Modernizing Cardiovascular Medicine Board Certification: Leveraging Innovation and Strengthening Continuous Competency and Professional Growth
Achieving and maintaining clinical competence in the practice of cardiovascular medicine is fundamental to providing appropriate, patient-centric, high-value health care and is part of the ethical and professional duties of all physicians in their responsibility to patients and the public. HFSA at the 10th Anniversary Meeting of the Heart Failure Association of India – New Partnerships and Opportunities Overseas
This past February two landmarks were celebrated in cardiovascular medicine. In the United States, we commemorated the 60th year that Heart Month has been observed, while across the globe in India, the Heart Failure Association of India (HFAI) celebrated its 10th anniversary. The HFAI is comprised of experienced physicians and nurses spanning the entire Indian subcontinent where 10 million people are (under)estimated to live with heart failure1. The collective HFAI mission is to reduce HF-related mortality through the building of optimal medical therapy clinics, raising awareness among general physicians, training nursing ...

President's Page in JCF: HFSA and the World
The Heart Failure Society of America is actively engaged across the globe in efforts to reduce the burden of heart failure. HFSA does this through a range of initiatives on education, research and advocacy. (Source: Journal of Cardiac Failure)

Hospice in Heart Failure is Visible, Yet Still Out of Reach for Many
Palliative care is included in the latest heart failure guidelines in Europe, the United States, and Japan.1-3 All heart failure guidelines define palliative care as a multidisciplinary approach aimed at relieving the physical, psychological, and spiritual distress of patients and caregivers. They emphasize the importance of palliative care in all stages of heart failure with integration early in the trajectory of illness. They also agree that palliative care should include symptom management, communication about prognosis and life-sustaining therapies, and advance care planning (ACP). Growth Differentiation Factor-15 and Clinical Outcomes in Patients With Chronic Heart Failure
Heart failure (HF) is associated with increased mortality and hospitalization rates. Biomarkers have an important role in the early diagnosis of HF as well as in prognosis assessment and the monitoring of responses to treatment. Natriuretic peptides are the gold-standard biomarkers in HF. (Source: Journal of Cardiac Failure)

Functional mitral regurgitation and heart failure with preserved ejection fraction: clinical implications and management
Common pathophysiological mechanisms are observed in HFpEF and functional MR, such as diastolic dysfunction, preserved EF, atrial dilation and dysfunction, and annular remodeling. However, less is known about the prevalence and clinical value of functional MR in the context of HFpEF. The true prevalence of FMR has been overlooked for decades until a few years ago, and this has led to a significant underestimation of the diagnosis. This review aims to provide an overview of the known association between MR and HFpEF, discuss underlying mechanisms common to these diseases, and summarize potential targeted pharmacological tre...

Assessment of temporary warfarin reversal in patients with left ventricular assist devices: the KVAD study
Due to advances in technology and limited availability of donor hearts, thousands of patients with advanced heart failure are supported with left ventricular assist devices (LVAD) either as a bridge-to-transplant, bridge-to-candidacy, or as destination therapy for those ineligible for a heart transplant.1 –3 The survival profile continues to improve, with 1 and 5 year survival rates of 83% and 52% respectively, allowing patients supported on these devices to live for longer periods of time.3–5 (Source: Journal of Cardiac Failure)

Guide to Lung-Protective Ventilation in Cardiac Patients
The incidence of acute respiratory insufficiency has continued to increase among patients admitted to modern-day cardiovascular intensive care units. Positive pressure ventilation (PPV) remains the mainstay of treatment for these patients. Alterations in intrathoracic pressure during PPV has distinct effects on both the right and left ventricles, affecting cardiovascular performance. Lung-protective ventilation (LPV) minimizes the risk of further lung injury through ventilator-induced lung injury and, hence, an understanding of LPV and its cardiopulmonary interactions is beneficial for cardiologists. Pulmonary Artery Diastolic Pressure as a Surrogate for Pulmonary Capillary Wedge Pressure in Cardiogenic Shock
Pulmonary artery catheter (PAC) derived invasive hemodynamics are used to guide care in approximately one out of every ten cardiac intensive care unit (CICU) admissions, and one in three patients with cardiogenic shock (CS)1. In the day-to-day care of those with indwelling PACs, it is common for clinicians to use the pulmonary artery diastolic pressure (PADP) as a surrogate for the pulmonary capillary wedge pressure (PCWP). The value of this approach is that it can be both timesaving and potentially safer given that the PADP measurement is continuously available and doesn't require PAC balloon inflation. Rates of Reproductive Counseling and Contraception Use in Patients with Heart Failure at a Tertiary Care Center
Increasing numbers of women of childbearing age have cardiac disease including heart failure. In these women, pregnancy can cause significant morbidity and mortality. Contraceptive use and pregnancy counseling in women with heart failure is an essential part of their medical care. Here, we assess contraceptive use and pregnancy counseling of patients with heart failure at a single tertiary care center. (Source: Journal of Cardiac Failure)
