Intraoperative robotic surgical system-related problems in robot-assisted thoracoscopic surgery
ConclusionTeams performing robot-assisted thoracoscopic surgery should be familiar with the features of robotic surgical systems and various robotic devices, be aware of reported problems during robot-assisted thoracoscopic surgery, and be prepared for emergencies. Clover stitch technique for aortic valve in mitral valve repair
AbstractIn the case of mitral repair with severe aortic regurgitation, aortotomy and selective cardioplegia are necessary for myocardiac protection. In this situation, the saline test for mitral valve repair cannot be accomplished due to incomplete left ventricular filing. In patients undergoing mitral valve repair concomitant with severe aortic valve insufficiency, after cardiac stand still was achieved by selective cardioplegia. Each center of the aortic leaflet, termed the node of Arantius, was stitch up using a 5-0 polypropylene suture, forming a clover leaflet shape. This stitch inhibits aortic valve opening and reduc...

Prognosis after non-surgical therapy for acute type A aortic dissection
ConclusionsMortality from AAAD is 51.4%, including inappropriate candidates for surgery. When patients were evaluated as suitable candidates for surgical intervention but subsequently refused the surgical procedure, in-hospital mortality was 37%. Long-term survival of hospital survivor was acceptable. These data can be a benchmark for patient and patient ’s family to select medical therapy for AAAD in consideration with the patient’s will. Early thrombus formation including hypo-attenuated leaflet thrombosis after surgical bioprosthetic aortic valve replacement
ConclusionHALT was observed in one-fifth of the cases after surgical aortic valve replacement during the very early postoperative phase in an institution wherein administration of continuous heparin infusion after surgery is a standard practice. HALT did not affect the early prognosis or incidence of cerebral infarction. (Source: General Thoracic and Cardiovascular Surgery)

Acquired von Willebrand syndrome and post-operative drainage: a comparison of patients with aortic stenosis versus coronary artery disease
ConclusionThis study has shown that there is a correlation between von Willebrand factor antigen, von Willebrand factor activity and total drainage to the level of statistical significance in aortic stenosis patients and in the overall study population. A modified adventitial inversion with graft insertion technique in acute Type A aortic dissection
AbstractAcute type A aortic dissection may originate from a primary intimal tear located in the ascending aorta and often extends retrogradely into the aortic root. How to prevent bleeding in the aortic root and eliminate false lumen is very important in aortic dissection. We have developed a modified anastomotic technique that involves inverting adventitial and graft into aorta and reinforcing with a felt strip on the external border of the aortic wall. Since 2020, 45 consecutive patients with type A aortic coarctation have undergone this aortic root reconstruction procedure, to date, none have been reopened for bleeding ...

Evaluation of myocardial work changes after lung resection —the significance of surgical approach: an echocardiographic comparison between VATS and thoracotomy
ConclusionsUnlike RV function, LV function remained preserved after lung resection. The thoracotomy group exhibited greater RV function impairment than did the VATS group. Further studies should evaluate the long-term impact of surgical approach on cardiac function. (Source: General Thoracic and Cardiovascular Surgery)

Presence of manubrium –sternum joint does not assure sufficient elevation of sternum in Nuss procedure for pectus excavatum patients
ConclusionEven when the MSJ is present, horizontal separation enhances the elevation of the sternum. Furthermore, horizontal separation of the sternum reduces postoperative pain. (Source: General Thoracic and Cardiovascular Surgery)

Reduction in drain-related adverse events using the barbed suture method for chest tube wound closure
ConclusionsThe barbed suture method could reduce drain-related adverse events compared to the conventional method. Therefore, it might be a potential standard method for chest tube wound closure in patients undergoing general thoracic surgery. (Source: General Thoracic and Cardiovascular Surgery)
