Experimental long-term sub-normothermic machine perfusion for non-allocable human liver grafts: first data towards feasibility
ConclusionThis study provides promising data on the feasibility of long-term SNMP as a  tool to gain time during MP to optimize ECD organs to decrease the gap between organ demand and supply.Long-term (24  h) sub-normothermic liver machine perfusion seems to be possible, although some adjustments to the protocol might be necessary to improve the general outcome. This has so far been shown for normothermic machine perfusion, bearing some drawbacks compared to the sub-normothermic variant. (Source: European Surgery)

Simulation-based training as a  continuous means of vascular surgical education—a roadmap
ConclusionSimulation-based training is a  safe, efficient, and well-manageable possibility for improving the education of future experts in open vascular surgery. It can help hone simple skills such as suture techniques as well as prepare for complex surgical procedures. In time we hope to further integrate simulation-based training in th e existing curriculum. (Source: European Surgery)

Surgical treatment of deep-lying ano-/rectovaginal fistulas using a  de-epithelialized “Singapore flap” (pudendal thigh flap)
ConclusionThe treatment of an ano-/rectovaginal fistula using the “de-epithelialized pudendal thigh flap” (Singapore flap) is a promising treatment alternative. In particular, patients who have had previous proctological interventions show a benefit from this procedure. (Source: European Surgery)

Case report: the rare clinical picture of vasculitis of the pericardium in rheumatoid arthritis
ConclusionBased on the fact that tumor necrosis factor blockers can induce vasculitis, etanercept might have been responsible for the exacerbation of pericarditis. The underlying rheumatoid arthritis could also be considered as a  trigger. The detection ofStaphylococcus aureus DNA in the pericardium and the exacerbation of pericarditis could be attributed to secondary vasculitis after an infection withS.  aureus, whereas the tendency to infection due to humoral immunodeficiency after years of immunosuppressive therapy has to be discussed as a  trigger. T  stage-dependent lymph node and distant metastasis and the accuracy of lymph node assessment in rectal cancer
ConclusionIn rectal cancer, at leastn = 12 lymph nodes are to be minimally investigated. The investigation of fewer lymph nodes is associated with a higher risk of false-negative pN0 findings. In particular, in pT3 rectal cancer, the investigation of more than 12 lymph nodes lowers the risk of false-negative pN0 findings. An ups taging effect by the investigation of a possibly maximal number of lymph nodes could not be detected. (Source: European Surgery)

Elective colectomy financial and opportunity cost analysis: diagnosis, case type, diversion, and complications
ConclusionInflammatory diagnoses appear to drive the indication for diversion following elective colectomy. Colectomies with diversion have complication rates 1.36 times higher than without diversion, as well as excess LOS and costs. Resections AR with diversion had more than twice the proportion of complications as without diversion. Preoperative mitigation of inflammatory risk factors may decrease the rate of diversion and reduce complications, financial risk, and opportunity cost. (Source: European Surgery)

Classification and evaluation of anastomotic leaks after esophageal surgery —a tertiary university experience
ConclusionThe results of this study showed that the classification proposed by the Surgical Working Group on Endoscopy and Ultrasound can be validated and that there is a  clear differentiation between the subtypes. Standardized diagnosis and management improve the overall outcome of patients.Main novel aspectsThis article gives an introduction to classifying anastomotic insufficiencies according to the classification proposed by the Surgical Working Group on Endoscopy and Ultrasound. Results of the classification can be validated, with a  clear differentiation of postoperative outcome between subtypes.
