[The revenue from finger replantation and revision amputation: Is it cost-covering?]
CONCLUSIONS: In the German DRG system, the reimbursement for the treatment of finger amputation depends on the type of health insurance. In patients with workers' compensation insurance, cost-covering revenue is generated from both finger revision amputation and finger replantation, whereas in patients with statutory health insurance, only the revenue of primary finger revision amputation appears to be cost-covering. Hence for finger amputation injuries with subsequent complex reconstruction procedures, a revision of the cost calculation is required to avoid inappropriate incentives in patient care. Treatment of neglected, severely displaced, open malunion of the distal radius and ulna in an elderly person with Alzheimer's disease - a case report.
Authors: Zyluk A, Puchalski P Abstract The case of a patient suffering from Alzheimer's disease who sustained epiphyseal, severely dislocated fractures of the distal radius and ulna is reported. As the patient was incapacitated and attempts to contact her curator failed, the decision was made on a conservative treatment of the fractures. This treatment eventually failed and resulted in malunion with exposure of bone fragments above the skin. After obtaining informed consent from her curator, a corrective osteotomy was performed using an original method, followed by fixation of bone fragments in almost anatomical po...

Successful treatment of paediatric lower limb CRPS by continuous epidural anaesthesia: a report of 2 cases.
Authors: Zyluk A, Puchalski P Abstract CRPS occurs in children less frequently than in adults, but in the last two decades it has become a well-established entity in children and adolescents. The symptomatology, course, responsiveness to treatment and prognosis of the paediatric disease is also different from the "adult" form. The paper presents the successful treatment of two cases of paediatric CRPS involving the lower limb, by continuous epidural anaesthesia with bupivacaine. Both patients developed the condition after minor trauma or overuse. CRPS diagnosis was based on clinical grounds. Both patients...

Microsurgery training with smartphone.
Authors: Capkin S, Cavit A, Kaleli T Abstract Microsurgery is widely used in experimental research models and clinical surgery. However, microsurgical applications require precise technical skills and continuous training. Here, we proposed a low cost, practical microsurgery model, which can be easily applied using smartphones at home or at office. Test platform was created using latex gloves, and a phone holder was then positioned at one side of a table. A smartphone with 10x magnification was secured on the phone holder. Microsurgical device habit, stitching and knotting exercises by making cuts at different angle...

[DRG revenues and costs of multimorbid patients in the German DRG system - Analysis of the surgical treatment of chronic wounds at a university hospital based on the example of pressure ulcers].
CONCLUSION: The treatment of most multimorbid patients with decubital ulcers is often not adequately reimbursable within the German DRG system. A strict treatment algorithm with a reduced number of operations and intensive patient care are helpful approaches, but may not be sufficient to reduce cost-intensive complications. It seems important to re-evaluate the current DRG codes for these cases. PMID: 30130832 [PubMed - in process] (Source: Handchirurgie Mikrochirurgie Plastische Chirurgie)

[Breast reconstruction with the free TRAM or DIEP flap - What is the current standard? - Consensus Paper of the German Working Party on the Microsurgery of Peripheral Nerves and Vessels].
Authors: Eisenhardt SU, Momeni A, von Fritschen U, Horch RE, Stark GB, Bannasch H, Harder Y, Heitmann C, Kremer T, Rieger UM, Kneser U Abstract The most common donor-site for autologous breast reconstruction is the abdomen. Over the past several decades technical advances have resulted in the development of flaps that have been associated with a progressive decrease in abdominal wall morbidity. However, controversy exists related to the differences between muscle-sparing (MS)-TRAM and deep inferior epigastric perforator (DIEP) flaps. Hence, the question which approach should be considered standard of care remains u...

Elevation Technique for Medial Branch based Superficial Circumflex Iliac Artery Perforator flap.
Authors: Suh YC, Hong JP, Suh HP Abstract According to early studies on the superficial circumflex iliac artery perforator flap, there are two major types of perforators that are presumed to originate from the superficial circumflex iliac artery: the medial perforator with a direct cutaneous vessel (superficial branch) - located relatively medially on the flap, and the lateral perforator, traveling laterally beneath the deep fascia and often with an intramuscular pathway penetrating the deep fascia on the lateral aspect (deep branch) of the flap. Although there are well described studies on the anatomy of the arter...

The current gold standard in therapy of such defects is the tissue transplantation in terms of free or local flaps. Unfortunately, donor site morbidity remains a considerable risk of flap surgery. Therefore, tissue engineering of autologous vascularized long term stable adipose tissue constructs could enrich the therapeutic possibilities of soft tissue defects. De novo adipose ...
