A case of malignant phyllodes tumor that responded to pazopanib and developed pneumothorax
AbstractHere, we present a 59-year-old female with recurrent malignant phyllodes tumor with multiple lung and lymph node metastases who developed a pneumothorax after the administration of pazopanib. The patient received pazopanib as the second-line chemotherapy. After 2.5  months of the therapy, computed tomography (CT) showed a decrease in the sizes and cavitation of lung lesions; however, a left pneumothorax was newly observed. It was difficult to distinguish the pneumothorax by upright chest X-ray. Typical symptom or physical finding of pneumothorax, such as dysp nea, chest pain or decreased breath sound was not obser...

Complete response to pembrolizumab in a patient with recurrent and metastatic urothelial bladder carcinoma reflecting coexisting sarcomatoid subtype and glandular differentiation: a case report
AbstractIn advanced urothelial carcinoma (UC), approximately 20% of patients respond to pembrolizumab, an anti-programmed cell death-1 (PD-1) antibody. Herein, we reported a single case of UC showing coexistence of sarcomatoid subtype and glandular differentiation. Notably, only glandular differentiation was recurrent, probably progressive, and metastatic, which showed complete response to pembrolizumab. An 80-year-old woman presented with hematuria and dysuria, and an intra-vesical tumor was detected on ultrasound. Transurethral resections (TUR) were performed three times. In the first TUR, a sub-pedunculated tumor and a ...

Rapid-onset paraneoplastic cerebellar degeneration successfully treated by radiotherapy and tumorectomy
We report the first-ever documented case of successful treatment of paraneoplastic cerebellar degeneration (PCD) with radiotherapy. A 31-year-old female presented with rapidly progressing neurological symptoms, which were revealed to be due to PCD secondary to an undiagnosed breast cancer. The cancer responded well to chemotherapy, but her neurological status continued to deteriorate, eventually progressing to complete expressive aphasia and dyssynergia with paraparesis. Due to the extraordinarily rapid progression of the disorder, a treatment with tumorectomy and radiotherapy of the whole brain was performed. This proved ...

Solitary pulmonary capillary hemangioma mimicking a preinvasive malignant lesion in an asymptomatic middle-aged female patient
AbstractPulmonary capillary hemangiomatosis (PCH) is a rare disease characterized by a proliferation of capillaries in the alveolar septa, bronchial and venous walls, pleura, and regional lymph nodes. However, the etiology of the disease remains unknown due to its rarity. Therefore, we present a case of a solitary PCH lesion without symptoms in a 38-year-old female patient. According to computed tomography, she was diagnosed with lung carcinoma, indicated by a tiny nodule with ground-glass opacity detected in her right upper lung. However, no other lesions were detected on systemic examination. Consequently, partial lung r...

Surgical removal of giant pelvic liposarcoma after preoperative transcatheter arterial embolization
We report a case of a 73  year-old man who was admitted for urination and defecation disorder. Contract computed tomography (CT) revealed a mass of approximately 21 cm occupying the pelvis, bladder, ureter, prostate, and rectum being compressed by the tumor. Since intraoperative mass bleeding was predicted, we embolized t he tumor nutrition artery and performed a total pelvic exenteration. Regarding the course of the operation, SSI and pelvic infection were developed after the operation. The patient was discharged 21 days after surgery. Moreover, there is no evidence of local recurrence and distant metastasis at 18  mo...

Antemortem diagnosis of pulmonary tumor thrombotic microangiopathy associated with gastric cancer and response to immediate chemotherapy
We describe the case of a 43-year-old woman who was hospitalized with cough and respiratory distress that lasted for 2  months. Computed tomography findings demonstrated multiple areas of interlobular septal thickening and ground-glass opacities in both lungs. Transthoracic echocardiography demonstrated a D-shaped left ventricle suggesting right heart overload, and pulmonary blood flow scintigraphy revealed multipl e small, peripheral, and patchy areas of reduced blood flow. Upper gastrointestinal endoscopy revealed a signet-ring carcinoma. The patient was diagnosed with pulmonary tumor thrombotic microangiopathy based on...

Two case reports of immune checkpoint therapy on chromophobe renal cell carcinoma with sarcomatoid differentiation
AbstractRenal cell carcinoma (RCC) is the most predominant type of kidney cancer in adults and comprises several histological subtypes. Among them, the chromophobe RCC (ChRCC) with sarcomatoid differentiation is a rare subtype, and its therapeutic strategy remains unclear. Hence, to provide more information on effective therapeutic strategies against ChRCC, we report two cases of ChRCC with sarcomatoid differentiation treated with nivolumab monotherapy or ipilimumab –nivolumab combination therapy. One patient was treated with nivolumab monotherapy after the failure of sunitinib, while the other was treated with ipilimuma...

Eosinophilic cystitis mimicking hemorrhagic cystitis in relapsed follicular lymphoma
AbstractEosinophilic cystitis (EC) is a rare and non-infectious inflammatory disorder characterized by transmural infiltration of eosinophils in the bladder wall. The diagnosis of EC is made only by the pathophysiological findings. Because the urinary symptoms of EC are quite similar to other urinary tract disorders including hemorrhagic cystitis (HC), it can be misdiagnosed or left undiagnosed. A 49-year-old woman with relapsed and refractory follicular lymphoma presented with sudden-onset gross hematuria after the chemo-immunotherapy. The patient was initially treated as HC with continuous bladder irrigation, resulting i...

Systematic review and meta-analysis of reports of patients with gastric cancer aged 80  years and older
AbstractThe number of elderly patients in gastric cancer surgery is rapidly rising. Almost all data on gastric cancer in people over the age of 80 come from a single institution, and there is no systematic review of a large number of patients. Therefore, we conducted a comprehensive analysis of the prognosis of patients with gastric cancer surgery who were aged 80  years or older. From January 2010 to November 2021, reports on gastric cancer in the elderly aged 80 and over were gathered. We searched PubMed for “Gastric cancer and elderly and 80 years old” as a keyword, and 253 reports were extracted. The Ichushi-Web ...

A case of a pregnant woman with locally advanced cervical esophageal cancer treated with definitive chemoradiotherapy
AbstractThe information of definitive radiotherapy for a pregnant woman with malignancy was limited; however, it was reported to be potentially feasible with minimal risks. We performed definitive chemoradiotherapy for a pregnant woman with locally advanced cervical esophageal cancer. Feasibility of radiotherapy and safety of fetus were confirmed by the phantom study estimating fetal dose, and monitoring it in each radiotherapy session. The planned chemoradiotherapy completely eradicated esophageal cancer while preserving her laryngopharyngeal function. A female infant was delivered by cesarian section after planned chemor...

A case of recurrent refractory cervical primary histiocytic sarcoma treated with pembrolizumab
AbstractHistiocytic sarcoma (HS) is a rare disease with a poor prognosis. The efficacy of immune checkpoint inhibitors for this disease has not been established. A 13-year-old boy with HS refractory to conventional chemotherapy was treated with pembrolizumab, an immune checkpoint inhibitor. After treatment, the primary lesion and the bone metastases showed improvement; however, new metastatic lesions also occurred. This case suggests that the effect of immune checkpoint inhibitors might depend not only on programmed death ligand-1 expression and the ratio of tumor mutational burden, but also on other factors, such as the t...

Cystic degeneration during neo-adjuvant chemotherapy predicts squamous metaplasia of triple negative breast cancer: report of two cases
AbstractNeo-adjuvant chemotherapy (NAC) has become a standard treatment for advanced breast cancer because of the advantage of monitoring drug sensitivity and enabling breast-conserving therapy. The changes during NAC are also important to know the biological characteristics of the tumor. We experienced two cases with cystic degeneration and enhancement of the cyst wall during NAC for triple negative breast cancer (TNBC). They were diagnosed to have breast cancer with squamous metaplasia. In case 1, a 37-year-old woman with right breast cancer diagnosed as TNBC, T3N3M0, Stage 3b was treated with NAC. MRI showed a cystic de...

Long-term treatment with ALK inhibitors for postoperative recurrence of ALK-rearranged lung cancer
AbstractWe encountered a 40-year-old female patient who developed, in chronological order, carcinomatous pleuritis and lymphangitis, multiple lymph node metastases, brain metastases, and intramedullary spinal cord metastases after resection of lung adenocarcinoma followed by adjuvant chemotherapy. Echinoderm microtubule-associated protein-like 4 (EML4) and the anaplastic lymphoma kinase (ALK) fusion gene, variant 2 was identified in her cancer cells. By changing the ALK inhibitors from the 1st to 3rd generation each time when metastases were identified and incorporating local treatments in a timely fashion, such as metasta...

Post-pubertal ovarian yolk sac tumor with unusual follicular growth pattern, simulating struma ovarii
AbstractYolk sac tumors (YST), formerly known as endodermal sinus tumors, are uncommon ovarian neoplasms. They are most common in the second and third decades of life and are rare in women older than 39  years of age. YST exhibit various histologic patterns. Microfollicular pattern resembling thyroid tissue has not been reported. We introduce here a case of yolk sac tumor in a 39 y/o female, presented with pelvic masses and high level of serum Alpha-fetoprotein (Alpha Fetoprotein) (>  1000 IU/ml). The patient underwent left salpingo-oophorectomy. On microscopic study, morphologic findings demonstrated a neoplasm com...

A rapid and durable response to larotrectinib in a patient with NTRK fusion-positive secretory carcinoma originating from the external auditory canal
AbstractFor decades, no clear consensus existed on the standard treatment option for malignant tumors of the external auditory canal, an extremely rare disease. Here we report the case of a 55-year-old female patient with secretory carcinoma that originated from the left external auditory canal. Magnetic resonance imaging (MRI) at baseline showed that the tumor had extended to the medulla oblongata despite surgical and radiation treatments for more than 20  years from the initial diagnosis. Based on the results of a next-generation sequencing test of a formalin-fixed paraffin-embedded surgical specimen indicating that the...
