The role of arthroscopy in diagnostics and treatment of arthritis of the ankle joint
Unfallchirurg. 2022 Jan 18. doi: 10.1007/s00113-021-01133-0. Online ahead of print.ABSTRACTThe arthroscopic treatment of arthritis of the ankle joint is very limited and is only indicated for early stages of arthritis with impingement, strictly localized forms and more for young patients. The most important aim of arthroscopy is to improve the range of motion (ROM) of the ankle by removal of bony projections (dorsal and ventral) and an extensive synovectomy with debridement. In cases of narrowing of the joint space to less than 2 mm and/or malalignment, the indications for arthroscopy should be considered very cautiously. ...

Conservative treatment options for arthritis of the ankle : What is possible, what is effective?
Unfallchirurg. 2022 Jan 18. doi: 10.1007/s00113-021-01122-3. Online ahead of print.ABSTRACTIn this article the causes of arthritis in the region of the ankle are introduced and the conservative treatment options are described and discussed more extensively. The risks of treatment with nonopioid analgesics (NOPA) are presented in detail. The topical use of nonsteroidal anti-inflammatory drugs (NSAID) should always be considered in the clinical routine. If contraindications for oral NSAIDs are present, intra-articular treatment is a meaningful option. The best evidence is currently available for viscosupplementation but the ...

E-scooter, e-bike and bicycle injuries in the same period-A  prospective analysis of a level 1 trauma center
CONCLUSION: The three patient collectives examined showed different causes and profiles of injuries. The reasons for an increased proportion of seriously injured people compared to cyclists are electromobility, driving under the influence of alcohol and inadequate wearing of a helmet on e‑scooters when head injuries dominate. 73.5% (n = 50) of the e‑scooter accidents recorded by us were not registered by the police and therefore do not appear in the current statistics of the statistisches Bundesamt. As a result, a much higher number of e‑scooter accidents can be assumed. Preventive measures could include the introduc...

Survival of proximal femoral fractures vs. mean life expectancy of the general population : Retrospective analysis of 1000 geriatric patients minimum 10  years after operation
CONCLUSION: Geriatric patients with PF have significantly shorter life expectancy than the general population evaluated by the German Federal Statistical Office. This reflects fragility and morbidity of patients with PF.PMID:35024871 | DOI:10.1007/s00113-022-01140-9 (Source: Der Unfallchirurg)

Retropharyngeal tendinitis : Rare differential diagnosis of acute neck pain
Unfallchirurg. 2022 Jan 13:1-5. doi: 10.1007/s00113-021-01138-9. Online ahead of print.ABSTRACTAtraumatic neck pain is a frequent reason for patients to attend the emergency department (ED). It remains a challenge for the treating physician to rule out possible life-threatening causes, such as a retropharyngeal abscess, meningitis or septic spondylodiscitis. Herein, we report the rare case of a patient suffering from retropharyngeal tendinitis. This rare cause of atraumatic neck pain is characterized by a significant decrease in the range of motion of the cervical spine, elevated infection parameters and pathognomonic MRI ...

Contrast-enhanced ultrasound in the diagnostics of non-unions
Unfallchirurg. 2022 Jan 7. doi: 10.1007/s00113-021-01129-w. Online ahead of print.ABSTRACTNon-union represents a severe complication and a major socioeconomic challenge in orthopedics and trauma surgery. Timely and reliable diagnostics are obligatory to be able to carry out the treatment of non-unions in a patient-specific and efficient manner. Contrast-enhanced ultrasound (CEUS) is an important interface between clinical signs, imaging investigations and the results of the paraclinical diagnostics, e.g. C‑reactive protein (CRP) and leukocyte count. It can display the microperfusion inside the non-union gap in real time ...

Interventional sonography
Unfallchirurg. 2022 Jan 6. doi: 10.1007/s00113-021-01130-3. Online ahead of print.ABSTRACTInterventional sonography describes the step from pure sonographic imaging diagnostics to sonographically controlled treatment that can be connected directly to sonographic imaging diagnostics instead of postponing it over a longer time interval with possibly further (X-ray, computed tomography, magnetic resonance imaging) diagnostics. The sonographically controlled interventional measures range from a simple puncture of fluid-filled spaces to infiltration of deeper lying areas, such as the labrum acetabulare, the dorsal knee joint ca...
