[A bone-destroying tumor of the maxilla. Reparative giant cell granuloma or brown tumor?]
We report the case of a 31-year-old pregnant patient with a rapidly growing tumor of her left maxilla whose first symptoms during pregnancy mimicked chronic sinusitis. After swelling of the cheek, diplopia, and recurrent epistaxis appeared, she was referred to our Department for further diagnostics. After CT scan, biopsy was performed under the presumption of a malignant process with the surprising histological result of a reparative giant cell granuloma. At the same time, hyperthyroidism and nodular goiter were diagnosed and further endocrinological examinations were planned. Not until a parathyroid adenoma was diagnosed aft...

Authors: Kleinschmidt S, Plinkert PK, Fuchs-Buder T, Seyfert UT Abstract In the first part of the medical education article on haemostatic disorders in ENT patients the basic physiology of haemostasis and main diagnostic tools were presented and discussed. The second part presents disorders of the coagulation system, thrombocyte function and blood vessels with special emphasis on clinical practice in ENT surgery. In this context, haemophilia A and B, von Willebrand disease and different forms of thrombocytopenia are of main clinical importance. Some underlying diseases such as malignomas, renal and hepatic insufficie...

Möglichkeiten und Grenzen der Fluoreszenzdiagnostik und photodynamischen Therapie : Teil 1: Fluoreszenzdiagnostik. HNO. 2003 Dec;51(12):1019-1035 Authors: Betz CS, Leunig A Abstract Fluorescence diagnostic methods enable tumor visualization via specific excitation of endogenous or exogenous tumor-selective fluorescent markers and subsequent optical measurement of the induced fluorescence signals. In the following review, the reader will be given a clear and extensive understanding of all aspects of these methods relevant for ENT specialists. Apart from a demonstration of the clinical difficulties ...

CONCLUSION: Adjusted for gender and age to the European Standard Population (ESP), the prevalence of hearing impairment observed both in HÖRSTAT and the Aalen sample is considerably lower than reported for international studies. Since the analysis refers to cross-sectional data only, possible cohort effects are not considered in the prevalence projection. PMID: 28271167 [PubMed - as supplied by publisher] (Source: HNO)

[Intraoperative monitoring of the facial nerve : Vestibular schwannoma surgery].
CONCLUSION: Identification and mapping of the facial nerve in the surgical field can be reliably achieved by direct electrical stimulation; potentially dangerous events can be identified in real time by the free-running EMG and the processed EMG, and almost in real time by facial nerve evoked potentials. Intraoperative prognostics are hampered by false-positive results with all available methods and have limited reliability. PMID: 28271169 [PubMed - as supplied by publisher] (Source: HNO)

Episodisch auftretendes Schwindelgefühl. HNO. 2003 Oct;51(10):845-859 Authors: Schmäl F, Stoll W Abstract While an acute loss of unilateral vestibular function (for example in case of neuritis vestibularis or temporal bone fracture) leads to long-term vertigo, this paper deals with the phenomenon of episodic vertigo (duration: seconds up to hours). Both peripheral-vestibular and central disturbances can be responsible for this symptom. The aim of this paper is to present otological and neurological diseases which can lead to episodic attacks of vertigo. PMID: 28246679 [PubMed] (Source: HNO)

[The physiologic and the pathologic swallowing process].
Authors: Cantemir S, Laubert A Abstract It is estimated that 5 million people in Germany currently suffer from swallowing disorders. Due to increasing life expectancy and the good outcome of very premature infants, this number is expected to increase. Swallowing normally occurs in four phases, and for each phase there are diagnostic tools to assess whether the problem is anatomic-organic or functional. A detailed case history is followed by clinical examination of the cranial nerves, the orofacial region, the swallowing reflex, and pharyngeal and esophageal transit. Attention is also paid to age-related c...

[Biological therapies in otology. German version].
Authors: Roemer A, Staecker H, Sasse S, Lenarz T, Warnecke A Abstract Millions of people worldwide suffer from hearing loss. Current treatment for patients with severe to profound hearing loss consists of cochlear implants. Providing the cochlear nerve is intact, patients generally benefit enormously from this intervention, frequently achieving significant improvements in speech comprehension. There are, however, some cases where current technology does not provide patients with adequate benefit. New therapeutic concepts based on cell transplantation and gene therapy are developing rapidly, at least in the research...

Cochlear implant treatment of patients with single-sided deafness or asymmetric hearing loss.
This report presents the first evidence of successful binaural rehabilitation with CI in a relatively large patient cohort and the advantages over (Bi)CROS and BCI in smaller subgroups, thus confirming the indication for CI treatment. Moreover, patients with long-term acquired deafness (>10 years) show a benefit from the CI comparable to that observed in patients with shorter-term deafness. [Relationship between functional endoscopy and impedance-pH measurement. German version].
Authors: Michel F, Stroh T, Fritsche R, Ahrens P Abstract Classic gastroenterological diagnostic tools are proving increasingly insufficient for analyzing the complex causal relationship between extra-esophageal symptoms and presumed pathological reflux activity. Some new methodological approaches, such as functional endoscopy (videopanendoscopy, VPE), are considered to be helpful. However, there are currently no data objectively verifying the usefulness of this method. In a pilot study, a good correlation between the reflux symptom index (RSI) and endoscopic findings was shown. Impedance-pH measurement i...

[Treatment of supine position-related obstructive sleep apnea with smartphone applications].
Authors: Haas D, Birk R, Maurer JT, Hörmann K, Stuck BA, Sommer JU Abstract BACKGROUND: Positional obstructive sleep apnea (POSA) is common in mild and moderate forms of obstructive sleep apnea (OSA). Two smartphone applications (apps) professing to avoid the supine position (SP) are available: for Android the "Apnea Sleep Position Trainer" and for iOS the "SomnoPose-Sleep Position Monitor". The smartphone needs to be attached to the chest to recognize SP, which then triggers a vibration alarm. This is intended to encourage the patient to change position and the vibration stops as soon...

[Radiosurgery and fractionated stereotactic radiotherapy of vestibular schwannoma].
Authors: Hamm K, Herold HU, Surber G, Rosahl S Abstract Management of vestibular schwannoma (VS) should always be interdisciplinary and results better than the natural course. Particularly in small VS, either microsurgical resection or radiosurgery (RS) can be employed. RS is a special method (initially only possible stereotactically) for delivering high-precision radiation from many directions to the target point (the isocenter) in a single high dose. With the development of three different systems-Gamma Knife (Elekta, Stockholm, Sweden), special linear accelerators, and CyberKnife (Accuray, Sunnyvale, C...

This article presents a case of malignant transformation of vestibular schwannoma 13 years after stereotactic radiation therapy, which lead to an acute life-threatening condition. Although the observation is currently only based on case reports, an increasing number of these support the hypothesis that there is a relevant risk of malignant transformation in the long-term course of previously irradiated vestibular schwannomas.
