The Management of Posttraumatic Nasal Deformities
Facial Plast Surg DOI: 10.1055/a-2152-8670Nasal bone fractures are the most common fractures of the facial skeleton and are often accompanied by bony, cartilaginous, and soft tissue injuries. These injuries are often complex, and when untreated or inadequately treated, can lead to posttraumatic nasal deformity. The most common deformities are the crooked nose and the saddle nose. Both deformities may result in significant cosmetic and functional concerns. The treatment of these deformities can be complex, requiring careful evaluation of the nose and thoughtful planning to correct the cosmetic deformity and restore function...

Functional Nasal Surgery in a Compromised Soft Tissue Envelope
Facial Plast Surg DOI: 10.1055/a-2145-7072Functional nasal surgery in the setting of a compromised soft tissue envelope (STE) requires significant planning and preparation. Proper preoperative evaluation is imperative to assess the degree of STE compromise and determine the best surgical approach. Alternative approaches to functional nasal surgery, such as limited access surgical approaches, should be considered to minimize complications while achieving functional improvement. Given the increased risk of ischemia and necrosis in individuals with a history of a compromised STE, a comprehensive postoperative treatment algori...

Implications of Ozempic and Other Semaglutide Medications for Facial Plastic Surgeons
Facial Plast Surg DOI: 10.1055/a-2148-6321Obesity is a growing global health concern, leading to various health issues, including diabetes. Semaglutide-based medications, such as Ozempic, Wegovy, and Rybelsus, have emerged as potential treatments. These medications, belonging to the glucagon-like peptide-1 (GLP-1) receptor agonist class, mimic the action of GLP-1, regulating appetite and promoting weight loss. Clinical trials have shown their effectiveness in reducing body weight and improving metabolic parameters. Ozempic, though Food and Drug Administration-approved for diabetes, is also used off-label for weight loss al...

Closure of Nasal Septal Perforations Using a Diced Cartilage in Fascia Graft
The objective of this study was to assess the feasibility of using a diced cartilage in fascia (DC-F) graft for successful closure of nasal septal perforations and to evaluate symptom reduction. This was a retrospective study of 18 patients undergoing surgical repair of symptomatic nasoseptal perforations of different etiologies using a DC-F graft from 2020 until 2021. The procedure was feasible in all of the 18 patients. Reconstruction of septal defects with a DC-F graft led to reduction of crust formation, reduction of epistaxis, and improvement of nasal breathing in 13 out of the 18 patients when seen for their 2-month ...

Pediatric Functional Rhinoplasty
Facial Plast Surg DOI: 10.1055/s-0043-1772576Surgery on the growing pediatric nose poses many questions regarding the indications and proper timing. Historically, surgeries on the pediatric nose led to facial growth alterations so many urged caution and waiting until skeletal maturity at the age of 18. However, animal studies showed some promise that facial growth may not be affected. Investigations into effects of pediatric sinus surgery and septal surgery on facial growth showed no issues. The external nose is somewhat different in that minor variations potentially can grow to a cosmetic deformity. However, numerous stud...

Rhinoplasty with Concurrent Repair of Septal Perforation
Facial Plast Surg DOI: 10.1055/s-0043-1772577Concurrently performing septal perforation repair and rhinoplasty is challenging. Successful management of patients who require or desire these procedures performed simultaneously can be achieved with appropriate preoperative workup and optimization, thoughtful surgical planning, masterful execution, and diligent postoperative care. Consideration of perforation etiology and status of the perforation along with overall nasal health are the foundation upon which surgery may be planned. Preoperative evaluation to determine locations of obstruction, aesthetic goals, and feasibility ...

Cosmetic Treatments with Energy-Based Devices in Skin of Color
This article reviews various modalities that have shown positive outcomes in Fitzpatrick skin types 4 to 6.Radiofrequency (RF) skin tightening devices have emerged as a noninvasive method for improving skin laxity by stimulating collagen and elastin production without generating excessive heat, making them suitable for darker skin tones. Microfocused ultrasound (MFUS) is another noninvasive option for skin tightening and lifting. By producing thermal coagulation in the deeper layers of the skin, MFUS stimulates collagen production, resulting in improved skin tightness. Fractional laser resurfacing, both ablative and nonabl...

Management of the Congenital Cleft Earlobe with a Conchal Cartilage Graft
Facial Plast Surg DOI: 10.1055/s-0043-1772195Congenital earlobe clefts are the most common lower auricular malformations. They represent a unique reconstructive challenge. The goal of surgery includes restoration of a natural lobular contour and volume as well as a fine surgical scar. Several surgical techniques have been described, most of which only address the cleft deformity but not the soft tissue deficiency. We hereby describe a technique which addresses both the cleft and improves soft tissue deficiency through a conchal cartilage graft. Rhinoplasty: Considerations for Patients with Facial Paralysis
Facial Plast Surg DOI: 10.1055/a-2127-3768Nasal obstruction is a common sequela of flaccid facial paralysis but one that is often underaddressed surgically. Weakness of nasal musculature on the paralyzed side of the face leads to nasal valve narrowing through loss of static and dynamic nasal side wall tone as well as inferomedial displacement of the alar base. Standard rhinoplasty techniques such as alar batten grafts or flaring sutures may be used to support the nasal side wall in facial paralysis. However, to address the inferomedial alar displacement, suspension techniques are often required. Suture resuspension and fas...

Cleft Rhinoplasty: Correcting the Alar Base
Facial Plast Surg DOI: 10.1055/a-2127-3665The characteristic cleft nasal deformity presents several challenges for the rhinoplasty surgeon attempting to provide the optimal nasal function and appearance for their patient. One of the difficulties with cleft rhinoplasty is how best to address the malposition alar base. The purpose of this review is to evaluate the various surgical approaches and techniques for properly repositioning the alar base for cleft patients. Outcomes are ultimately dependent on individual patient factors, anatomy, surgical technique, and surgeon experience. Below, we will review the variety of techni...
