Transcatheter valve–in–valve aortic valve replacement with bioprosthetic valve Fracture/Stretching technique in a degenerated mitroflow valve
We describe a successful bioprosthetic annular stretching in a patient with severe prosthetic aortic valve stenosis from a degenerated 19-mm Mitroflow valve (Sorin Group USA Inc, Arvada, CO, USA). This technique allowed for implantation of a 23-mm Evolut-R Pro valve (Medtronic, Minneapolis, MN, USA) with significant improvement in hemodynamics after prosthetic annular stretching. We have also summarized other case series and case reports which have previously described similar techniques.<Learning objective: Transcatheter valve-in-valve procedure may not be feasible in certain patients who have a relatively smaller siz...

A case of pulmonary arterial hypertension with chronic hepatitis that resulted in hepatosplenomegaly after administration of prostaglandin I2
We report a rare case of PAH with chronic hepatitis C that resulted in hepatosplenomegaly after PGI2 administration. In cases of chronic liver disease, it is important to keep in mind that administration of PGI2 may result in hepatosplenomegaly.> (Source: Journal of Cardiology Cases)

Management of a patient presenting with giant cell myocarditis — A case report
We describe a patient who presented with congestive heart failure that required a left ventricular assist device and a temporary right ventricular assist device along with immunosuppressive therapy. This case also brings to attention how undocumented immigration status can limit transplantation opportunities in the USA.<Learning objective: Giant cell myocarditis is a rare and frequently fatal cardiac disease that, until the 1980s, was only diagnosed during autopsy. It is commonly seen in young and middle-aged, otherwise healthy, individuals with median transplant-free survival of 5.5 months from diagnosis. Current trea...

Laser Atherectomy to Treat Severe In Stent Restenosis of the Superior Mesenteric Artery
We present a case of chronic mesenteric ischemia in a 53-year-old man caused by severe in-stent restenosis who had abdominal pain and weight loss. We used a 0.9 mm laser catheter with low energy and pulse rate as an adjunct to balloon therapy. After revascularization, the patient’s symptoms improved dramatically. To the best of our knowledge, this is the first published case in which atherectomy with laser was used to treat severe in-stent restenosis of a mesenteric vessel in chronic mesenteric ischemia.Learning ObjectiveThe treatment of severe in-stent restenosis of the superior mesenteric artery using laser atherectomy...
