Acute unilateral inner retinal dysfunction with photophobia: importance of electrodiagnosis
ConclusionsThe common features in the eight patients with unilateral negative ERGs suggest a new disease entity of unilateral acute inner retinal layer dysfunction. In most patients, the only subjective complain was photophobia. (Source: Japanese Journal of Ophthalmology)

Relationships between the vascular structure and neural function of the macula in patients with diabetes mellitus
ConclusionsThe OPs of the focal macular ERG are smaller with prolonged implicit times in association with capillary loss in the macula of diabetic patients. The implicit times are the most sensitive functional parameter that reflects the early changes of the microvasculature in the macula caused by diabetes. (Source: Japanese Journal of Ophthalmology)

Imaging of a retinal pigment epithelium aperture using polarization-sensitive optical coherence tomography
ConclusionThe RPE aperture was characterized as a round discontinuity of depolarization. The findings with PS-OCT suggest atrophic changes in the overlying RPE of the PED. The degree of depolarization was associated with retinal sensitivity. The current results indicate that RPE apertures developed within the spectrum of atrophic AMD. Endovascular surgery in the field of ophthalmology
AbstractIn this article, we provide an overview of the current perspectives on endovascular surgery in ophthalmology, including a description of the various approaches, recent clinical results and future prospects. Experimental studies of endovascular surgery in ophthalmology started in the 1980s; since then, a considerable amount of research has been done to develop the procedure for clinical use. During the past two decades endovascular surgery has been performed on eyes with retinal vascular disorders, including central retinal vein occlusion and central retinal artery occlusion. The first endovascular surgery on human ...

Four-year outcome of aflibercept administration using a treat-and-extend regimen in eyes with recurrent neovascular age-related macular degeneration
ConclusionIn clinical settings, aflibercept treatment using a TAE regimen may successfully maintain visual acuity for up to 4  years even in recurrent cases of AMD. (Source: Japanese Journal of Ophthalmology)

Effectiveness and limitations of minimally invasive glaucoma surgery targeting Schlemm ’s canal
AbstractGlaucoma surgery is performed to lower intraocular pressure (IOP); ideally, the IOP reduction is safely maintained for an extended period of time. Although trabeculectomy was considered the gold standard for glaucoma surgery for many years because of its effective IOP reduction, yet now it is considered unsafe because of serious complications. In recent years, minimally invasive glaucoma surgery (MIGS), which emphasizes safety and can be performed rapidly, has become widespread. Because MIGS does not involve conjunctival incisions, patients can undergo future trabeculectomy. If IOP reduction can be maintained safel...

Recent trends in vitreoretinal surgery: a nationwide database study in Japan, 2010 –2017
ConclusionThe total number of cases of vitreoretinal surgery per fiscal year increased throughout the period. The increases in ERM and RRD with PPV and the decreases in DR and RRD with SB were remarkable. (Source: Japanese Journal of Ophthalmology)

Clinical characteristics and efficacy of methotrexate in Japanese patients with noninfectious scleritis
ConclusionOur study suggests the significance of prompt initiation of corticosteroid-sparing immunosuppressive treatment in treating patients with refractory scleritis or those intolerant of systemic corticosteroids. Moreover, MTX may be used effectively and safely for the treatment of noninfectious scleritis in Japanese patients. (Source: Japanese Journal of Ophthalmology)

Retrospective analysis of paediatric glaucoma at a tertiary referral centre in Hong Kong
ConclusionMost cases of childhood glaucoma in Hong Kong are secondary rather than primary, similar to other regions with low incidence of parental consanguinity. The large proportion of secondary glaucoma which generally has poorer prognosis, is reflected by the frequency of multiple surgery, and a high incidence of visual deterioration despite significant intraocular pressure reduction in most eyes. Reduced steroid-induced intraocular pressure elevation in tacrolimus-treated refractory allergic ocular diseases
ConclusionsTopical tacrolimus may lessen the steroid-induced elevation in IOP in younger individuals and may be a good adjunctive therapy to avoid IOP elevation in refractory cases. (Source: Japanese Journal of Ophthalmology)

Hypothetical pathogenesis of age-related macular degeneration and pachychoroid diseases derived from their genetic characteristics
AbstractGenetic studies have investigated the pathogenesis of age-related macular degeneration (AMD). The pachychoroid concept has recently garnered attention as a possible explanation for AMD pathogenesis; the genetic characteristics of pachychoroid diseases have also been elucidated. In this review, we summarize previously reported genetic characteristics of AMD and pachychoroid diseases, and analyze these data to understand the pathogenesis of AMD and pachychoroid diseases. Previous studies show thatVIPR2 and theCFH I62V A allele promote development of pachychoroid and central serous chorioretinopathy (CSC), while theCF...
