Cauterization-mediated restriction from penetrating orbital trauma
A healthy 32-year-old woman presented with binocular diplopia immediately after sustaining a penetrating injury to the left periocular adnexa with a hot metal skewer. Examination revealed an incomitant esotropia, with complete limitation of abduction of the left eye with downshoot in left gaze and normal afferent visual function. Computed tomography and magnetic resonance imaging demonstrated no fracture, but there was mild thickening of the medial rectus muscle and associated fat stranding. Lack of orbitomuscular tethering or hematoma led to the presumptive diagnosis of thermal cauterization injury causing left medial rec...

Concomitant optic disk drusen and papilledema due to idiopathic intracranial hypertension in a pediatric cohort
Optic disk drusen (ODD) in pediatric patients typically presents with pseudopapilledema. Diagnosing concomitant papilledema due to idiopathic intracranial hypertension (IIH) in these patients can be challenging. The purpose of this study was to evaluate the incidence and clinical features of papilledema due to IIH among pediatric patients with a new diagnosis of ODD and to discuss the clinical and paraclinical findings that helped diagnose this group. Seeing Is Believing
At first, the other kids in the class – those who hadn’t been brave enough to stand up to Big Literacy – were envious of the freedom that allowed us to doodle at our desks during reading time. But after a few days, something began to shift. We began to get bored. And all the other kids started to be able to do something we couldn’ t. They were looking at letters but seeing words. Suddenly they had a superpower, and we didn’t. My days of academic protest were over. (Source: Journal of AAPOS)

An Eye on the Arts —the Arts on the Eye
He begins, “The first time I saw the place where clouds are formed was from the window of a train…”Another time was in a mirage in the heat outside Tucson.Once he thought he saw it in the dry light of stars. The place he remembers best was when he saw it in the eyes of a woman he spoke to.When he first no ticed it she hid it by lowering her gaze.Soon she let him look freely. There were times when she opened her eyes wide allowing an unobscured view.Sometimes he saw her eyes smolder with dryness on a summer day. Blue donut technique for staining the trabecular meshwork in young phakic patients
Angle-based procedures, such as gonioscopy-assisted transluminal trabeculotomy (GATT), can be particularly challenging in patients with little to no trabecular meshwork (TM) pigmentation, a situation frequently encountered in pediatric patients. The TM can be nicely delineated with trypan blue, but the long-term effects of trypan blue on a clear crystalline lens are unknown. The blue donut technique allows staining of the TM without inadvertent staining of the lens and increases the concentration of the stain in the TM. (Source: Journal of AAPOS)

Resolution of cyclic esotropia with 5-year follow-up after brief Fresnel prism treatment
We report a case of cyclic esotropia in which the cycling resolved with 2 months of Fresnel prism for the amount of the distance deviation on her “straight” days. Five years later, with low plus hyperopic correction, she remains with a stable esophoria and normal stereopsis. (Source: Journal of AAPOS)

Double-bellied inferior rectus muscles with aplasia of medial rectus muscles
This report describes a rare occurrence of double-bellied inferior rectus muscle with aplasia of the medial rectus muscle in both eyes of a young woman seeking cosmetic correction for her strabismus. The patient presented with a large-angle exotropia and a −4 limitation of adduction in each eye. On attempted adduction, there was minimal downshoot and upper eyelid retraction. Magnetic resonance imaging of the orbit revealed hypoplastic medial rectus muscles; however, intraoperatively the muscle was not found at the insertion or when traced back as fa r as possible.
