Factors involved in the clinical profile of carpal tunnel syndrome.
The objective of this study was an investigation into the hypothesis that several distinct factors are predictive of the clinical profile for the condition. The records of 1,117 patients: 909 women (81 %) and 208 men (19 %) with a mean age of 63 years, were analysed. The whole group was divided into subgroups, depending on the variables analysed: sex, age, duration of the condition, severity of symptoms, severity of electrophysiological abnormalities and occurrence or lack of comorbidities. RESULTS: Of all the considered variables, the Levine symptom scores had the greatest impact on the condition's clinical profile: t...

[Results of the reconstruction of the middle phalangeal base of the finger through an osteochondral autograft from the hamate bone].
Authors: Pillukat T, Kalb K, Windolf J, van Schoonhoven J Abstract OBJECTIVE: The fractured base of the middle phalanx was reconstructed in 13 patients using an osteochondral transplant from the carpometacarpal joint surface of the hamate bone. The goal was to restore joint stability with preservation of mobility. Indications were acute and missed isolated destruction of the palmar middle phalanx base ≥ 30 %. Contraindications were destruction of the head of the proximal phalanx, advanced chondropathy of the head of the proximal phalanx, and extensive soft tissue injury with loss of skin coverage for the proxima...

A case of severe gouty tophi-induced carpal tunnel syndrome: Operative finding and its outcome.
Authors: Lee JK, Kim JW, Kim YS, Koo BS Abstract Gout is a disease featuring acute arthritis, joint deformity and severe pain caused through the deposition of monosodium urate crystals in and around synovial tissue 1. Tophi are stone-like deposits of monosodium urate in the soft tissues, synovial tissues, or bones adjacent to the joints. The tophi can compromise joint motion and deteriorate bony structure, finally leading to functional disability. Therefore, early treatment using uric acid lowering agents is standard practice in gout patients to prevent the formation of tophi through controlling uric acid levels. ...

One-stage wound healing of fingertip injuries induced by treatment of artificial dermis.
CONCLUSIONS: One-stage wound healing of fingertip injuries induced by artificial dermis treatment is an easy and effective approach to restoring defects after injury with excellent functional and cosmetic results. Nearly all kinds of fingertip injuries can be managed with this method without any further surgical treatment. Therefore, this is a good alternative for the management of fingertip injuries. CONCLUSION: This review details the evolution of the classical methods of TE in Regenerative Medicine into the evolving field of biofabrication by bioprinting. The advantages of 3D bioprinting over traditional tissue engineering techniques are based on the assembling of cells, biomaterials and biomolecules in a spatially controlled manner to reproduce native tissue macro-, micro- and nanoarchitectures, that can be utilized not only to potentially produce functional replacement tissues or organs but also to serve as new models for basic research. CONCLUSION: Transposition of DIE-vessels to the subcutaneus compartment is a good alternative for free flap revascularisation in this patient group. [Diagnosis Related Groups in reconstructive plastic surgery of malignant head and neck skin tumours].
Conclusion The current representation of reconstructive surgery for malignant skin tumours by DRGs is associated with potential disincentive effects, which may have an impact on patient care. In search of a diversified portfolio, the main group DRG J08 should be considered in addition to the common practice of coding. The authors of this study think that this may be a possibility to compensate for the actual devaluation and to achieve resource-related reimbursement in high-cost cases. However, incorrect and inflationary use should be avoided due to the lack of long-term orientation caused by dilution of the DRGs. PMID:...

Authors: Osinga R, Babst D, Bodmer ES, Link BC, Fritsche E, Hug U Abstract Introduction This work assessed both subjective and objective postoperative parameters after breast reduction surgery and compared between patients and plastic surgeons. Patients and Methods After an average postoperative observation period of 6.7 ± 2.7 (2 - 13) years, 159 out of 259 patients (61 %) were examined. The mean age at the time of surgery was 37 ± 14 (15 - 74) years. Conclusion In contrast to cortisone injection treatment, a persistent pain reduction was achieved in the fat injection group in the 3-month follow-up period, whereas cortisone injection only resulted in a short-term reduction of pain lasting about 6 weeks. The diagnosis of CTS was made solely on the basis of clinical findings in 551 patients (58 %); 392 patients (42 %) also had electrodiagnostic tests performed, for various reasons. Patients were followed-up at 1 and 6 months with assessments that included the Levine scores, filament tests, grip and pinch strength. No significant differences in Levine scores were found at th...

Multiple glomus tumours in multidigit nail bed.
We report a rare case of painful multidigit subungual glomus tumours in both hands.A 34-year-old male patient complained of painful fingertips (right: thumb, index finger, long finger, and ring finger; left: thumb, long finger, ring finger, small finger) on both hands for more than 5 years. Magnetic resonance imaging (MRI) detected multiple subungual glomus tumours in the fingertips (right long finger, ring finger, and left thumb, long, ring, and small finger) of both his hands, so the authors conducted tumour resection and sent biopsies for relevant fingers. We found the glomus tumour underneath the nail bed and achieved ...

Authors: Ludolph I, Lehnhardt M, Arkudas A, Kneser U, Pierer G, Harder Y, Horch RE Abstract During the last two decades, understanding and experience of microsurgical reconstruction have increased. This is linked to improved instruments and technical devices, as well as to the increased number of available donor sites for microvascular flap transfer. If the indication is appropriate, it is now possible to perform personalised, safe and successful microsurgical reconstructions, almost independently of the patients' age. Despite these constant developments, there is little specific literature on microsurgery related ...

[Legal quality criteria of expert opinions in liability for medical malpractice].
Authors: Neu J Abstract In litigation regarding to liability for medical malpractice the court itself for lack of medical expert knowledge cannot judge a claimed error in treatment. That applies too for the question if there is causation between the error in treatment and claimed damage to patient's health. In this respect, the court is dependent on a medical expert and is bound to his assessment, unless there is no quality deficit in respect of medical and/or legal criteria. The more the medical expert knows the legal background of physician's liability for medical malpractice, the less is the risk of legal qualit...

[Conciliatory proceedings of a German mediation center after breast augmentations].
Discussion The greater frequency of a causal relationship between the damage caused to patient health and malpractice - in comparison to the general statistics - may be due to the greater complexity of breast augmentation than generally expected. Not only the surgery itself, but - to an even greater extent - adequate planning and aftercare are of predominant importance in satisfying the patient. All these factors and the high expectations of the patients lead to a higher request rate. PMID: 28958116 [PubMed - in process] (Source: Handchirurgie Mikrochirurgie Plastische Chirurgie)
