Spinal cord ischaemia after radical pleurectomy/decortication operation for mesothelioma [CASE REPORT]
Radical pleurectomy/decortication was performed in a patient with malignant pleural mesothelioma. During the surgery, the left subclavian artery was injured, and repaired via resection and anastomosis. However, the patient was diagnosed with paraplegia following extubation. After deliberate investigations, the mechanism of injury to the critical vascular zone of the spinal cord was determined. Here, we aim to draw attention to possible similar injuries. Minimally invasive extracorporeal circulation improves quality of life after coronary artery bypass grafting [ADULT CARDIAC]
CONCLUSIONS The current study proves that MiECC significantly improves HRQoL after coronary surgery compared with cCPB. This finding, combined with results from large-scale studies showing superior clinical outcomes from its use, enhances the role of MiECC as a dominant technique in coronary revascularization surgery. Bilateral mammary artery grafting increases postoperative mediastinitis without survival benefit in obese patients [ADULT CARDIAC]
CONCLUSIONS In obese patients, CABG surgery using BIMA instead of SIMA increased the risk of postoperative DSWI, without improving survival. According to our results, short-term postoperative risks of infection associated with BIMA are not offset by longer-term benefits in that patient population. A matched pairs analysis of non-rib-spreading, fully endoscopic, mini-incision technique versus conventional mini-thoracotomy for mitral valve repair [ADULT CARDIAC]
CONCLUSIONS An endoscopic procedure supported by 3D-visualization enables superior depth perception, facilitating an excellent quality of repair results. 3D-visualization is a helpful tool especially for complex reconstruction cases and exact placement of artificial neochordae. With this, an experienced mitral valve surgeon takes shorter operation times. Patients benefit from shorter hospitalization with reduced postoperative pain and early mobilization.
