A novel surgical technique for removal of intramedullary nails when there is a failure of the proximal extraction device: report of two cases and review of literature.
Diaphyseal fractures of the tibia is a common injury with an incidence of 16.9/100000 population. Surgical treatment with intramedullary interlocking nail is the treatment of choice in closed diaphyseal tibial injuries. Removal of the nail is required in non-union, infected implant, implant failure or during exchange nailing procedure. Implant removal surgery presents some unique challenges to the Orthopaedic surgeons like broken nails, broken screws and failure of the extraction devices to engage with the threads of the nail due to stripping of the threads in the proximal part of the nail. Are extra locking bolts or fibular plating more important in extreme nailing of distal tibia fractures? A cadaveric biomechanical analysis
Distal tibia fractures can be treated with intramedullary tibial nailing or plating [1,2]. Nailing may be preferred to plating because of its minimally invasive approach which may decrease operative time and avoid fracture devascularization [3]. Further, nailing may permit earlier weightbearing [4]. However, it can be challenging to achieve acceptable fixation when the fracture location is so distal that it only permits placement of one or two interlocking screws through the distal portion of the nail, and there are concerns for higher rates of malunion in these fractures treated with nails [4]. Acute kidney injury (AKI) is a heterogeneous clinical syndrome involving a range of risk factors and acute injuries that affect both short- and long-term clinical outcomes [1]. Studies have shown that AKI occurs in 10-15% of hospitalized patients and in over 50% of intensive care unit (ICU) patients [2,3]. Obesity has become an epidemic over recent years. Existing literature shows that AKI is common in surgical patients with severe obesity. The Influence of Pelvic Tilt on Sacral Insufficiency Fracture Occurrence: Insights into the Prevalence of High Pelvic Tilt Among Patients Affected
Pelvic ring injuries are relatively rare injuries [1,2], accounting for only about 3% of all fractures. However, polytraumatized patients were diagnosed with pelvic ring injuries in almost 25% of cases [1,2] and they are quite frequently in older patients [1,3] due to the low-energy trauma mechanism, which leads to fractures in bone with lower bone density [1]. The incidence of pelvic ring fractures in older people is 90/100,000 and it is increasing due to demographic changes [3].
