Reconsidering the Diagnostic Construct Validity of Depressive Disorders for Young Children
Depression is a leading cause of global disease burden and premature death, with early age at depression onset portending poorly for a host of negative outcomes. Children who experience depression are at risk of having a more severe course of illness, including a greater number of and more severe episodes, increased risk of suicide attempts, greater number of hospitalizations, more medical and psychiatric comorbidity, and greater impairment in social, cognitive, and occupational functioning, compared with onset in adolescence or adulthood. The treatment costs for childhood mental disorders in the US are estimated to be $10...

Reexamining Quality Assessment and Effect Size Reporting Among Studies Showing Major Changes in Depressive Symptoms —Reply
In Reply In a letter regarding our systematic review and meta-analysis on COVID-19 –related child and adolescent changes in depression symptoms where we reported a standardized mean change (SMC) of 0.26 (95% CI, 0.19-0.33), Daly asserts that (1) inclusion of 6 of the 7 largest effect sizes “may have led to an exaggerated estimate of change in depressive symptoms,” (2) the fi ndings are “at odds with the broader literature,” and (3) the inclusion of conference abstracts is suspect given the “limited information on which to base a quality assessment.” We address each point below. Reexamining Quality Assessment and Effect Size Reporting Among Studies Showing Major Changes in Depressive Symptoms
To the Editor In a systematic review and meta-analysis of 53 longitudinal cohort studies, Madigan and colleagues concluded that depressive symptoms increased notably among children and adolescents during the COVID-19 pandemic (standardized mean change [SMC]  = 0.26). Seven studies included in the meta-analysis reported major changes in depressive symptoms (≥0.6 SDs, considered indicative of moderate to large changes). These studies are at odds with the broader literature, and their inclusion may have led to an exaggerated estimate of change in de pressive symptoms. For this reason, I reexamined these studies and foun...
