American Academy of Ophthalmology Clinical Practice Guidelines and Financial Disclosures —Where Is the Money Going?—Reply
In Reply We want to thank Tinsley et al for directing the JAMA Ophthalmology readership toward considerations when assessing conflicts of interest among physicians. First, regarding physician gender, self-reported identification was not used in our article “Financial Disclosures Reported by Industry Among Authors of the American Academy of Ophthalmology Clinical Practice Guidelines.” Gender was determined using Gender API, an application program interface. Despite its limitations (gender not considered as a spectrum and not self-reported genders), Gender API is widely used in research due to its high accuracy. In our s...

American Academy of Ophthalmology Clinical Practice Guidelines and Financial Disclosures —Where Is the Money Going?
To the Editor We would like to commend Nguyen et al on their excellent study investigating the financial disclosure practices among male and female physicians. The authors use rigorous methodology in their acquisition, analysis, and interpretation of their data. However, we feel that certain study limitations could have been overcome or mitigated with additional considerations. (Source: JAMA Ophthalmology)

Beyond Accessibility in Exploring Access to Eye Care to Achieve Vision Health Equity
Shaffer et al used the Medicare part B claims, a large data set from the Centers for Medicare and Medicaid, to determine if expanding laser eye surgery privileges to optometrists was associated with improved patient access to laser procedures. Shaffer et al reported that the expansion of optometrists ’ ability to perform laser eye surgery procedures did not provide shorter drive times for patients when receiving laser eye surgeries. Distance and drive time are concerns for patients who have limited access to care, especially patients from low-resource communities. However, beyond accessibility , the theory of access fram...

Which Optical Coherence Tomography Parameter, If Any, Identifies Glaucoma in High Myopia?
Patients with high myopia present a diagnostic dilemma to clinicians. Although they may be at higher risk of glaucoma, detecting coprevalent glaucoma can be challenging due to the characteristically small optic nerve heads, which can be very tilted, rotated, and even anomalous in appearance. Such myopic optic nerve head morphology makes it difficult to appreciate optic nerve head cupping or other classic signs of glaucomatous damage such as optic nerve rim thinning or notching. On the other hand, findings on structural and functional testing in individuals with high myopia can masquerade as glaucoma, leading to overdiagnos...

Femtosecond Laser-Assisted Cataract Surgery —Conclusive Evidence of Lack of Incremental Value
B énard and colleagues report a detailed cost-utility study of femtosecond laser-assisted cataract surgery (FLACS) in comparison with standard phacoemulsification cataract surgery (PCS) and did not find FLACS to be cost-effective. FLACS was introduced into cataract surgical practice about 12 years ag o with much excitement and anticipation. Here was an elegant procedure with the promise of greater precision and enhanced clinical outcomes compared with conventional ultrasound phacoemulsification for cataract surgery, the most performed operation. For many years, achievement of that promise was cl aimed based on small studi...
