Mitigating imperfect data validity in administrative data PSIs: a method for estimating true adverse event rates
AbstractQuestionAre there ways to mitigate the challenges associated with imperfect data validity in Patient Safety Indicator (PSI) report cards?FindingsApplying a methodological framework on simulated PSI report card data, we compare the adjusted PSI rates of three hospitals with variable quality of data and coding. This framework combines (i) a measure of PSI rates using existing algorithms; (ii) a medical record review on a small random sample of charts to produce a measure of hospital-specific data validity and (iii) a simple Bayesian calculation to derive estimated true PSI rates. For example, the estimated true PSI r...

The value of federated learning during and post-COVID-19
AbstractFederated learning (FL) as a distributed machine learning (ML) technique has lately attracted increasing attention of healthcare stakeholders as FL is perceived as a promising decentralized approach to address data privacy and security concerns. The FL approach stores and maintains the privacy-sensitive data locally while allows multiple sites to train ML models collaboratively. We aim to describe the most recent real-world cases using the FL in both COVID-19 and non-COVID-19 scenarios and also highlight current limitations and practical challenges of FL. Time and predictors of time to treatment for aneurysmal subarachnoid haemorrhage (aSAH): a systematic review
ConclusionsThe pathway from onset to definitive treatment of patients with aSAH consists of multiple stages with multiple influencing factors. This review provides the first comprehensive understanding of extent and factors associated with time to treatment of aSAH. There is an opportunity to target modifiable factors to reduce time to treatment, but further research considering more factors are needed. (Source: International Journal for Quality in Health Care)

Developing a new flexible tool for handover
ConclusionImplementing a new handover tool —FAST—was challenging, but rewarding. Using PDSA methodology enabled continuous monitoring, oversight and adaptive corrections for better implementation of this new handover reporting tool. (Source: International Journal for Quality in Health Care)

Prioritization and management of calls from older people to GP out-of-hours services
ConclusionOlder people are in urgent need of medical services when they contact GPOOH service, which plays an effective and patient-centred gatekeeping role, particularly directing the oldest old to the appropriate level of care outside GP office hours. The promotion of GPOOH services should be enhanced to ensure older people understand their role in supporting community living. (Source: International Journal for Quality in Health Care)

Beyond tokenism in quality management policy and programming: moving from participation to meaningful involvement of people with HIV in New York State
ConclusionWe present a model that offers a paradigm for practical implementation of routine consumer involvement in QM programs that can be replicated in other health-care settings, both disease-specific and general, reflecting the priority of active participation of consumers in QM activities at all levels of the health system. Measuring the impact of delayed access to elective cholecystectomy through patient ’s cost-utility: an observational cohort study
ConclusionsThere is a need to focus on minimizing loss of quality of life for patients affected by delayed surgeries. Faced with equal delayed access to elective surgery, triage may need to prioritize older patients to maximize their health over their remaining life years. Healthcare utilization associated with adherence to antibiotics for abdominal surgeries in Japan: cross-sectional analysis of administrative database
ConclusionThe results revealed that unnecessary healthcare utilization was associated with failure to adhere to therapeutic guidelines for prophylactic antibiotic therapy in elective general surgeries. We concluded that the progress of reduction in length of hospitalization over the decade was successful. Notably, adherence to treatment duration was better than that was 10 years ago. In this decade, administrators in hospitals have attempted to reduce the duration of hospitalization by developing various clinical pathways for surgical procedures and quality indicators. However, 15  877 patients (8.7%) were prescribed ora...

Looking for the right balance between human and economic costs during COVID-19 outbreak
ConclusionsOur analysis shows that considering health and economic outcomes in the assessment of response to pandemics offers a bigger picture perspective of the outbreak and could allow policymakers and health managers to choose systemic, ‘personalized’ strategies, in case of a feared second epidemic wave. Healthcare handoffs among lay caregivers
ConclusionIt was anticipated that lay participants would be more willing to ask for help due to lack of subject matter expertise and trained participants would be more confident in completing tasks due to their superior subject matter expertise, but the opposite was true in both cases. It was also anticipated that lay and trained participants would be equally confident of the instructions given by their fellow caregiver, yet trained participants were less confident. The results from this study have impacts on the design of instructions (often by formal caregivers) for informal caregivers. (Source: International Journal for...

Measurement and monitoring patient safety in prehospital care: a systematic review
ConclusionsThere is no single method of measuring and monitoring safety in prehospital care. Arguably, most safety monitoring systems have evolved, rather than been designed. This leads to safety blind spots in which information is lacking, as well as to redundancy and duplication of effort. It is suggested that the findings from this systematic review, informed by the MMS framework, can provide a structure for critically thinking about how safety is being measured and monitored in prehospital care. This will support the design of a safety surveillance system that provides a comprehensive understanding of what is being don...
