[Defect coverage of the hand using a rare Siamese combined flap].
We report 2 cases of extensive soft tissue defects simultaneously affecting the palm and dorsum of the hand. The defect was covered with a Siamese flap combination consisting of a free, distally extended lateral upper arm flap with microvascular anastomosis, and a posterior interosseous artery flap. We discuss possible indications, the operative technique and advantages versus disadvantages of this rarely applied flap combination. [Transtrapezoid carpal dislocation: a case report].
We present the case of a 50-year-old worker, who sustained a crush injury of his left hand with a dorsal fracture dislocation of the trapezoid bone and the associated second metacarpal bone at the STT, and a concomitant compartment syndrome of the hand. The fracture was reduced immediately and compartment release was performed, followed by percutaneous stabilisation using three K-wires. Material removal after six weeks was followed by intensive physical therapy. At the final follow-up examination after three months, we observed a good recovery of hand function compared to the unaffected right side with 75 % grip strength...

[Lundborg's resection arthroplasty vs. Pyrocarbon spacer (Pyrocardan ®) for the treatment of trapeziometacarpal joint osteoarthritis: a two-centre study].

[Mastopexy in Massive Weight Loss Patients - Extended Ribeiro Technique and Usage of the Lateral Intercostal Artery Perforator Flap (LICAP Flap) for Autoaugmentation].
Conclusion Modification of the Ribeiro technique by using the medial and lateral breast pillar (LICAP flap) allows autoaugmentation of the upper pole. Parenchymal shaping through plication and suspension of the breast mound should improve breast shape over time, with the aim of reducing the incidence of recurrent ptosis. [The Body Dysmorphic Disorder in Plastic Surgery - A Systematic Review of Screening Methods].
Conclusion The limited availability of validated screening instruments for BDD in plastic surgery is markedly in contrast to the supposedly high prevalence of the disease of 2.4 %. Among the currently used screening tools, the BDDQ-DV and the DCQ appear to be most suitable. Further research efforts are needed to establish better screening methods for the BDD in the plastic surgical patient population and to examine the effects of BDD on treatment results. [Adequate Antibiotic Prophylaxis and Therapy of Periprosthetic Infections of the Breast - Review of 468 Consecutive Implant Removals].
Conclusion In the majority of cases, pathogen detection was an incidental finding, while capsular contracture caused surgical revision. Pathogens and resistance patterns found in this study differed from the majority of international publications. In our institution, Cefuroxime and amoxicillin/clavulanic acid have been proven to be a reasonable choice for prevention and treatment of periprosthetic infections. [Thumb Lengthening Following Amputation at the Proximal Phalanx using a Semicircular Distraction Ring Fixator: Results of 5 Cases].
We report the results of proximal phalanx lengthening in 5 patients using a semicircular ring-type external fixator device. This prototype was developed at our clinic in collaboration with a specialised small company (Rotomed AG, Allmenstrasse 4, CH-4512 Bellach, www.rotomed.ch) based on the principles of Ilizarov's external fixator. In all patients, subjective and objective results were satisfactory (26.2 mm distraction length and an average Michigan Hand Outcome Score of 82.2%) without any rotational/axial deformities or complications requiring revision surgery. [Atypical Lipomatous Tumour of the Hand].
We present the case of a 49-year-old man with a lipomatous tumour measuring 8×4 cm at the dorsum of the right thumb. MRI demonstrated an inhomogeneous signal after contrast medium application. The tumour was excised in its entirety. Since histology confirmed the diagnosis of an ALT, the thumb was irradiated. The patient remained free of recurrence. An ALT has to be suspected if a lipomatous tumour is very large. We recommend an MRI prior to surgery in tumours larger than 5 cm. Excision should follow oncologic principles. [The Extended Percutaneous Needle Fasciotomy Technique (PNF+) For Dupuytren's Contracture].
Authors: Lenze WP Abstract The extended percutaneous needle fasciotomy (PNF+) is a minimal invasive technique especially suitable for treating severe Dupuytren's contractures and recurrences. The familiar needle fasciotomy is complemented by a spontaneous tearing of the skin and a full-thickness graft to cover the defect. Skin tears in fingers and palm are not a complication, but an integral part of the PNF+. The neurovascular bundles remain intact. This surgical procedure may be repeated as often as required.
