Safety of intra-articular platelet rich plasma injections for large joint osteoarthritis: a review article
Platelet-rich plasma (PRP) use in intraarticular injections is thought to be potentially efficacious in the treatment of osteoarthritis (OA) and as an alternative to corticosteroid injections. However, little is known about the safety of PRP usage in the treatment of large joint osteoarthritis. In the 21 identified studies, there were primarily minor adverse effects include pain, redness, swelling, nausea, and dizziness. The limitations of this review include the relative paucity of well-designed studies that describe detailed adverse effects using safety as an outcome measure. Intraarticular injection of platelet-rich pla...

Intraarticular hip preservation procedures in healed Perthes disease: a systemic review and meta-analysis
Conclusions: FHRO and RNFL are effective and safe modalities for post-Perthes sequelae. However, the postoperative clinical outcomes were observed to be in favor of RFNL, with less incidence of postoperative complications compared to the FHRO. Moreover, the FHRO group needed more secondary acetabular procedures than the RFNL group. On the other hand, FHRO showed greater improvements in radiographic coverage indices. Gender and outcomes in total joint arthroplasty: a systematic review on healthcare disparities in the United States
Background: The authors evaluated the currently available evidence regarding the effect gender has, if any, on utilization of and outcomes after total knee (TKA) and hip (THA) arthroplasty. Recent studies have shown that TKA and THA are not utilized equally across different demographic cohorts. Methods: A comprehensive search of MEDLINE database was performed using keywords “disparities,” “arthroplasty,” “joint replacement,” “hip,” “knee,” “gender,” and “outcomes” in all possible combinations. All English studies with levels of evidence I to IV published over the last 20 yr were incl...

Up to 4 millimeters excessive distal femoral resection in total knee arthroplasty has no significant effect on patellar height and functional score in patient with severe flexion contracture: a retrospective study
Conclusions: An excessive distal femoral cut does not seem to change patellar height in TKA patients, so it could be used safely for patients with a severe preoperative flexion deformity. Level of Evidence: Level IV. (Source: Current Orthopaedic Practice)

Orthopaedic surgery residency match trends during COVID-19 pandemic: a cross-sectional study
Conclusions: This increase is likely due in part to the familiarity of students to programs where they could still rotate during the pandemic. Level of Evidence: Level IV. (Source: Current Orthopaedic Practice)

A multicenter retrospective analysis of risk factors for poor outcomes after tibial pilon fractures
Conclusions: Increased caution may be warranted when treating tibial pilon fractures in patients with certain risk factors. In patients with a history of diabetes, the additional dissection needed to place more implants may contribute to higher rates of infection. Level of Evidence: Prognostic Level III. Two-year outcome comparison of custom versus conventional total knee (TKA) implants: a retrospective cohort study
Conclusions: At a minimum follow-up of 2 years, the authors found no significant differences in patient-reported outcome measures including the OKS, SF-12 mental and physical scores between custom and conventional TKAs. Additionally, there were no significant differences in complication or revision rates. Level of Evidence: Level III. (Source: Current Orthopaedic Practice)

Comparison of the efficacy of physiotherapy, subacromial corticosteroid, and subacromial hyaluronic acid injection in the treatment of subacromial impingement syndrome: a retrospective study
This study evaluated the efficacy of subacromial corticosteroid, hyaluronic acid injection and physiotherapy in patients with SIS to determine which treatment is most effective. Methods: SIS patients treated with either physiotherapy or subacromial injection of triamcinolone (40 mg), or subacromial hyaluronic acid injection were included in this retrospective study. The outcome measures were evaluated 3 and 6 mo after the intervention and included pain evaluated by a visual analog scale (VAS) and limb function assessed by the Disabilities of the Arm, Shoulder, and Hand (DASH) and American Shoulder and Elbow Surgeo...

Variation in core clerkship grading reported on the Medical Student Performance Evaluation (MSPE) for orthopaedic surgery applicants: a retrospective review
Background: Heterogeneity of grading schema among medical schools complicates utility of core clerkship grades reported. The purpose of this study was to understand the variation in number and verbiage in grading schemes during third-year core clerkships for orthopaedic surgery residency applicants. Methods: Applications to a single institution’s orthopaedic surgery residency program during the 2017-2018 match cycle were reviewed. Data extracted from the Medical Student Performance Evaluation (MSPE) included medical school name, number of core clerkships, grade options/tiers, and % grade distribution in clerks...

Standardized protocol for hip fracture care leads to similar short-term outcomes despite socioeconomic differences in patient populations: a retrospective cohort study
Conclusions: There was a similar length of stay, complication rate, and mortality rate after hip fracture surgery despite demographic differences between the groups. A fragility fracture protocol can lead to similar outcomes in patients of differing demographics and insurance payor mixes. Level of Evidence: Level III. (Source: Current Orthopaedic Practice)

Temporary hemiepiphysiodesis for correction of genu valgum due to cystinosis: a preliminary interventional study in children
Conclusions: Temporary hemiepiphysiodesis is a reproducible, efficient, and safe approach for correction of genu valgum in patients with cystinosis with few complications in children. Level of Evidence: Level III. (Source: Current Orthopaedic Practice)

Functional outcome of proximal fibular osteotomy in knee osteoarthritis: a prospective study
Background: Osteoarthritis of the knee joint causes debilitating pain that hampers the daily activities of affected patients. Proximal fibular osteotomy provides an option in the disease’s early stages to prolong the life of the native joint. Methods: All consenting patients with diagnosed osteoarthritis of the knee were enrolled in the study. Preoperative radiographic and clinical assessment with functional scores was conducted. Proximal fibular osteotomy was performed at a distance of 6 to 8 cm from the fibular head. Osteotomy was done and postoperative functional scores (Visual Analogue Score, Knee Society...

Shoulder MRI lag times after implementation of the Affordable Care Act (ACA): a retrospective review
This study was initiated to evaluate how the timing of MRI completion for shoulder pathology correlates with implementation of the ACA by comparing lag times between the initial visit and the date of MRI completion. In addition, the access to orthopaedic care after ACA implementation was evaluated by comparing shoulder MRI lag times among three cohorts based on insurer status. Methods: All shoulder and proximal upper extremity MRIs between 2009 and 2017 were reviewed, 5 yr before and 3 yr after ACA implementation. Patients were grouped into commercial, Medicare, and Medicaid cohorts. Average lag times for the pre-...

Conversion total hip arthroplasty after previous intertrochanteric hip fractures: a review of the epidemiology, technical issues, and outcomes
Conversion total hip arthroplasty (THA) after previous intertrochanteric (IT) hip fracture is a unique operation that is different from the routine primary total hip arthroplasty (THA) done for osteoarthritis or femoral head necrosis. Conversion THAs are similar to revision THAs with regard to complexity, technical challenges, implant costs, and perioperative complications. A literature review was performed with specific focus on the surgical techniques, patient outcomes, and financial burdens of conversion THAs after IT fracture treatment with either a sliding compression hip screw with side plate (SSP), or a cephalomedul...
