Xanthogranulomatous lesion in recurrent appendicitis
Publication date: Available online 24 May 2016 Source:Formosan Journal of Surgery Author(s): Asri Che Jusoh, Syahmi Abdul Ghani Acute appendicitis (AA) is the most common acute surgical condition of the abdomen. Most resected specimens have been reported to have marked cellular infiltration, predominantly by neutrophils. By contrast, the occurrence of a xanthogranulomatous (XG) lesion is extremely rare. To date, only a few cases have been reported in the literature. Moreover, its clinical implications remain to be evaluated. Here, we report the case of a 16-year-old boy who presented with a typical history of AA fo...

Salvage of ineffective transarterial embolization with liver resection in a patient with repeated hepatic angiosarcoma bleeding
In conclusion, TAE is only a temporary treatment for internal bleeding from a ruptured hepatic angiosarcoma. Recurrent, potentially fatal bleeding may be caused by supplemental vascularization of the portal vein. Surgical tumor resection is necessary for the curative treatment of hepatic angiosarcomas and the prevention of repeated internal bleeding. (Source: Formosan Journal of Surgery)

Supratentorial leptomeningeal hemangioblastoma resection after tumor embolization
Publication date: April 2016 Source:Formosan Journal of Surgery, Volume 49, Issue 2 Author(s): Ching-Yi Lee, Shiu-Jau Chen Cerebral hemangioblastomas are vascular tumors and rarely occur in supratentorial locations. Here we present a case of a 52-year-old woman with a supratentorial hemangioblastoma mimicking a meningioma. Brain computed tomography and magnetic resonance imaging revealed an intensely contrast-enhanced dural-based solid mass in the left frontal region. We demonstrate that vascular supratentorial hemangioblastomas can be resected with limited blood loss through preoperative middle meningeal artery em...

Coronary artery revascularization in hemodialysis patients with multivessel coronary artery disease
Publication date: Available online 23 April 2016 Source:Formosan Journal of Surgery Author(s): Chan-Yang Hsu, Yih-Sharng Chen Coronary artery disease (CAD) is a major cause of morbidity and mortality in patients with end-stage renal disease (ESRD). Such a combination poses a considerable technical challenge for revascularization because of multiple comorbidities and higher surgical risk. Here, we conducted a literature review on different revascularization strategies, on-pump and off-pump bypass grafting outcomes, harvested internal mammary artery numbers, and potential complications (e.g., steal syndrome) in manag...

Management of traumatic arterial pseudoaneurysms as a result of limb trauma
Conclusion Publications relating to traumatic artery PSAs have increased significantly over the past decade. The target population is often the young adult male. The treatment of arterial trunks is largely dominated by conventional surgery, despite the technical innovations of the endovascular method. However, it is interesting for PSA-dependent arterial branches. (Source: Formosan Journal of Surgery)

Using inhaled nitric oxide for a patient with pulmonary hypertension during biventricular assist device surgery in the operating room
Publication date: Available online 30 March 2016 Source:Formosan Journal of Surgery Author(s): I-Chun Hou, Fan-Yen Lee, Su-Fang Wang, Szu-Hui Wu, Mei-Ling Hsueh, Mei-Lein Tu Many patients experience hemodynamic collapse associated with severe heart failure despite receiving an optimal medical therapy. Therefore, mechanical circulatory assist devices, such as the intra-aortic balloon pump (IABP), extracorporeal membrane oxygenation (ECMO) device, and ventricular assist device (VAD), are used. In this paper, we report the successful treatment of a 59-year-old male patient with heart failure awaiting heart tra...

Occlusion of acute distal brachial, proximal radial and ulnar arteries in a young thrower
Publication date: Available online 24 March 2016 Source:Formosan Journal of Surgery Author(s): Wei-Chien Hsu, Chung Da Wu Acute arterial occlusion, a rare condition in throwers, requires early detection and treatment for avoiding further complications. Thus far, no study has mentioned the occurrence of distal brachial, proximal radial, and ulnar artery occlusion in baseball players. An adolescent baseball pitcher presented with acute occlusion of the distal brachial, proximal radial, or ulnar artery. The patient complained of a cold sensation in the hand, wrist, and distal forearm. On physical examination, decrease...

A rare case of Rapunzel syndrome with multiple small bowel intussusceptions and bowel obstruction
We report a case of a 5-year-old girl with Rapunzel syndrome causing multiple small bowel intussusceptions. We were unable to identify the main cause of this condition during laparoscopic reduction. Definite diagnosis was made after the development of gastrointestinal tract obstruction. Surgeons should consider the possibility of Rapunzel syndrome when diagnosing the main cause of intussusceptions. (Source: Formosan Journal of Surgery)

Effects of tamoxifen on traumatic brain injury-induced depression in male rats
Conclusion Our results suggest that intraperitoneal injection of TMX may ameliorate TBI-induced depression-like behavior in rats by increasing neuronal p-ERK1/2 expression, which may be associated with neuronal Bcl2 and BDNF expression and decreased neuronal apoptosis. This effect might represent a mechanism underlying the recovery from depression-like behavior. (Source: Formosan Journal of Surgery)

Abdominis rectus intramuscular myositis ossificans
Publication date: February 2016 Source:Formosan Journal of Surgery, Volume 49, Issue 1 Author(s): Wei-Tang Li, Shyue-Yih Horng, Hsiung-Fei Chien Myositis ossificans (MO) is a benign, localized, and self-limiting soft tissue tumor. The condition is associated with prominent heterotrophic bone formation within the muscles, ligaments, and fascia. Clinically, MO could be confused with malignant lesions, such as osteosarcoma and soft-tissue sarcoma. Few reports have described the cytological findings and differential diagnoses of MO and malignant tumors trough diagnostic imaging. Thus, we reviewed the literature on MO...

Extensor-pollicis-longus or -brevis tendon rupture after corticosteroid injection
We present two cases and review possible rupture mechanisms. In case 1, the patient presented with a painful swelling on the dorsal side of her right first metacarpophalangeal joint. An orthopedic surgeon prescribed three local corticosteroid injections over 4 weeks, and the patient experienced a sudden loss of extension on the distal phalanx of her thumb 3 weeks after the last injection. Two days later, the proximal phalanx of the thumb lost its extension. Surgical exploration revealed that the extensor-pollicis-longus and extensor-pollicis-brevis tendons were completely severed. Both tendon ends had edematous necrosis ...

A rare hepatic trauma caused by vertical inertial force
We report the case of a 33-year-old man who presented to our emergency department with severe abdominal pain. He had no trauma history. Physical examination revealed peritonitis. Abdominal computed tomography revealed a large hepatic laceration with massive hemoperitoneum. The patient underwent emergency laparotomy and surgical repair of the hepatic laceration. A vertical inertial force was postulated as the cause of such a rare hepatic trauma. He recovered uneventfully. We review and discuss the detailed trauma history, clinical manifestations, imaging study observations, and mechanisms underlying inertial force injury in...
