Proenkephalin A 119 –159, a Possible Early Biomarker of Acute Kidney Injury in Complex Endovascular Aortic Repair: a Single Centre Observational, Cross Sectional Study
Acute kidney injury (AKI) is a common organ dysfunction following aortic intervention and may be associated with worse outcomes.1 The current definition of AKI is based on flawed parameters, namely serum creatinine (SCr) concentration and urine output, which could delay the diagnosis of AKI. Proenkephalin A 119 –159 (penKid) is an endogenous monomeric peptide and is described as a novel and reliable biomarker of renal function, stable after collection, with a long half life in vivo and not affected by sex or inflammation. Screening Men and Women above the Age of 50 Years for Abdominal Aortic Aneurysm: A Pilot Study in an Upper Middle Income Country
Screening for abdominal aortic aneurysm (AAA) is recommended in high risk populations based on local conditions. Differences in lifestyle and risk factors between countries with different income status make risk stratification based on geographic location necessary. The majority of epidemiological studies on AAA have reported data from high income countries. The aim of this study was to explore the prevalence and risk factors for AAA in an upper middle income country in Eastern Europe. (Source: European Journal of Vascular and Endovascular Surgery)

The BioMimics 3D Helical Centreline Nitinol Stent in Chronic Limb Threatening Ischaemia and Complex Lesions: Three Year Outcomes of the MIMICS-3D Registry
There is a need for improved outcomes in the endovascular treatment of patients suffering from chronic limb threatening ischaemia (CLTI), highly calcified lesions and chronic total occlusions (CTOs). The helical centreline self expanding BioMimics 3D stent might be particularly useful in these high risk subsets, combining flexibility and fracture resistance with radial strength. Herein, the performance of the BioMimics 3D stent was assessed in these high risk subsets. (Source: European Journal of Vascular and Endovascular Surgery)

Editor's Choice -- Exercise Therapy for Chronic Symptomatic Peripheral Artery Disease: A  Clinical Consensus Document of the European Society of Cardiology Working Group on Aorta and Peripheral Vascular Diseases in Collaboration With the European Society of Vascular Medicine and the European Society for Vascular Surgery
All guidelines worldwide strongly recommend exercise as a pillar in the management of patients affected by lower extremity peripheral artery disease (PAD). Exercise therapy in this setting presents different modalities, and a structured programme provides optimal results. This clinical consensus paper is intended to promote and assist the set up of comprehensive exercise programmes and best advice for patients with symptomatic chronic PAD. Different exercise training protocols specific for patients with PAD are presented. (Source: European Journal of Vascular and Endovascular Surgery)

Effect of a Home Based Exercise Programme and a Smartphone app: WalkingPad Randomised Clinical Trial
Objective: Home based exercise therapy (HBET) for patients with peripheral artery disease (PAD) and intermittent claudication (IC) is a low cost, low risk, and effective therapeutic measure to improve walking distances, walking performance, and quality of life (QoL). However, adherence is low due to a lack of supervision and intrinsic motivation. The aim was to investigate whether HBET programmes with a behaviour change intervention, supported and unsupported by a smartphone application (app), is effective in improving walking distances and performance, as well as QoL over six months. Multi-Institution Analysis Demonstrates that Augmented Intelligent Maps Improve Intra-Operative Safety During Physician Modified Endograft Repairs
Objective: Augmented intelligence (AI) tools have become popular additions to medicine. AI can transform operating rooms with advanced visualisation and aid surgeons with critical intra-operative decision making. AI tools allow patient specific maps to be created with a cloud computing software based on pre-operative CT scans which are superimposed with fluoroscopic images in the operating room. The image fusion arguably permits easier identification and cannulation of visceral vessels. A unique feature of this software is that the map can be manually moved and adjusted in the operating room as the aorta is shifted with ri...

Phase Contrast MRI Can Separate Conventional ≥ 50% Carotid Stenosis and Near Occlusion
Objective: Carotid near occlusion is a severe stenosis causing a diameter reduction ( “collapse”) of the distal internal carotid artery (ICA). This collapse can be moderate with normal appearing distal ICA (without full collapse, Fig. 1A,B), or severe with a threadlike distal ICA (with full collapse, Fig. 1C,D). Conservative management is recommended for symptomatic near occlusio n.1 However, the diagnosis is difficult, and near occlusion is likely underdiagnosed and undergo unnecessary surgery.2,3 The current diagnostic reference in studies has been systematic feature interpretation of angiography (such as computed to...

Defining Near Occlusion With Full Collapse on CTA
Objective: Carotid near occlusion with full collapse, the severe subset of near occlusions (Fig. 1), has been associated with a high risk of early pre-operative stroke recurrence.1,2 Full collapse has traditionally been defined as a threadlike appearance of the distal internal carotid artery (ICA).2 Recently, a measurement based definition of full collapse on computed tomography angiography (CTA) was suggested based on prognostic data: distal ICA diameter ≤ 2.0 mm and/or side to side distal ICA diameter ratio (ICA ratio) ≤ 0.42.
