Effect of metabolic control on oxidative stress, subclinical atherosclerosis and peripheral artery disease in diabetic patients.
CONCLUSIONS: Our study showed poor glycemic control leads to worse CIMT by increasing age and duration of diabetes. However we did not find a significan correlation between glycemic status and TAS levels. We suggest CIMT measurement along with other SA markers in poor glycemic diabetics, especially in older patients with longer duration of diabetes, to identify high risk CVD patients. Reduced frequency and severity of ketoacidosis at diagnosis of childhood type 1 diabetes in Northwest Saudi Arabia.
CONCLUSIONS: In NWSA, the frequency and severity of DKA at onset of childhood T1D were reduced following 4 years awareness campaign; but the rate is still high. Maintaining the campaign may result in further improvement following a longer period of observation. PMID: 26565539 [PubMed - as supplied by publisher] (Source: Diabetes Metab)

Cardiorespiratory Fitness and Light-Intensity Physical Activity Are Independently Associated with Reduced Cardiovascular Disease Risk in Urban Black South African Women: A Cross-Sectional Study.
CONCLUSION: Both physical activity and cardiorespiratory fitness were associated with reduced total and central fat mass, VAT, and reduced cardiometabolic risk for CVD and T2D. Longitudinal studies are required to confirm whether the promotion of increasing light physical activity, while reducing sedentary time and increasing cardiorespiratory fitness, reduces the risk for obesity, CVD and T2D. Risk Stratification in Women with Gestational Diabetes According to and Beyond Current WHO Criteria.
Abstract The pregnancy outcomes in women with gestational diabetes mellitus (GDM) and 'overt diabetes in pregnancy' were compared and the need for further subclassification was investigated with respect to postpartum outcome risk. Data from 944 women who had been uniformly diagnosed as having GDM in Munich, Germany, between 1998 and 2010, were re-classified into GDM and 'overt diabetes in pregnancy'. Pregnancy related outcomes in the offspring were derived from Bavarian birth registry data. Classification and regression trees were used to identify further GDM sub-phenotypes. In total, 88 women (9.3%) were ...

Severe paraneoplastic hypoglycemia secondary to a gastrointestinal stromal tumour masquerading as a stroke.
We report the case of a 70-year-old previously healthy female who presented acutely to the Accident and Emergency department with left-sided vasomotor symptoms including reduced muscle tone, weakness upon walking and slurred speech. Physical examination confirmed hemiparesis with VIIth nerve palsy and profound hepatomegaly. A random glucose was low at 1.7 mmol/l, which upon correction resolved her symptoms. In hindsight, the patient recalled having had similar episodes periodically over the past 3 months to which she did not give much attention. While hospitalized, she continued having episodes of symptomatic hypoglycaem...

Transcription Factor-7-Like 2 Gene Variants Affect the Metabolic Phenotypes of Polycystic Ovary Syndrome.
CONCLUSIONS: Although individual TCF7L2 variants were not associated with the presence of PCOS in Bahraini women, specific TCF7L2 haplotypes were identified, which were both positively and negatively associated with PCOS. PMID: 26536002 [PubMed - in process] (Source: Diabetes Metab)

Short-term, high-fat diet accelerates disuse atrophy and protein degradation in a muscle-specific manner in mice.
CONCLUSIONS: Taken together, a short-term, high-fat diet augments denervation muscle atrophy by induction of protein degradation in the mitochondria-rich soleus but not in the glycolytic EDL. These findings suggest that the denervation-induced loss of mitochondria and HF diet-induced impairment of mitochondrial function may combine to promote skeletal muscle atrophy. 17-hydroxylase/17,20-lyase deficiency due to a R96Q mutation causing hypertension and poor breast development.
We report an Arab family with four affected XX siblings. The eldest presented with abdominal pain and was diagnosed with a retroperitoneal malignant mixed germ cell tumour. She was hypertensive and hypogonadal. One sibling presented with headache due to hypertension while the other two siblings were diagnosed with hypertension on a routine school check. A homozygous R96Q missense mutation in P450c17 was detected in the index case who had primary amenorrhea and lack of secondary sexual characters at 17 years. The middle two siblings were identical twins and had no secondary sexual characters at the age of 14. All siblings h...

Effect of the EndoBarrier Gastrointestinal Liner on obesity and type 2 diabetes: Systematic review and meta-analysis.
Abstract In contrast to bariatric surgery, less invasive and reversible techniques to counteract obesity and type 2 diabetes (T2D) have been developed, including the EndoBarrier Gastrointestinal Liner (duodenal-jejunal bypass sleeve (DJBS)). We conducted a systematic review with meta-analyses of eligible trials to evaluate the efficacy and safety of DJBS. Five randomized controlled trials (n = 235 subjects) and 10 observational studies (n = 211 subjects) were included. Risk of bias was evaluated as high in all studies. Mean body mass index ranged from 30 to 49.2 kg/m(2) and 10-100% of the subject...

Patient-Reported Outcomes Among Type 2 Diabetes Mellitus Patients Treated with Dapagliflozin in Triple Therapy Regimen for 52 Weeks.
Abstract Patients with T2DM and inadequate glycemic control on combination metformin (MET) and sulfonylurea (SU) were enrolled in a 24-week, double-blind, randomized, placebo-controlled study with a 28-week extension. The EQ-5D, SHIELD Weight Questionnaire-9 (WQ-9), Impact of Weight on Quality of Life-Lite (IWQOL-Lite), and Diabetes Treatment Satisfaction Questionnaire (DTSQ) evaluated health status and health-related quality of life (HRQOL) at baseline and week 52. Patients with dapagliflozin 10 mg + MET + SU (n=108) were compared with patients treated with placebo + MET + SU (n=108), using repeated measu...

Use of Incretin Agents and Risk of Pancreatic Cancer: A Population-Based Cohort Study.
CONCLUSIONS: We found that incretin use was not associated with pancreatic cancer after adjustment for the severity of the underlying T2DM. The elevated risk of pancreatic cancer in recent starters of incretin agents is likely to be caused by protopathic bias or other types of unknown distortion. The presence of considerable confounding by disease severity and the lack of duration of use relationship do not support a causal explanation for the association between incretin agents and pancreatic cancer. Cost-effectiveness of a national population-based screening program for type 2 diabetes: the Brazil experience.
CONCLUSIONS: In the base case analysis, not considering the intangible benefit of transferring diabetes management to primary care nor the benefit of using statin to treat eligible diabetic patients, CE ratios were not cost-effective considering thresholds proposed by the World Health Organization. However, significant uncertainty was demonstrated in sensitivity analysis. Our results indicate that policy-makers should carefully balance the benefit and cost of the program while considering using a population-based approach to screen for diabetes.
