CT coronary angiography: Is it the new standard in coronary assessment for liver transplant candidates?
Cardiovascular events continue to impact post-liver transplant survival [1]. Although the etiologies of these cardiac events are multifactorial, the presence of significant coronary artery disease can contribute to a patient's ability to tolerate the multifactorial stressors and physiological insults that occur throughout the transplantat process [2]. In addition, cardiovascular death is predominant in the post-liver transplant population, with sudden cardiac death contributing to 1 out of 3 post-transplant cardiac mortalities [3]. New era in right ventricular imaging with AI: Challenges and horizons
The intricate anatomy and complex physiological functions of the right ventricle (RV) have historically positioned it as a challenging area within cardiovascular imaging and diagnostics. Traditional imaging modalities, while foundational to the early understanding and assessment of cardiac function, often fall short in accurately delineating the unique morphology and dynamic behavior. This discrepancy arises from the crescent-shaped structure, extensive trabeculations, and the pivotal role it plays in pulmonary circulation and systemic venous return, all of which demand more sophisticated imaging solutions [1,2]. Levosimendan in cardiac amyloidosis: Unveiling promising paths
Cardiac amyloidosis (CA) is a progressive infiltrative disease primarily caused by the accumulation of misfolded, cleaved, and aggregated proteins in the extracellular space of the myocardium. CA leads to heart failure as the most frequent clinical manifestation. In recent years, research have focused on identifying and developing disease-modifying targeted therapies for CA. These efforts have resulted in improved prognosis, particularly in the setting of transthyretin CA. However, there has been less scientific interest in investigating the treatment and prompt management of patients with CA hospitalized for acute heart f...

Use of apixaban in adults with congenital heart disease and atrial arrhythmias: The PROTECT-AR study
Adults with congenital heart disease (ACHD) and atrial arrhythmias (AA) face an increased risk of thromboembolic events. Limited data exist on the use of non-vitamin K oral anticoagulants for thromboprophylaxis in ACHD. We aimed to assess the effectiveness and safety of apixaban in ACHD patients with AA. Fixed-dose combination therapy in pulmonary arterial hypertension: Pros & cons
Quality of life of patients suffering from chronic diseases is inevitably conditioned by the number of pills taken during the day. To improve patients' tolerability, compliance and quality of life and reduce healthcare costs, pharmaceutical companies are focusing on the commercialization of fixed-dose combination (FDC) therapies. The last ESC/ERS guidelines for the treatment of pulmonary arterial hypertension (PAH) recommend initial dual combination therapy for newly diagnosed patients at low or intermediate mortality risk. Contemporary outcomes of surgical aortic valve replacement in patients referred for a transcatheter approach
The purpose of this study is to examine which patients referred to our structural valve clinic for potential transcatheter aortic valve replacement (TAVR) are receiving surgical aortic valve replacement (SAVR) whether due to unsuitable anatomy for TAVR versus other reasons. Predictors of adverse cardiac events of coronary myocardial bridging diagnosed with computed tomography angiography
Myocardial bridging (MB) is a frequent congenital anomaly of the epicardial coronary arteries commonly considered a benign condition. However, in some cases a complex interplay between anatomical, clinical and physiology factors may lead to adverse events, including sudden cardiac death. Coronary CT angiography (CCTA) emerged as the gold standard noninvasive imaging technique for the evaluation of MB. Aim of the study was to evaluate MB prevalence and anatomical features in a large population of patients who underwent CCTA for suspected CAD and to identify potential anatomical and clinical predictors of adverse cardiac eve...

Temporal progression of replacement and interstitial fibrosis in optimally managed dilated cardiomyopathy patients: A prospective study
Background: To prospectively examine the dynamic evolution of fibrotic processes within a one-year in patients with dilated cardiomyopathy (DCM). Methods: Between May 2019 and September 2020, 102 DCM patients (mean age 45.2  ± 11.8 years, EF 29.9 ± 11.6%) underwent cardiac magnetic resonance (CMR-1). After 13.9 ± 2.9 months, 92 of these patients underwent a follow-up CMR (CMR-2). Replacement fibrosis was assessed via late gadolinium enhancement (LGE), quantified in terms of LGE mass and extent. Inter stitial fibrosis was evaluated via T1-mapping and expressed as extracellular volume fraction (ECV). (Sour...

Comment on “Subclinical myocardial injury increases the risk of heart failure in patients with and without type 2 diabetes post-acute coronary syndrome”
In this study, 50 acute coronary syndrome (ACS) patients who underwent revascularization were prospectively enrolled, and the univariate analysis was used for the evaluation of the effect of subclinical myocardial injury (sMi) on heart failure risk after ACS. They found and concluded that the sMi increased the risk of heart failure after ACS, irrespective of diabetes status. However, we raise some statistical concerns which could change the conclusion of this study. (Source: International Journal of Cardiology)
