Outcomes of total hip arthroplasty after failed free vascularized fibular grafting for osteonecrosis of the femoral head
ConclusionTHA after VFG may have similar short to midterm outcomes with primary THA after removal of the residual abutting bone graft and correct positioning of the femoral stem. (Source: European Journal of Orthopaedic Surgery and Traumatology)

Watchful waiting for the undisplaced hip dysplasia when undergoing contralateral hip reconstruction
ConclusionWatchful expectancy of undisplaced hip dysplasia presenting in patients undergoing contralateral hip reconstruction during years of acetabular growth is safe as the risk of persistent dysplasia and additional surgery is low.Level of evidenceIII. A Comparison of Outcomes of Culture positive and Culture negative Acute Knee Prosthetic Joint Infection following Debridement, Antibiotics and Implant Retention (DAIR)
ConclusionDAIR with polyethylene exchange will give comparable results irrespective of the culture positivity. Symptom duration of more than 30  days for DAIR is significantly associated with DAIR failure. DAIR failures can be successfully managed with two-stage revision. The “purse string” technique for anterior glenohumeral instability: long-term results 7–13-year follow-up

Outcomes of multiligament knee injury treated with versus without internal brace suture augmentation
ConclusionIn this cohort of patients with MLKI treated with versus without IB, outcomes and reoperation rates trended toward favoring IB, but the study was not sufficiently powered to reach statistical significance. Internal bracing could be useful in the management of MLKI.  In the future, matched patient cohorts with more patients are warranted to further evaluate the clinical impact of the internal brace in MLKI. Anterolateral knee complex considerations in contemporary anterior cruciate ligament reconstruction and total knee arthroplasty: a systematic review
AbstractThe anterolateral ligament (ALL) was first described in 1879 in the context of Segond fractures, which correlate with a 75 –100% chance of an anterior cruciate ligament (ACL) tear or a 66–75% chance of a meniscal tear. The purpose of this paper is to provide an updated comprehensive review on the anterolateral ligament complex of the knee focusing on the: (1) anatomy of the ALL/ALC; (2) associated biomechanics/funct ion; and (3) important surgical considerations in contemporary anterior cruciate ligament (ACL) reconstruction and total knee arthroplasty (TKA). A systematic review of studies on ALL was conducted ...

The long-term results of shoulder hemiarthroplasty in irreducible four-part fracture-dislocation of the proximal humerus without rotator cuff tear arthropathy
ConclusionIn long-term follow-up, the greater tuberosity was superiorly migrated and externally rotated. Active forward flexion and abduction in the last follow-up were significantly limited. However, in terms of pain relief, a satisfactory result was seen. (Source: European Journal of Orthopaedic Surgery and Traumatology)

Oblique anterior pelvic external fixator for intraoperative reduction of rotationally unstable pelvic ring injuries
AbstractIncomplete sacroiliac joint injuries are often associated with external rotation and extension deformities on the injured hemipelvis. To appropriately correct this deformity, an oblique reduction force from caudal to cranial and lateral to medial is helpful. These injuries are often associated with traumatic disruption of the pubic symphysis. However, in injuries without traumatic disruption to the pubic symphysis, a two-pin oblique anterior external fixator can be used to obtain and maintain reduction of the sacroiliac joint, while percutaneous fixation is subsequently placed. Through a small case series and three...

Surgical treatment of popliteomeniscal fascicles tears is associated with better patient-reported outcome measures. A systematic review and meta-analysis
ConclusionThis SR and meta-analysis showed a significant improvement in the Lysholm score and subjective IKDC score following surgery for PMFs tears. However, controversy persists regarding the optimal surgical approach, with current literature favoring arthroscopic procedures.Level of evidenceIV. Timing of venous thromboemboli in patients with acetabular and pelvic ring fractures
ConclusionThe 2015 OTA expert panel recommends 4  weeks of anticoagulation for orthopedic trauma patients at high risk of VTE, which may be too short a duration. In our cohort, 25% of VTEs occurred greater than 4 weeks after injury. Additional research is needed to clarify the exact duration of anticoagulation after pelvic and acetabular fractur es; however, surgeons may want to consider anticoagulating patients for greater than 4 weeks.Level of evidenceLevel III —retrospective cohort. A prospective randomized controlled study of hamstring and bone-free quadriceps tendons autografts in arthroscopic ACL reconstruction
ConclusionAfter ACL reconstruction, we found a significant decrease in strength at the donor sites in both techniques, with similar patient-reported measures and stability outcomes. The H/Q strength ratio was higher in the QT group postoperatively, which may have a protective effect on the reconstructed ligament.Level of Evidence I It is a randomized controlled trial. A multi-institutional study of short-term mortality in COVID-positive patients undergoing hip fracture surgery: is survival better than expected?
ConclusionIn this multi-institutional analysis of COVID-positive patients undergoing hip fracture surgery, 30-day mortality was 10%. The 95% C.I. did not include 30%, suggesting that survival may be better than initially reported. While COVID-positive patients with hip fractures have high short-term mortality, the clinical situation may not be as dire as initially described, which may reflect initial publication bias, selection bias introduced by testing, or other issues.Levels of EvidenceTherapeutic Level III. Can gabapentinoids decrease perioperative opioid requirements in orthopaedic trauma patients? A single-centre retrospective analysis
ConclusionAnalgesic requirements vary from patient to patient depending on the injury ’s severity and surgery duration. However, there are no strict guidelines for pain relief in limb trauma surgeries which often leads to overuse and opioid-related complications or underuse and chronic pain. Gabapentinoids can supplement the analgesic effect of paracetamol in trauma patients during the perioperative period, decreasing the need for opioids.
