Why prescribe exercise as therapy in type 2 diabetes? We have a pill for that!
Abstract
The majority of T2D cases are preventable through a healthy lifestyle, leaving little room for questions that lifestyle should be the first line of defense in the fight against the development of T2D. However, when it comes to the clinical care of T2D, the potential efficacy of lifestyle is much less clear‐cut, both in terms of impacting the pathological metabolic biomarkers of the disease, and long‐term complications. A healthy diet, high leisure‐time physical activity and exercise are considered to be cornerstones albeit adjunct to drug therapy in the management of T2D. The prescription and effective imple...

Micro ‐environment and Intracellular Metabolism Modulation of Adipose Tissue Macrophage Polarization in Relation to Chronic Inflammatory Diseases
ConclusionsObesity‐associated metabolic dysfunctions, including those of glucose, fatty acid, cholesterol and other nutrient substrates such as vitamin D and iron in AT, promote the pro‐inflammatory polarization of ATMs and AT inflammation via regulating the interaction between ATMs and adipocytes and intracellular metabolic pathways, including glycolysis, fatty acid oxidation (FAO), and reverse cholesterol transportation (RCT). Focusing on the regulation of ATM metabolism will provide a novel target for the treatment of obesity‐related chronic inflammatory diseases, including insulin resistance, cardiovascular disea...

Circulating PIIINP levels identify adipose tissue ‐associated inflammation in individuals with type 2 diabetes mellitus with and without non‐alcoholic fatty liver disease.
ConclusionsGreater circulating PIIINP levels specifically identify T2DM individuals with AT expansion and systemic pro‐inflammatory profile suggestive for AT dysfunction; our results point towards a new role of PIIINP as a marker of fibro‐inflammation in dysmetabolic conditions, likely related to AT expansion. Lessons learned from the 1 ‐hour post‐load glucose level during the OGTT ‐ Current screening recommendations for dysglycemia should be revised
Abstract
This perspective covers a novel area of research describing the inadequacies of current approaches for diagnosing dysglycemia and proposes that the 1‐hour post‐load glucose level during the 75 gram OGTT may serve as a novel biomarker to detect dysglycemia earlier than currently recommended screening criteria for glucose disorders. Considerable evidence suggests that a 1‐hour post‐load plasma glucose value >155 mg/dl (8.6 mmol/l) may identify individuals with reduced β‐cell function prior to progressing to prediabetes and diabetes and is highly predictive of those likely to progress to diabetes more t...

Glucose effectiveness is a critical pathogenic factor leading to the emergence of glucose intolerance and type 2 diabetes mellitus: an ignored hypothesis.
ConclusionIn “at risk” individuals, a low GE and genetically determined vulnerable β‐cell function are more critical determinants of future glucose intolerance than their preexisting insulin resistant state. (Source: Diabetes/Metabolism Research and Reviews)

Methylation of Tcf712 promoter by high ‐fat diet impairs β‐cell function in mouse pancreatic islets
ConclusionAn HFD was shown to induce aberrant methylation of the Tcf7l2 promoter in mouse islets, which resulted in diminished gene expression. This study provides an evidence of the association between nutrient consumption and gene expression. (Source: Diabetes/Metabolism Research and Reviews)

Association between sociodemographic determinants and health outcomes in individuals with type 2 diabetes mellitus in Sweden
ConclusionsDiabetes patients' health was associated with sociodemographic prerequisites. In addition to demographics (age, sex) and disease history; educational level, marital status and region of birth are important factors to consider when benchmarking health outcomes, e.g. average HbA1c level, between organizational units or between different administrative regions. Fast publication, a must for a journal
Abstract
As Diabetes/Metabolism: Research & Reviews (DMRR) begins its transition to a new editorial team, incoming Editor‐in‐Chief, Dr Paolo Pozzilli, reflects on the lessons learned from the past editorship and envisions the future of the journal. In today's fast‐paced world, a medical journal should be fast in communicating decisions. Authors are eager to publish their work as fast as possible, and a journal must guarantee this, whilst at the same time ensuring a rigorous peer‐review process. With this in mind, we hope to help the diabetes scientific community. (Source: Diabetes/Metabolism Research and Reviews)
