Symptoms at presentation to the emergency department: Predicting outcomes and changing clinical practice?
ConclusionThe number of symptoms did not correlate with the main adverse outcomes (ICU‐admission and in‐hospital mortality). However, clinical practice was influenced by the nature and number of symptoms. This was shown by associations with hospitalisation, length‐of‐stay, use of resources and follow‐up presentations. Furthermore, the number of symptoms correlated with the caregivers’ disease severity ratings and the attributed triage categories. This may indicate that caregivers respond to the number of symptoms by a higher investment in their immediate work‐ups and later follow‐ups. (Source: International...

Effects of exenatide twice daily, exenatide once weekly or insulin in patients with type 2 diabetes and baseline HbA1c ≥10.0%: Two pooled analyses including 20 randomised controlled trials
ConclusionsFor patients with high HbA1c, treatment with exenatide or insulin both improved glycaemic control. Given the associated weight loss and low risk of hypoglycaemia, exenatide may be a suitable alternative to treatment with insulin in certain patients with T2D and high HbA1c. Deutetrabenazine for tardive dyskinesia: A systematic review of the efficacy and safety profile for this newly approved novel medication —What is the number needed to treat, number needed to harm and likelihood to be helped or harmed?
ConclusionsDeutetrabenazine is the second FDA‐approved agent specifically indicated for the treatment of TD. Head‐to‐head comparisons with other VMAT2 inhibitors among patients with TD in the “real world” are needed. (Source: International Journal of Clinical Practice)

Pro ‐protein subtilisin kexin‐9 (PCSK9) inhibition in practice: lipid clinic experience in 2 contrasting UK centres
ConclusionsThis study suggests that PCSK‐9 inhibitors are effective but that prescriptions should not be changed to long‐term delivery until patients have been reviewed and shown to be adherent. (Source: International Journal of Clinical Practice)

A practical approach to the management of nocturia
ConclusionsThe cause(s) of nocturia should be first evaluated in all patients. Afterwards, the underlying pathophysiology should be treated specifically, alone with lifestyle interventions or in combination with drugs or (prostate) surgery. (Source: International Journal of Clinical Practice)

Prospective cohort study of risk factors for extended ‐spectrum ß‐lactamase‐producing Escherichia coli urinary tract infections in elderly patients admitted to hospital
Summary BackgroundExtended‐spectrum beta‐lactamase (ESBL)‐producing Escherichia coli are currently common in community‐onset infections, limiting therapeutic options. In this work we aim to identify the prevalence of and risk factors for ESBL‐producing E. coli in elderly patients with urinary tract infections (UTI) admitted to hospital. MethodsProspective cohort study on elderly patients with E. coli UTI admitted to a university hospital in Spain, from January 2013 to December 2015. Clinical features, microbiology and outcomes were recorded from the electronic medical records and reviewed by two researchers. Ca...

Fourteen ‐day high‐dose esomeprazole, amoxicillin and metronidazole as third‐line treatment for Helicobacter pylori infection
ConclusionCure rates of this 14‐day schedule using high‐dose esomeprazole, amoxicillin and metronidazole as a third‐line eradication regimen were suboptimal, especially in patients who had received metronidazole in previous failed eradication regimens. The prevalence and preventability of potentially relevant drug ‐drug interactions in patients admitted for cardiovascular diseases: A cross‐sectional study
ConclusionsUse of drug interaction screening tools can be beneficial risk mitigation strategy for potentially relevant pDDIs in CVD patients. DDI screening software could be linked to the patient's laboratory results or clinical data regarding renal or liver function, as an approach to reinforce DDIs alert quality. The utility and feasibility of assessing sleep disruption in a men's health clinic using a mobile health platform device: A pilot study
ConclusionsThis pilot study demonstrated the feasibility and potential utility of an mHealth platform to assist clinicians, within a men's health clinic, in detecting sleep disturbances. Disrupted sleep was revealed in well over half of this sample of patients. As a result of the growing evidence linking poor sleep and sleep disorders (eg, OSA) to the conditions relevant to men's health (eg, erectile dysfunction, hypogonadism and BPH), further efforts beyond this pilot study are necessary to identify the aetiological processes underlying the association between specific disrupted sleep disorders and urological conditions. ...
