Impact of the first surge of the coronavirus disease pandemic on general thoracic surgery practices in Kanagawa: a questionnaire survey by the Kanagawa General Thoracic Surgical Study Group
ConclusionsRestrictions on surgery had a significant impact on the care provided by general thoracic surgery departments. To avoid patient inconvenience, establishing a collaborative system that refers patients to operational medical institutions in case of medical treatment restrictions may be useful. Artificial pneumothorax suppresses postoperative inflammatory reaction in mediastinal tumor surgery
ConclusionsArtificial pneumothorax suppressed the postoperative inflammatory response, pleural effusion, and albumin reduction, and shortened the hospital stay in patients undergoing mediastinal tumor surgery. (Source: General Thoracic and Cardiovascular Surgery)

Prosthetic vascular graft infection: A systematic review and meta-analysis on diagnostic accuracy of 18FDG PET/CT
ConclusionAccording to our results, 18FDG PET/CT is a useful diagnostic method in detecting active VGI with high diagnostic accuracy. Because of its ability to evaluate morphology and main texture using SUVmax, the 18FDG PET/CT provides an objective assessment of aspects and extent of disease activity, which results in preventing unnecessary surgery, proper treatment planning, and evaluating the effectiveness of treatment. Computed tomography-guided lymphangiography from the para-aortic lymph node: a useful approach for chylothorax after esophagectomy
ConclusionsCT-guided lipiodol lymphangiography from the PALN for chylothorax after esophagectomy was safe and thus can be considered an alternative treatment option to ultrasonography-guided intranodal lymphangiography. The flexible use of CT and ultrasonography for lymphangiography may improve the treatment outcomes of chylothorax after esophagectomy. (Source: General Thoracic and Cardiovascular Surgery)

Proximal versus extensive repair in acute type A aortic dissection: an updated systematic review and meta-analysis
ConclusionsIn patients with ATAAD, PR was associated with lower odds of in-hospital mortality but higher odds of late mortality. ER and PR demonstrated similar post-operative complication and reoperation rates. (Source: General Thoracic and Cardiovascular Surgery)

Successful transfemoral-transcatheter aortic valve replacement in high-risk patients with a grade 4 atheroma in the ascending aorta: cerebral protection with a filter device
AbstractPerioperative stroke is one of the most serious complications after transcatheter aortic valve replacement (TAVR), and it should be especially noted in patients with an atheroma in the ascending aorta. Here, we report the first experience of transfemoral (TF)-TAVR with the insertion of a filter device for two patients with severe aortic stenosis who were incidentally detected a grade 4 atheroma in the ascending aorta preoperatively. The patients had a favorable postoperative course without any cerebrovascular events or procedure-related complications. (Source: General Thoracic and Cardiovascular Surgery)

Thoracoscopic resympathectomy for persistent or recurrent palmar hyperhidrosis: single-center experience
ConclusionsResympathectomy is highly effective procedure for patients who have persistent or recurrent symptoms. However, the indication of the operations should be more dissuasive as possible to avoid the risk of any undesirable psychologically side effects. Effectiveness of early rehabilitation following aortic surgery: a nationwide inpatient database study
ConclusionEarly rehabilitation within 3  days of aortic surgery was associated with improved physical functions at discharge, shorter ICU and hospital stays, and lower hospitalization costs without increased mortality. (Source: General Thoracic and Cardiovascular Surgery)

Combined cardiac surgery procedures and liver transplant: a single-center experience
ConclusionConcomitant cardiac procedure and liver transplant is a valid treatment option and should be considered with risk stratification criteria of the patient with end-stage liver disease and cardiac surgical pathologic characteristics. (Source: General Thoracic and Cardiovascular Surgery)

Hyperlactatemia as a prognostic indicator for contemporary left ventricular assist device implantation
ConclusionsThis study demonstrates early post-operative hyperlactatemia is associated with increased morbidity and mortality following LVAD implantation. Even early post-operative lactate trends within the first 24 post-operative hours appear to have a useful role in predicting longitudinal survival following implantation. Careful monitoring of post-operative lactate with measures to normalize levels should be considered in the early care of LVAD patients. (Source: General Thoracic and Cardiovascular Surgery)

Mid-term results of mitral valve reconstruction with autologous pericardial patch for active infective endocarditis with extensive leaflet destruction
ConclusionMitral valve reconstruction using autologous pericardial patch is a useful treatment option for extensive mitral valve destruction due to active infective endocarditis. Our clinical data revealed that good mid-term results were obtained for posterior leaflet lesions, while extensive anterior leaflet  and bi-leaflet lesion repair did not yield satisfactory results. The indication for surgical repair should be carefully evaluated when an extensive anterior region is involved. (Source: General Thoracic and Cardiovascular Surgery)

Pericardiectomy for constrictive pericarditis at a single Japanese center: 20  years of experience
ConclusionsThe diagnosis using multimodality imaging for CP was effective in our institution and tagged cine MRI had the greatest sensitivity in diagnosing CP. Preoperative condition, including preoperative low LVEF or inotrope use, was correlated with long-term outcome after pericardiectomy. Pericardiectomy should be performed early and as radically as possible to prevent recurrent heart failure. (Source: General Thoracic and Cardiovascular Surgery)

Efficacy, safety, and optimal timing of single-trocar video-assisted flexible thoracoscopic debridement under local anesthesia for complicated parapneumonic empyema
ConclusionVAFTS-D might be an acceptable first-line procedure in patients with suspected CPE. VAFTS-D should be performed as early as possible for a successful outcome, and to obtain useful information on the pleural cavity. (Source: General Thoracic and Cardiovascular Surgery)

A new technique to detect communication sites for pleuroperitoneal communication
AbstractPleuroperitoneal communication is one of the complications for continuous ambulatory peritoneal dialysis. Communication sites, such as defects or bleb-like lesions, are found in the diaphragm, but it is sometimes difficult to detect these sites. We combined the infrared thoracoscopy and negative pressure technique to detect communication sites. We think our new technique will become an alternative option for difficult and complex cases.
