The yield of total body CT in the workup of fever of unknown origin in hospitalized medical patients
CONCLUSIONS: TBCT has a clinically significant yield under specific clinical scenarios in medical patients with FUO- reaching 55 % in patients with an elevated CRP and leukocyte count and low hemoglobin. It is reasonable to proceed to TTBCT when FDG-PET/CT is unavailable and in well-defined clinical situations.PMID:38307732 | DOI:10.1016/j.ejim.2024.01.027 (Source: European Journal of Internal Medicine)

Treatment in primary biliary cholangitis: Beyond ursodeoxycholic acid
Eur J Intern Med. 2024 Feb 2:S0953-6205(24)00037-2. doi: 10.1016/j.ejim.2024.01.030. Online ahead of print.ABSTRACTPrimary biliary cholangitis (PBC) is a rare cholestatic immune-mediated liver disease. The clinical course varies from mild to severe, with a substantial group of patients developing cirrhosis within a decade. These patients are at risk of hepatocellular carcinoma, decompensation and liver failure. First line Ursodeoxycholic acid (UDCA) treatment improves the cholestatic surrogate markers, and was recently associated with a favorable survival free of liver transplantation, even in case of an incomplete biochem...

The usefulness of 18F-FDG-PET/CT in detecting musculoskeletal and vascular involvement in patients with polymyalgia rheumatica receiving glucocorticoids
CONCLUSION: Even though GC therapy may decrease the 18-FDG uptake, PET/CT continues to be a useful tool to detect musculoskeletal and LVV involvement in PMR.PMID:38302404 | DOI:10.1016/j.ejim.2024.01.022 (Source: European Journal of Internal Medicine)

Neurological hypertensive emergencies: Correlation of blood pressure values with in-hospital outcomes in ischemic stroke
CONCLUSIONS: BP values and its management can not be related to in-hospital mortality in stroke patients, presented with HE, while they are associated to discharge disability and hospitalization length. In subgroup analysis, results were confirmed only in untreated (not anti-hypertensive therapies nor thrombolytic).PMID:38296661 | DOI:10.1016/j.ejim.2024.01.029 (Source: European Journal of Internal Medicine)

Osteonecrosis risk after steroids-related treatment of COVID-19 is not negligible: A cross sectional study
CONCLUSIONS: COVID-19-related therapy with steroids resulted in lower prevalence of osteonecrosis than that previously recorded in patients with severe acute respiratory syndrome caused by coronavirus-type-1. However, this risk seems not negligible and therefore, high clinical suspicion for early diagnosis is warranted, given the fact that a great proportion of hospitalized patients received steroids during the COVID-19 pandemic.PMID:38281818 | DOI:10.1016/j.ejim.2024.01.006 (Source: European Journal of Internal Medicine)

Detection of subclinical atrial fibrillation with cardiac implanted electronic devices: What decision making on anticoagulation after the NOAH and ARTESiA trials?
Eur J Intern Med. 2024 Jan 27:S0953-6205(24)00002-5. doi: 10.1016/j.ejim.2024.01.002. Online ahead of print.ABSTRACTAtrial fibrillation (AF) may be asymptomatic and the extensive monitoring capabilities of cardiac implantable electronic devices (CIEDs) revealed asymptomatic atrial tachi-arrhythmias of short duration (minutes-hours) occurring in patients with no prior history of AF and without AF detection at a conventional surface ECG. Both the terms "AHRE" (Atrial High-Rate Episodes) and subclinical AF were used in a series of prior studies, that evidenced the association with an increased risk of stroke. Two randomized c...

Transition of management strategies and long-term outcomes in cancer-associated venous thromboembolism from the warfarin era to the direct oral anticoagulant era
CONCLUSIONS: In the current historical comparison of cancer-associated VTE between the 2 large real-world registries, there was a striking change in the treatment strategies with decreased risks of recurrent VTE and major bleeding in the DOAC era compared with those in the warfarin era, while there seemed to be unmet needs of DOAC-related gastrointestinal bleeding.CLINICAL TRIAL REGISTRATION: URL: http://www.umin.ac.jp/ctr/index.htm UNIQUE IDENTIFIER: UMIN000044816.PMID:38278660 | DOI:10.1016/j.ejim.2024.01.012 (Source: European Journal of Internal Medicine)

The lowest well tolerated blood pressure: A personalized target for all?
Eur J Intern Med. 2024 Jan 25:S0953-6205(24)00032-3. doi: 10.1016/j.ejim.2024.01.025. Online ahead of print.ABSTRACTThe optimal blood pressure (BP) target for prevention of cardiovascular complications of hypertension remains uncertain. Most Guidelines suggest different targets depending on age, comorbidities and treatment tolerability, but the underlying evidence is not strong. Results of randomized strategy trials comparing lower (i.e., more intensive) versus higher (i.e., less intensive) BP targets should drive the definition. However, these trials tested different BP targets based on systolic BP, diastolic BP or combin...
