Failure Rate in Pediatric Cochlear Implantation and Hearing Results Following Revision Surgery
In a discussion of the risks and benefits of pediatric cochlear implantation, device failure and the need for revision surgery is often overlooked. The failure rate has not been investigated extensively for this population of patients. Hearing results are under-reported following revision surgery as well. We will review our experience with cochlear implant failure, revision, and hearing results when available to better guide the preoperative counseling of families considering cochlear implantation. Dichotic training in children with auditory processing disorder
Several test batteries have been suggested for auditory processing disorder (APD) diagnosis. One of the important tests is dichotic listening tests. Significant ear asymmetry (usually right ear advantage) can be indicative of (APD). Two main trainings have been suggested for dichotic listening disorders: Differential Interaural Intensity Difference (DIID) and Dichotic Offset Training (DOT). Congenital Complete Absence Of Tracheal Rings With Trifurcate Carina: Case Report Of A Rare Clinical And Endoscopic Presentation
We describe the case of a child with isolated absence of cartilaginous tracheal rings and a trifurcate carina. At 6 months of age, the patient presented to our multidisciplinary airway clinic with stridor and recurrent severe respiratory infections requiring hospitalization. Radiographs showed airway narrowing. Exam demonstrated biphasic stridor. Flexible fiberoptic laryngoscopy demonstrated only mild laryngomalacia. Central auditory processing disorder (APD) is defined as a deficit in auditory performances such as sound localization and lateralization, auditory temporal processing, auditory discrimination, auditory performance in presence of noise, or comprehension of degraded signals (1). Intracranial complications of acute sinusitis in children: The role of endoscopic sinus surgery
Up to 24% of hospitalized patients with acute rhinosinusitis have an intracranial complication including meningitis, cavernous or sagittal sinus thrombosis, intraparenchymal and epidural abscesses, or subdural empyema [1,2]. In pediatric admissions, there is a 3% incidence of intracranial complications from acute rhinosinusitis [3]. Mortality rates range from 0% to 7%. Prolonged complications have been described in 10 –25% of patients, more commonly neurologic sequelae such as seizures, followed by dysphasia and focal paresis [4–6]. Pediatric septoplasty and functional septorhinoplasty: A quality of life outcome study
This study assessed disease-specific quality-of-life outcomes among pediatric patients undergoing septoplasty or functional septorhinoplasty. The Nasal Obstruction Symptom Evaluation (NOSE) score was obtained pre- and post-operatively. Additional analyses determined whether demographics, nasal trauma, prior nasal surgery, or allergic rhinitis history affected NOSE scores. Tympanostomy Tube Placement and Ear Drops: Evidence-Based Cost Saving Models
Tympanostomy tube placement is the most common surgical procedure in the pediatric population with important financial implications to our healthcare institutions. Nearly 700,000 cases are performed in the United States per year on patients under 15 years of age[1]. Otorrhea can occur after tube insertion and both intra- and post-operative instillation of antimicrobial otic drops is common practice. Infection of the spheno-occipital synchondrosis: A morbid complication following adenoidectomy
Two 2-year-old males presented post-operatively following adenoidectomy with persistent fever and neck stiffness. After multiple office visits, both patients were admitted and found to have a widened spheno-occipital synchondrosis and other imaging findings indicative of skull base osteomyelitis. Treatment with antibiotics allowed for recovery with good long-term outcomes. Infection involving the spheno-occiptal synchondrosis is rare and its circuitous presentation of these two children no doubt led to delayed diagnosis. Idiopathic cervical tracheal stenosis in an 11-year-old male
We report on an 11-year-old male who developed progressive dyspnea and stridor. Bronchoscopy revealed 90% stenosis of the cervical trachea. He underwent serial balloon dilation with steroid injection; stenosis decreased to 20%. He had no history of intubation or trauma. Tonsillar antimicrobial peptide (AMP) expression profiles of periodic fever, aphthous stomatitis, pharyngitis, cervical adenitis (PFAPA) patients
PFAPA (periodic fever, aphthous stomatitis, pharyngitis and cervical adenitis) is the most frequent non-infectious cause of high fever observed among the European child population.
