Modern practical management of acute severe colitis
AbstractAcute severe ulcerative colitis (ASUC) is one of life-threatening complications that occur in one-fifth of ulcerative colitis (UC) patients with significant morbidity and an estimated mortality rate up to 1%. There are no validated clinical scoring systems for ASUC. Intravenous corticosteroids remain the cornerstone for the management of ASUC patients However, one-third of patients are steroid refractory and require colectomy in the pre-biologic era or salvage therapy in the post-biologic era. The currently available predictors of non-response to steroids and salvages therapy are sub-optimal. Furthermore, there is ...

Renal dysfunction in routine proton-pump inhibitor use may be linked to comorbidities: A real-world observational study
ConclusionsMild and non-progressive increase in serum creatinine occurred in 10% to 15% of patients on routine PPI use. A majority of them had other risk factors. Small sample size and short follow-up duration are a few limitations of this study.Graphical abstract (Source: Indian Journal of Gastroenterology)

Comprehensive assessment of nutritional and functional status of patients with ulcerative colitis and their impact on quality of life
ConclusionsMacronutrient deficiency, micronutrient deficiency, lower functional status and poorer QOL are highly prevalent among patients with UC. The independent predictors of poor QOL were histologically active disease and low serum vitamin D levels. Identifying and correcting the deficiencies may help in improving the QOL of patients with UC.Graphical Abstract (Source: Indian Journal of Gastroenterology)

Uphill battle: Innovation of thiopurine therapy in global inflammatory bowel disease care
AbstractInflammatory bowel disease (IBD) is a chronic inflammatory disorder of the gastrointestinal tract that encompasses two major conditions: Crohn ’s disease (CD) and ulcerative colitis (UC). Historically, IBD has been primarily reported in western countries, but over the past decades, its prevalence is rapidly increasing, especially in lower and middle-income countries (LMICs) such as India and China and also in Sub-Saharan Africa. The prev alence of IBD in LMICs has been the subject of growing concern due to the impact of access to public healthcare and the burden it places on healthcare resources. The classical th...

Antibiotics for inflammatory bowel disease: Current status
AbstractThere is abundant literature reporting about the use of antibiotics in inflammatory bowel disease (IBD), but their role in the management of IBD is not entirely clear. Diverse infectious organisms have been implicated in the pathogenesis of Crohn ’s disease. Also, infections are believed to be a trigger for flares of ulcerative colitis. The benefit of the routine use of antibiotics in IBD is equivocal. However, there are certain situations, where antibiotics have a clear role and evidence of benefit: perianal fistula, intra-abdominal absc esses in Crohn’s disease, acute pouchitis and infection-related flares. ...

Gut bacteriome in inflammatory bowel disease: An update on recent advances
AbstractInflammatory bowel diseases (IBD) are chronic inflammatory gut disorders, majorly classified as ulcerative colitis and Crohn ’s disease. The complex, multifactorial etiopathogenesis of IBD involves genetic predisposition, environmental cues, aberrant mucosal immune response and a disturbed gut microbiota. Epidemiological trends, studies in gnotobiotic mice models and genome-wide association studies, identifying genes in volved in microbial handling, together mount evidence in support of the gut microbiota playing a pivotal role in IBD pathogenesis. Both Crohn’s disease and ulcerative colitis are characterized b...

Clinical profile of 1208 newly diagnosed colorectal cancer patients in Tamil Nadu —A multi-centric survey
ConclusionsA majority of patients with newly diagnosed CRC in Tamil Nadu belonged to the lower socioeconomic  classes. About 60% had CRCs located within the reach of the flexible sigmoidoscope. Two-thirds of the patients exceeded stage II disease at presentation.Trial registrationNot applicableGraphical Abstract (Source: Indian Journal of Gastroenterology)
