Evolving Trends in Immunohistochemistry Use in Dermatopathology
In this issue of JAMA Dermatology, Ojukwu and colleagues analyzed the use of immunohistochemistry (IHC) in melanoma specimens within the Surveillance, Epidemiology, and End Results –Medicare cancer registry database over 18 years. The authors were able to identify 132 547 distinct melanoma biopsies in 116 117 individual patients 65 years or older. Ojukwu et al found that the use of IHC staining increased among all specimen types. In particular, the use of IHC in melanoma cases increased from 11% in 2000 to 51% in 2017, with a statistically significant increase noted in association with all demographic and tumor chara...

Improving Outcomes in Chronic Graft-vs-Host Disease
Chronic graft-vs-host disease (cGVHD) is a frequent, heterogeneous, and often life-threatening complication of allogeneic hematopoietic stem cell transplantation. A total of 25% to 50% of all transplant survivors develop cGVHD, and cutaneous manifestations are seen in approximately 75% of these patients. Cutaneous manifestations of cGVHD include skin fibrosis (sclerotic cGVHD), epidermal involvement resembling lichen planus, or a combination of both findings. Alopecia, genital inflammation/scarring, nail dystrophy, pigmentary changes, ulceration, and poor wound healing are additional features. Associated skin changes range...

Cooling Research Fever in Dermatology —The Role of Community Service—Reply
In Reply We appreciate the thoughtful response by Zhang et al to our article and agree with their points. Community service is extremely relevant to dermatology and should be highlighted as a way for trainees to explore dermatology, serve and learn about diverse communities and their needs, and improve the field and our care of patients. As more and more residency programs implement mission-driven holistic review, we imagine that many will add community service as a specific target for student engagement. Reinforcing and sustaining this will take collaboration from educational leaders and others across the field of dermato...

Cooling Research Fever in Dermatology —The Role of Community Service
To the Editor We read with interest the recent Viewpoint by Gandelman et al that highlighted research fever in dermatology, which refers to the field ’s growing overemphasis on research output and the concurrent rise in the average number of abstracts, presentations, and publications of applicants matched into dermatology. We agree with the authors that research fever is a complex, multifactorial problem for which there may not be a simple, sin gular quick fix. However, one solution mentioned in mere passing, but which we believe deserves greater consideration and emphasis, is increasing the importance of community servi...

Matching Drug Prices to Their Clinical Benefit —The Final Frontier?
In this issue of JAMA Dermatology, Egilman et al show how efficiency frontiers (EFs) can be used to align the prices and clinical benefits of biologics for plaque psoriasis. This is particularly timely given that the Inflation Reduction Act of 2022 granted Medicare (the federal health insurance program for those 65 years and older) the right to negotiate drug prices (on the basis of comparative safety, effectiveness, and development costs and subsidies) for the first time. Negotiation is limited to the top 10 highest-spend drugs initially but will be expanded. In addition, as well as limits on above-inflation price increas...
