The Digital Hospital: A Scoping Review of How Technology Is Transforming Cardiopulmonary Care
Innovative models of health care that involve advanced technology in the form of a digital hospital are emerging globally. Models include technology such as machine learning and smart wearables, that can be used to integrate patient data and improve continuity of care. This model may have benefits in situations where patient deterioration must be detected quickly so that a rapid response can occur such as cardiopulmonary settings. A Clinical Guide for Assessment and Prescription of Exercise and Physical Activity in Cardiac Rehabilitation. A  CSANZ Position Statement
Patients with cardiovascular disease benefit from cardiac rehabilitation, which includes structured exercise and physical activity as core components. This position statement provides pragmatic, evidence-based guidance for the assessment and prescription of exercise and physical activity for cardiac rehabilitation clinicians, recognising the latest international guidelines, scientific evidence and the increasing use of technology and virtual delivery methods. The patient-centred assessment and prescription of aerobic exercise, resistance exercise and physical activity have been addressed, including progression and safety c...

Artificial Intelligence in Cardiology: An  Australian Perspective
Significant advances have been made in artificial intelligence technology in recent years. Many health care applications have been investigated to assist clinicians and the technology is close to being integrated into routine clinical practice. The high prevalence of cardiac disease in Australia places overwhelming demands on the existing health care system, challenging its capacity to provide quality patient care. Artificial intelligence has emerged as a promising solution. This discussion paper provides an Australian perspective on the current state of artificial intelligence in cardiology, including the benefits and cha...

An Unusual Case of Massive Cardiomegaly
A 28-year-old female presented with recurrent episodes of paroxysmal atrial flutter. On clinical examination, heart sounds were distant, particularly for her small build. Jugular venous pressure was normal. Work-up was significant for massive cardiomegaly on chest x-ray PA view (Figure  1A). (Source: Heart, Lung and Circulation)

A Call for Australian Clinical Practice Guidelines for Paediatric Hypertension
At present there are no national guidelines for the detection and management of hypertension in children and adolescents in Australia. This is a significant gap given that there is substantial and growing evidence linking high blood pressure (BP) in childhood and adolescence with end organ damage, cardiovascular risk factors in adulthood, and overt cardiovascular disease in mid-adulthood. Hypertensive children and adolescents often have evidence of end organ damage, including left ventricular hypertrophy and subclinical cardiac dysfunction [1,2], increased arterial stiffness and carotid intima-media thickness [3,4], and re...

Health Economic Analysis of CMR: A  Systematic Review
Uptake of cardiac magnetic resonance (CMR) in Australia has been limited by issues of cost and access. There is a need to inform future application of CMR by evaluating pertinent health economic literature. We sought to perform a systematic review on the health economic data as it pertains to CMR. Hybrid Totally Thoracoscopic Maze and Catheter Ablation for Persistent Atrial Fibrillation: Initial Experience
Atrial fibrillation is now a pandemic in our ageing community. Although James L. Cox devised a surgical procedure with near-universal curative success in 1987, catheter-based interventions have flourished. For persistent atrial fibrillation (AF), however, an isolated endocardial approach has limitations: procedural times are long, carry risk, and the outcomes are not durable.By combining left atrial endocardial and epicardial interventions with staged mapping, we optimise the benefits of both approaches. Efficacy and Safety of Mavacamten in the Treatment of Hypertrophic Cardiomyopathy: A Systematic Review
Current pharmacological options for hypertrophic cardiomyopathy (HCM) are not disease-specific; while it treats symptoms, mavacamten targets the underlying pathology. We aim to assess the efficacy and safety of mavacamten, a first-in-class cardiac myosin inhibitor, in symptomatic obstructive HCM. (Source: Heart, Lung and Circulation)

In Search of Adeno-Associated Virus Vectors With Enhanced Cardiac Tropism for Gene Therapy
Globally, adeno-associated virus (AAV) vectors have been increasingly used for clinical gene therapy trials. In Australia, AAV-based gene therapy is available for hereditary diseases such as retinal dystrophy or spinal muscular atrophy 1 (SMA1). Many preclinical studies have used AAV vectors for gene therapy in models of cardiac disease with outcomes of varying translational potential. However, major barriers to effective and safe therapeutic gene delivery to the human heart remain to be overcome. (Source: Heart, Lung and Circulation)

Incidental Finding of a Giant Left Main Coronary Artery Aneurysm as an Anterior Mediastinal Mass
A 79-year-old female with a history of hypertension, diabetes, and chronic kidney disease presented to the emergency department with an acute asthma exacerbation and was incidentally found to have a large anterior mediastinal mass on a chest X-ray (Figure  1A). The mass was further evaluated on a coronary computed tomography angiography (CCTA) which identified a giant 6.8x5.7x7.0 cm coronary artery aneurysm (CAA) located at the bifurcation of the left main coronary artery (LMCA), involving both the proximal left anterior descending and circumflex co ronary arteries. Therapeutic Angiogenesis Using Growth Factors After Myocardial Infarction: From Recombinant Proteins to Gene Therapies and Beyond
Ischaemic heart disease is the primary cause of death worldwide with myocardial infarction (MI) contributing to significant morbidity and mortality. The human heart has a limited capacity to regenerate and the significant loss of cardiomyocytes after MI can overwhelm this limited innate regenerative capability. This is in part compensated for by the creation of collagen-rich scar tissue. Therapeutic angiogenesis is an exciting prospect that can assist cardiac regeneration after MI with various approaches having been explored. (Source: Heart, Lung and Circulation)

The New South Wales Sudden Cardiac Arrest Registry: A Data Linkage Cohort Study
Sudden cardiac arrest (SCA) in young people aged 1 to 50 years often occurs with no presenting symptoms or risk factors prompting screening for cardiovascular disease prior to their cardiac arrest. Approximately 3,000 young Australians suffer from sudden cardiac death (SCD) each year, making this a major public health issue. However, there is significant variation in the way incidence is estimated resulting in discrepancy across reporting which impacts our ability to understand and prevent these devastating events. Cell, Gene and Regenerative Therapies in Cardiovascular Disease: Charting a Course From the Research Bench to Clinical Utility
We all know of the huge (and increasing) global burden of cardiovascular disease, and of the research efforts in so many disciplines to minimise the onset and impact of cardiac injury from inherited and acquired disease. In the areas of cardiac cell and gene therapy and tissue regeneration, early results of two or three decades ago fell short of their promise for one reason or another, or their priority lessened when other proven treatments became available. Along the way, the field of cardiac therapeutics has seen the heights of a Nobel Prize-winning advancement [1] but also the depths of unexpected fatalities in trial su...
