An update on atypical large glandular proliferations of the prostate
The differential diagnosis of prostatic atypical large gland proliferations includes several benign and malignant entities. This review focusses on issues relevant to the practising pathologist, particularly around areas of controversy such as high-grade prostatic intraepithelial neoplasia (HGPIN) and intraductal carcinoma of the prostate (IDCP). HGPIN is a putative precursor of prostate cancer, but its clinical relevance is as a surrogate marker of unsampled prostate cancer, thereby identifying patients who would benefit from a prompt repeat biopsy. Updates in the pathology of penile intraepithelial neoplasia
Penile intraepithelial neoplasia (PeIN) is the putative histologic precursor of penile squamous cell carcinoma. PeIN is classified into HPV-associated and HPV-independent (differentiated) (dPeIN). While HPV-associated PeIN is has been linked to the oncogenic effect of human papillomavirus (HPV), the HPV-independent pathway is driven by chronic inflammatory conditions. These two biologic pathways are associated with distinct histopathologic features. The most common morphologic patterns of HPV-associated PeIN are basaloid, warty, and mixed PeIN. Medical renal mentionables: minimal change disease, membranous nephropathy and monoclonality
Advances in our understanding and ability to diagnose medical renal disease occur daily. Herein is described the recent identification of anti-nephrin antibodies in a subset of patients with minimal change disease, how the application of laser microdissection and mass spectrometry has identified multiple new antigenic targets in membranous nephropathy, and how the application of a new immunofluorescence technique targeting the junction of heavy chain and light chain constant regions may assist in determining the monoclonal nature of renal disease. Burkitt-like lymphoma with 11q aberration in a patient with chronic lymphocytic leukaemia: a potentially unrecognised association
We report a case of a 68-year old male with a previous history of CLL, who presented with an obstructing caecal mass. Microscopy revealed high grade features resembling Burkitt lymphoma; however, no MYC rearrangement was detected and subsequent FISH studies detected 11q aberration. Although this profile is now a recognized entity in the revised 4th edition of the WHO classification of Tumours of Haematopoietic neoplasms as “Burkitt-like lymphoma with aberration in 11q”, this has not previously been reported in patients with previous or concurrent CLL. Salivary neoplasia cytopathology: a logical diagnostic approach to cytological interpretation using histopathology knowledge
Fine needle aspiration cytology (FNAc) plays a vital investigative tool in the diagnostic and management pathway of the benign and malignant salivary pathologies. The current practice guidelines dictate acquirement of the nature of the salivary gland disease prior to embarking on definitive surgery. FNAc is a rapid, relatively easy, non-invasive diagnostic tool with minimal complications that achieves this goal. The diagnostic adequacy and the accuracy of FNAc sampling depends on a variety of factors including the operator aptness, lesional characteristics and patient factors. This article provides a broad overview of oral potentially malignant disorders (OPMD) and associated entities. The review opens with an update on the terminology surrounding OPMD before summarizing the lesions and conditions currently recognized as OPMD by the World Health Organization (WHO). Approaches to diagnosing oral epithelial dysplasia (OED) are discussed in the context of the WHO's recently modified architectural and cytological criteria for OED. Current concepts in PD-L1 testing in head and neck squamous cell carcinoma: overview, developments and challenges
Cancer immunotherapy is arguably the biggest success story of personalized medicine in the past two decades. Monoclonal antibodies targeting immune checkpoint inhibitors like PD-1 have shown success in clinical trials in a variety of solid tumours. The histopathologist has a central role in determining patient eligibility for immunotherapy by virtue of the histological assessment of tumours and their characterization of the tumour immune microenvironment. A case of sclerosing odontogenic carcinoma: a rare entity
Sclerosing odontogenic carcinoma (SOC) is a very rare entity, first recognized in the WHO 4th edition (2017). It is a highly infiltrative intraosseous jaw tumour with small nests, strands and cords of cells showing only mild cytological atypia, surrounded by a dense fibrocollagenous stroma. The lesion frequently infiltrates adjacent tissues and bony structures and demonstrates perineural and intraneural invasion.
