Long-term Outcome of Radical Excision Versus Phenolization of the Sinus Tract in Primary Sacrococcygeal Pilonidal Sinus Disease: A Randomized Controlled Trial
CONCLUSIONS: Because of the previously shown favorable short-term results and the currently reported comparable long-term recurrence rate and quality of life between phenolization and excision, phenolization should be considered the primary treatment option in patients with pilonidal sinus disease. See Video Abstract at http://links.lww.com/DCR/C27. Dutch Trial Register ID: NTR4043. Ileoanal Pouch Syndrome Is Common and Associated With Significant Disability in Patients With Ulcerative Colitis Undergoing IPAA
This study was conducted in a tertiary center. PATIENTS: Data of 128 UC-related IPAA from October 2014 to May 2021 were collected. MAIN OUTCOME MEASURES: Primary outcomes were prevalence of IPS. RESULTS: One hundred twenty-eight patients were enrolled with a median postoperative follow-up of 2.64 (IQR, 1.31–3.80) years. The prevalence of IPS and its constituent symptoms and consequences are lower for patients with longer follow-up after ileostomy reversal. Fecal incontinence and pad usage had the greatest impact on the quality of life affecting 29% and 31% of patients. IPS group had a significantly h...

Bowel Functional Outcomes After Long-Course or Short-Course Radiation for Advanced Rectal Cancers: A Propensity-Matched Analysis
This study was conducted at a single tertiary cancer center. Patients were operated on between 2014 and 2020. PATIENTS: The study included patients with locally advanced, nonmetastatic, mid, and low rectal cancers who underwent low anterior resection with stapled anastomosis and diverting ostomy. Extended or beyond total mesorectal excisions and lateral node dissections were excluded. INTERVENTIONS: Long-course radiation delivered as a radiation dose of 50 Gy in 25 fractions or short-course radiation (5 Gy in 5 fractions) and delayed surgery after 4 to 6 weeks with or without chemotherapy. MAIN OUTCOME MEA...

A Rectal Cancer Surgical Decision Aid Is Not Enough: A Qualitative Study
CONCLUSION: Rectal cancer survivors, their caregivers, and colorectal surgeons wanted an educational support tool that would address substantial educational needs through the continuum of disease rather than a surgical decision aid focusing on a discrete surgical choice only. Differences in Surgical Outcomes and Quality-of-Life Outcomes in Pelvic Exenteration Between Locally Advanced Versus Locally Recurrent Rectal Cancers
This study aimed to compare surgical outcomes and identify any differences or predictors of quality of life of patients with locally advanced rectal cancer and locally recurrent rectal cancer undergoing pelvic exenteration. DESIGN: This was a cohort study. SETTING: This study was conducted at Royal Prince Alfred Hospital, Sydney, Australia. PATIENTS: This study included patients with locally advanced rectal cancer and locally recurrent rectal cancer who underwent pelvic exenteration between July 2008 and March 2019. Case-Matched Comparison of Functional and Quality of Life Outcomes of Local Excision and Total Mesorectal Excision Following Chemoradiotherapy for Rectal Cancer
BACKGROUND: Neoadjuvant chemoradiotherapy followed by radical surgery is the standard curative treatment for mid- to low-rectal cancer. However, the combination of these treatments may affect patients’ GI and genitourinary functions and their quality of life. In the cases of good clinical response to neoadjuvant treatment, local excision is a rectal sparing strategy that would limit these side effects. OBJECTIVE: The aim of this study is to compare the effects of local excision or conventional laparoscopic total mesorectal excision after chemoradiotherapy in patients with rectal cancer. DESIGN: The desi...

“Their Whole Lives Are Going To Change”: A Photo-Elicitation Study of Rectal Cancer Survivorship
CONCLUSIONS: This photo-elicitation study provided rectal cancer survivors with a unique opportunity to share how rectal cancer treatment has impacted their lives beyond traditional metrics such as postoperative complications and long-term survival. In addition to improving preoperative counseling and expectation setting, future efforts should focus on postoperative symptom surveillance and appropriate referral when needed. The Role of Preoperative Imaging in the Detection of Lateral Lymph Node Metastases in Rectal Cancer: A Systematic Review and Diagnostic Test Meta-analysis
CONCLUSIONS: For the preoperative identification of lateral lymph node metastasis in rectal cancer, this review found compelling evidence that pelvic MRI should constitute the imaging modality of choice. In contrast, to confirm the presence of lateral lymph node metastasis, 18F-FDG-PET/MRI modalities allow discarding false positive cases because of increased specificity. Acute Colonic Pseudo-Obstruction
CASE SUMMARY: A 33-year-old man with a history of chronic alcohol use, generalized anxiety disorder, and hypertension presented to the emergency department after a syncopal event. He was admitted to the medical intensive care unit for alcohol withdrawal, requiring intubation and sedation. On hospital day 7, abdominal x-ray image demonstrated a dilated cecum to 12 cm, transverse colon dilation to 7 cm, and decompressed distal colon (Fig. 1). CT scan of the abdomen and pelvis confirmed dilation of the cecum and ascending and transverse colons (Fig. 2). Colonoscopy showed no evidence of distal obstruction, but colonic...
