Giant Cell Tumor of Bone With Pseudosarcomatous Changes Leading to Premature Denosumab Therapy Interruption: A Case Report With Review of the Literature
We report a case of premature denosumab interruption due to radiological and clinical tumor expansion of a giant cell tumor of the distal ulna. Although denosumab is known to induce tumor regression, with progressive ossification and loss of the characteristic morphology of giant cell tumor of bone, the ulnar tumor specimen showed a moderately to highly cellular proliferation of short spindle-shaped cells, and no osteoclast-like giant cells. There were no abnormal mitotic figures. We considered the surgical specimen as a giant cell tumor of bone with partial regression after prematurely interrupted denosumab treatment. Thi...

Ovarian Yolk Sac Tumor With High-Grade Serous Carcinoma in a 62-Year-Old Woman
We present a case of mixed ovarian yolk sac tumor and high-grade serous carcinoma in a postmenopausal woman, including a brief discussion of the immunohistochemical findings and differential diagnosis. Despite the rarity of mixed ovarian yolk sac tumor and surface epithelial tumors, it is important to recognize the biphasic nature of the tumor, which should prompt a thorough immunohistochemical evaluation. The therapeutic and prognostic implications of proper diagnosis cannot be overemphasized. (Source: International Journal of Surgical Pathology)

Urothelial Carcinoma With Villoglandular Differentiation: A Rare Entity
We describe a rare case of urothelial neoplasm with villoglandular differentiation along with its immunohistochemical profile arising in an elderly male. Its poor prognosis signifies its need to be recognized. (Source: International Journal of Surgical Pathology)

Fibroadenoma of the Breast With Xanthomatous Features: Report of a Case
Xanthomas are common in cutaneous sites but may also be seen in unusual locations. In view of the paucity of reported cases, the occurrence of such lesions within the breast stroma would appear to be either unusual, underreported, or ignored. Indeed, most reports of the few breast xanthomas and of the even fewer xanthomatous fibroadenomas reported have appeared in the older literature. Herein, a case of fibroadenoma with multiple foci of xanthoma is reported, the literature is briefly reviewed, and the apparent difference between the previous cases and the current case is highlighted. Tubular Pyloric Gland Adenoma of the Left and Right Hepatic Ducts: Report of a Unique Case With Immunohistochemical and Molecular Studies
We report a case of a tubular PGA that led to biliary obstruction. The tumor was located at the confluence of the right and left hepatic ducts, extending to the left hepatic duct. The tumor cells expressed MUC6 and MUC5AC. MUC1 and CD10 were focally positive. MUC2, p53, and CDX2 were not expressed. The Ki67 positivity was estimated at <15%. None of the KRAS, NRAS, BRAF, EGFR coding regions resulted in clinically relevant amino acid substitutions. SNP rs1050171 (EGFR p.Q787Q, silent mutation) corresponding to c.2361G>A transition in exon 20 was noticed. Awareness of this rare lesion is important for pathologists and c...

An Unusual Case of Noninvasive Adenocarcinoma Arising in a Localized Adenomyoma of the Gallbladder and Review of Literature
We report a case of a 58-year-old asymptomatic woman found to have an incidental 3.3 x 3.2 x 2.3 cm well-circumscribed exophytic mass on surveillance ultrasound, originating from the fundus of the gallbladder and abutting the liver capsule. The patient underwent an open cholecystectomy with resection of the mass and underlying segment of the liver. The mass was discrete and well-circumscribed with a peripheral pseudocapsule. Histologically, it showed features typical of a benign adenomyoma with variably sized distended microcysts within fibromuscular stroma. Within the lesion, however, multiple small foci amounting to appr...

Composite Mucoepidermoid Carcinoma and Columnar Cell Variant of Papillary Carcinoma of the Thyroid: A Case Report and Review of the Literature
We report the first case of composite MEC and columnar cell variant of PCT. An 86-year-old man consulted for a 47-mm thyroid nodule, cytologically compatible with PCT. Total thyroidectomy was performed. Histological diagnosis, with support of histochemistry and immunohistochemistry, was mixed primary carcinoma of the thyroid, associating an MEC component with predominantly columnar cell variant of PCT. Sixteen months after surgery and external beam radiation therapy, the patient was free of recurrence or distant metastasis. This case report offers an opportunity to highlight the potential pitfalls concerning the interpreta...

Plasmacytoid-Type Cellular Differentiation in Polymorphous Low-Grade Adenocarcinoma
Conclusion. Although PD in PLGA is rare, it is necessary to be aware of this possibility, particularly in small incisional biopsies and in PLGA with limited invasion, to avoid confusion with cellular PA. (Source: International Journal of Surgical Pathology)

Assessment of CD-105 as an Angiogenic Modulator in Odontogenic Myxomas and Dental Follicles
Conclusion. A weaker expression of CD-105 in odontogenic myxoma might indicate a lower angiogenic activity, suggesting that vascular proliferation has a limited role in the growth mechanisms and in the aggressive behavior of this neoplasm. (Source: International Journal of Surgical Pathology)

Squamous Dysplasia of the Urinary Bladder: A Consecutive Cystectomy Series
Squamous dysplasia of the urinary bladder is uncommon and may represent a precursor to invasive squamous cell carcinoma. Though significant focus has been devoted to squamous differentiation in invasive bladder cancer, relatively little attention has been given to squamous dysplasia. We methodically reviewed microscopic slides from a consecutive cystectomy series at our institution (n = 303; 2001-2014), with special attention given to squamous dysplasia and squamous differentiation within association invasive carcinoma. Of these 303 cases, 3% (9 cases) had squamous dysplasia. The majority (89%; 8/9) had a similar morpholog...
