a case of a man aged 58 years with an old broad anterior myocardial infarction and relative bradycardia (about 60 bpm) suffering from symptoms of congestive heart failure at rest in spite of optimal medical therapy, including the use of the inotropes. Transvenous atrial pacing during right heart catheterization indicated that an increase in heart rate (up to 80 bpm) improved hemodynamics immediately. Implantation of a pacemaker (atrial pacing of 80 bpm) was effective for stabilizing the heart failure symptoms. Transvenous atrial pacing during right heart catheterization is effective for estimating the optim...

Acute Epstein-Barr related myocarditis: An unusual but life-threatening disease in an immunocompetent patient
We report a case of acute EBV-related myocarditis in a young female, complicated with malignant ventricular arrhythmias and cardiac arrest. After 20 days of hospitalization and treatment, the patient was fit for discharge on pharmacological therapy (tapering steroids, beta-blockers, amiodarone, angiotensin-converting enzyme inhibitors, and diuretics). Clinical course is described, cardiac magnetic resonance images are shown. This case underlines how myocarditis is a disease that should not be underestimated: it could present with life-threatening complications such as malignant arrhythmias and/or severe systolic dysfunction...

Fatal eosinophilic coronary periarteritis leads to dissection in a young patient: A case report
We report the case of a 35-year-old woman who presented with sudden unexpected cardiac arrest and was brought to the emergency ward. Cardiopulmonary resuscitation was started by ambulance paramedic. On arrival, the patient was transferred to the catheterization laboratory due to sudden aborted cardiac death. The angiography was performed and dissection of the left main coronary artery, extended to the left anterior descending artery was detected. Pathological study confirmed ECPA. It appears that young and healthy patients with a history of intermittent vasospastic angina should be evaluated for ECPA.<Learning objectiv...

Isolated pulmonary valvular stenosis with equilateral triangle orifice
Publication date: Available online 10 December 2019Source: Journal of Cardiology CasesAuthor(s): Sei Tsunoda, Yuka Kobayashi, Hiroshi Hirano, Ryoji Kitamura, Shin Nakamori, Satoaki MatobaAbstractIsolated congenital pulmonary valvular stenosis is a relatively common abnormality. A 52-year-old woman with an asymptomatic cardiac murmur since her childhood visited our hospital. The peak pressure gradient across the pulmonary valve (PV) was calculated 25 mmHg by Doppler velocity during the transthoracic echocardiography, the severity was graded into mild. The cardiac computed tomography indicated a dome-shaped PV with an e...

Percutaneous exclusion of ascending aorta pseudoaneurysms: still an interventional challenge?
We report the case of a 75-year-old man with a sinotubular junction pseudoaneurysm, likely developed after coronary artery bypass grafting, which was successfully treated by implanting a multifenestrated cribriform septal occluder, a device already used in few similar cases.<Learning objective: Aortic pseudoaneurysm could be a consequence of several types of aortic injuries. In frail patients endovascular approach represents a viable option instead of surgical repair. Such percutaneous treatment has to be individualized also according to the lesion location and size, as in our case where a sinotubular junction pseudoan...

Acute myocardial infarction in young newbie bodybuilder using multiple steroid and protein supplements
In conclusion, supraphysiological doses of protein supplements, anabolic steroids, and other nutritional products bear a risk factor for CAD.<Learning objective: This is evident from the case report that excessive supplements use by body builders for immediate mass gain and performance enhancement may lead to adverse cardiovascular complications. This is mostly prescribed by peer groups or untrained gym professionals without judging their adverse effects so we recommend a detailed history for steroid use and protein supplements in young patients presenting with acute myocardial infarction without other significant risk...

Successful manual thrombus aspiration in anterior ST-segment elevation myocardial infarction due to cardioembolic obstruction of the left main coronary artery
Publication date: Available online 5 December 2019Source: Journal of Cardiology CasesAuthor(s): Diego Della Riva, Matteo Bruno, Nevio TaglieriAbstractLarge clinical trials and meta-analyses have shown that thrombus aspiration (TA) in the setting of ST-T segment elevation myocardial infarction (STEMI) undergoing primary percutaneous coronary intervention (PPCI) does not improve clinical outcome, whilst it may be associated with an increased risk of stroke. Accordingly, in the most recent European Society of Cardiology guidelines the role of routine TA during PPCI has been downgraded to a class III recommendation with level ...
