Inspect, respect, incise: A novel non-resection repair technique for degenerative mitral regurgitation
Aim: Judging the optimal amount of leaflet resection is difficult when used alongside non-resectional repair in degenerative mitral regurgitation (MR), as it risks iatrogenic complications and repair failure. Our "pseudo-resection" incisional technique, serves the purpose of resection whilst still preserving native tissue. We aim to evaluate the feasibility, reproducibility, applicability to both valve leaflets and; long-term symptomatic, functional and survival outcomes of our technique. Is mitral valve repair superior to mitral valve replacement in patients with native mitral valve endocarditis? A systematic review and meta-analysis
The objective of this study is to systematically review the rate of morbidity and mortality associated with mitral valve repair or replacement in infective endocarditis. (Source: International Journal of Surgery)

Innominate vs Axillary artery cannulation in thoracic aortic surgery: A systematic review and meta-analysis
Aim: To investigate whether axillary artery cannulation has supremacy over innominate artery cannulation during hypothermic circulatory arrest during thoracic aortic surgery. (Source: International Journal of Surgery)

A large ascending aortic aneurysm secondary to idiopathic necrotising aortitis - A rare but important cause of thoracic aortic disease
Aneurysms of the ascending aorta are rare and their underlying aetiologies manifold. In the elderly they are typically associated with atherosclerotic processes, whilst in younger patients, connective tissue disorders predominate. (Source: International Journal of Surgery)

Quality of referrals to a tertiary cardiothoracic unit; How one simple intervention has made a drastic improvement
Aim: On commencing the lung cancer diagnosis pathway, a contrast CT scan is needed. PET-CT scan should be conducted and head imaging considered, on patients potentially requiring radical treatment. Lack of critical information on a referral to our Unit contributed to a Serious Untoward Incident(SUI), therefore, referral content and quality were investigated. Diabetic patients undergoing coronary bypass surgery: Are we prescribing their medications appropriately?
Aim: Our aim was to assess our department's care of diabetic patients undergoing coronary artery bypass surgery (CABG) and measure adherence to guidelines on prescribing diabetic medication peri-operatively. (Source: International Journal of Surgery)

Inpatient wait for urgent cardiac surgery: The patients ’ perspective
Aim: Procedures designated ‘urgent’, wherein patients remain in-hospital between diagnosis of high-risk pathology and definitive treatment, account for up to a third of the cardiac surgical workload in the UK. It is not known to what extent this inpatient wait is justified. We sought to evaluate the patient experience and perspective on this complex, multi-faceted issue. Diagnostic value of the ‘soccer ball’ morphology in the marfan aortic root
Aim: The aneurysmal root in Marfan Syndrome (MFS) is typically ‘soccer ball’ shaped, in contrast to the ‘rugby ball’ seen with other aetiology. We sought to evaluate the predictive accuracy of this morphologic description. (Source: International Journal of Surgery)

A Baker ’s cyst causing popliteal artery occlusion: Not just your usual case of intermittent claudication…
A 75-year-old gentleman presented to vascular outpatient clinic with progressive short distance claudication. Examination elicited only a palpable femoral pulse in the affected leg. All pulses were palpable in the contralateral leg. CT angiography and MRI revealed a short popliteal artery occlusion, intimately related to a 4x4x3cm cystic swelling in an otherwise healthy vessel. No other significant peripheral arterial disease was identified. The differential diagnosis was cystic adventitial disease and the patient was listed for excision of the occluded popliteal artery, followed by a venous interposition graft using a pos...

Shot through the heart! (But not to blame...)
Aim: Intracardiac foreign body (FB) impalement is rare. Sequelae depend on the FB intrathoracic trajectory and if survivable, presentation varies. The scarcity of such incidents precludes standardised protocol therefore operative intervention, which is invariably required, must be individualised. (Source: International Journal of Surgery)

Pneumonia prevents the need for lung volume reduction surgery
We describe two patients with chronic obstructive pulmonary disease who developed pneumonia while awaiting targeted surgical hyperinflation intervention. Post-pneumonic lung function was associated with improved respiratory function that would not have been seen with surgery alone. (Source: International Journal of Surgery)

De Garengeot's Hernia: A surgical challenge- Case report and literature review
Case Presentation: A 64-year-old female presented with a painful groin lump. CT demonstrated a strangulated right femoral hernia, however the appendix was not reported on.The patient underwent an emergency open hernia repair using a modified McEvedy approach through a previous TRAM (transverse rectus abdominis myocutaneous) flap scar. A gangrenous appendix was found within the hernia sac -known as de Garengeot's hernia. The appendix was removed, non-viable small bowel resected followed by hand-sewn anastomosis and suture repair of the femoral defect. (Source: International Journal of Surgery)
