Long-term results of external aortic ring annuloplasty for aortic valve repair [ADULT CARDIAC]
CONCLUSIONS External aortic ring annuloplasty provides a reproducible technique for aortic valve repair with satisfactory long-term results for each ascending aorta phenotype with bicuspid or tricuspid valve. Longer follow-up is ongoing with the AVIATOR registry. (Source: European Journal of Cardio-Thoracic Surgery)

The association between body mass index and outcome after coronary artery bypass grafting operations [ADULT CARDIAC]
CONCLUSIONS Class I and Class II/III obesity (BMI ≥30 kg/m2) was associated with increased pulmonary morbidity after CABG operations. There was no difference in mortality or infection morbidity in any BMI group compared with the normal group. (Source: European Journal of Cardio-Thoracic Surgery)

The prognostic value of metastatic lymph node number and ratio in oesophageal squamous cell carcinoma patients with or without neoadjuvant chemoradiation [THORACIC]
CONCLUSIONS Both MLN and MLR were significant prognostic factors in OSCC patients regardless of neoadjuvant chemoradiation. But in patients with neoadjuvant chemoradiation, the survival rates were similar between ypN2 and ypN3 patients, suggesting that there was no necessity of separating patients into ypN2 and ypN3 stages. Factors predicting occult lymph node metastasis in completely resected lung adenocarcinoma of 3 cm or smaller [THORACIC]
CONCLUSIONS The presence of micropapillary or solid pattern, as well as micropapillary/solid predominant pattern, is significantly associated with occult N2 lymph node metastasis in lung adenocarcinoma. Radical mediastinal lymph node dissection may help to identify occult lymph node metastasis in these patients. (Source: European Journal of Cardio-Thoracic Surgery)

Microscopic N2 disease exhibits a better prognosis in resected non-small-cell lung cancer [THORACIC]
CONCLUSION Microscopic N2 seems to be associated with better prognosis in patients with pIIIA-N2 NSCLC and these could benefit from a simple follow-up. Prospective cohort studies are necessary to confirm these preliminary results. (Source: European Journal of Cardio-Thoracic Surgery)

Renal cell carcinoma with thrombus extending to the hepatic veins or right atrium: operative strategies based on 41 consecutive patients [THORACIC]
CONCLUSION High-level RCC tumour thrombus is a rare clinical entity, the treatment of which is complex and requires dedicated operative teams. The operative technique should be tailored according to the level of extension and the extent of vena cava obstruction/occlusion of the tumour thrombus. Contemporary operative techniques may be conducted with excellent results. Mid-term survival is limited, supporting the necessity to pursue research efforts towards establishing effective adjunct therapies. (Source: European Journal of Cardio-Thoracic Surgery)

The management of the diaphragm during radical surgery for malignant pleural mesothelioma [THORACIC]
CONCLUSIONS In our cohort, there was no evidence of diaphragmatic invasion in over 30% of patients, and we have also failed to find evidence that peritoneal disease progression affects overall survival following radical management. It may therefore theoretically be unnecessary to resect the diaphragm in all cases, and a pleurectomy–decortication could suffice. However, there is an unknown risk of R2 resection which would prejudice survival, and as such we would advocate resecting the diaphragm in all cases to avoid an R2 resection. Surgical experience in the rehabilitation and reimplantation of disconnected pulmonary arteries and its effectiveness in restoring pulmonary haemodynamics and function [CONGENITAL]
CONCLUSIONS Our study demonstrated that, in paediatric age group, disconnected PAs can be restored with low risk and excellent outcomes even in patients with severely hypoplastic PA. Strategies to repair intracardiac defects to a single lung might not be the optimal approach. (Source: European Journal of Cardio-Thoracic Surgery)

A staged decompression of right ventricle allows growth of right ventricle and subsequent biventricular repair in patients with pulmonary atresia and intact ventricular septum [CONGENITAL]
CONCLUSIONS Tricuspid valve growth was not obtained by modified BTS with pulmonary valvotomy; therefore, tricuspid valve size at birth appeared to be a predictor for achieving BVR. Proportionate RV growth was seen only in patients who achieved BVR. However, RV growth was not seen in patients having 1 + 1/2 ventricular repair. Major RV-coronary artery fistula was a strong predictor for proceeding single-ventricle palliation. (Source: European Journal of Cardio-Thoracic Surgery)

Long-term outcomes of reoperations following repair of partial atrioventricular septal defect [CONGENITAL]
CONCLUSIONS The most common cause for reoperation following pAVSD repair was LAVV regurgitation through the LAVV cleft. Reoperation is performed with survival comparable to that of primary pAVSD repair, yet the rate of further reoperations remains high. The patch augmentation technique for LAVVR has significantly increased the rate of successful LAVV repair. (Source: European Journal of Cardio-Thoracic Surgery)

Genetic variations of phenprocoumon metabolism in patients with ventricular assist devices [TRANSPLANTATION AND MECHANICAL CIRCULATORY SUPPORT]
CONCLUSIONS VKORC polymorphism affects PC dosage in the initiation as well as the maintenance phase. High rates of bleeding complications and thromboembolic events were found at the beginning of PC therapy in VAD patients. Therefore, a genotype-guided dosage algorithm might be useful in VAD patients. (Source: European Journal of Cardio-Thoracic Surgery)
