Comparison of preoperative and postoperative radiation therapy for extremity soft-tissue sarcoma: a randomized clinical trial
In this study, the authors compared preoperative and postoperative radiotherapy in patients with extremity STS. Methods: Eighty patients with localized extremity STS were randomly assigned to either preoperative or postoperative radiotherapy group. Baseline characteristics of the patients were not significantly different between the two study groups. Primary outcome measures were pain, acute complications (wound dehiscence and infection), and late complications (limb edema, subcutaneous fibrosis, and joint stiffness). The severity of late complications was evaluated using the radiation therapy oncology group criteri...

The use of modular total knee prostheses in femoral defects in traumatic and revision patients: a prospective case series
Conclusions: Short-term clinical outcomes for the use of modular endoprosthetic total knee implants in the management of distal femoral defects in either traumatic injury or revision surgery result in good functional outcome and patient satisfaction. Level of Evidence: Level III. (Source: Current Orthopaedic Practice)

Risk factors for symptomatic hardware removal after plate fixation of olecranon fractures: a retrospective chart review
Conclusions: This study resulted in approximately 20% rate of symptomatic hardware necessitating removal. Patients who request removal tend to be younger. Fixating the plate closer to the olecranon tip may decrease the incidence of symptomatic hardware and need for subsequent removal. Level of Evidence: Level III. (Source: Current Orthopaedic Practice)

Functional outcome after dual mobility cups total hip replacement versus bipolar hemiarthroplasty in femoral neck fractures in active elderly patients: a randomized controlled trial
This study was a randomized comparative interventional single-blinded study that was performed at Ain Shams University Hospitals. A total of 33 patients underwent DMTHA or BA after displaced FNF. Primary outcomes were functional outcome and postoperative dislocation rate. Secondary outcomes were operative time and amount of blood loss. Results: There were no significant differences between the demographic data and preoperative parameters between the two groups. There was a significant difference in the postoperative Harris Hip Score (HHS) between the two groups (P=0.01). VAS of groin pain was significantly different...

Future of women orthopaedists: signs of optimism in a 3-year follow-up survey
Conclusions: Women have given many possible reasons for not choosing orthopaedic surgery, such as wanting a better work-life balance, perceptions of physical strength needed, a closed “boy’s club,” not enough exposure, and lack of female role models, yet most of them replied the field is “just not interesting.” The authors suggest in concordance to the literature that having more opportunity for exposure and more female role models would increase gender diversity in orthopaedics. Adult reconstruction call and prosthetic joint infection (PJI) management patterns across the United States: a cross-sectional study
Conclusions: There were a variety of models used by academic medical centers for triaging and management of PJI transfer patients. There does not seem to be one single algorithm universally accepted as optimum for initial management of patients. Level of Evidence: Level III. (Source: Current Orthopaedic Practice)

Evaluation and management of low-energy pelvic ring fractures in elderly patients: a narrative review
The incidence of pelvic ring fractures has shown a steady increase in elderly patients that has been caused by a rapidly aging population and increased life expectancy. Many of these injuries result from a low-energy mechanism such as a fall from standing height. Classification systems such as the Young and Burgess and Tile classifications commonly are used to describe these fractures. One of the major distinctions to be made is whether the fracture pattern is stable or unstable in order to determine treatment. The goal of treatment is to restore patients back to their preinjury level of function. Mortality rate has previo...

Genetic and other associations with pain: a literature review of potential identifiers for significant pain after total knee arthroplasty
Osteoarthritis of the knee is one of the leading causes of disability and work limitations in the United States, resulting in significantly decreased work productivity and loss of work. The definitive treatment of choice for end-stage osteoarthritis is total knee arthroplasty. However, up to 34% of patients who undergo TKA experience chronic pain. Long-term pain continues to pose a significant burden and directly undermines the goal in treatment of patients with osteoarthritis, and persistent postoperative pain is a major concern to the orthopaedic community. Finding ways to reduce the number of patients experiencing persi...

Factors associated with new persistent opioid use after ankle sprain: a cross-sectional analysis
Background: Chronic opioid use is a growing crisis in the United States. The authors examined the risk factors associated with new persistent opioid use in patients who had nonoperatively treated ankle sprains and offer evidence-based treatment recommendations for practitioners. Methods: A United States insurance claims database was used to select patients who underwent nonoperative treatment of an ankle sprain between January 2008 and December 2016. The authors examined opioid prescribing 1 yr before diagnosis and 6 mo after and defined new persistent opioid use as prescription fulfillment between 91 and 18...

Patients prefer physician-provided durable medical equipment over third-party vendors: a patient satisfaction survey
Conclusions: Patients receiving their DME in the office from their physician were significantly more likely to prefer using the in-office setting again and reported fit-scheduling to be easier when compared with patients using a third-party vendor. DME can be provided by physicians in the office setting without negatively impacting patient satisfaction. Level of Evidence: Level II. (Source: Current Orthopaedic Practice)

Factors that influence patients’ recommendations of orthopaedic surgeons: a cross-sectional study of a popular online rating website
Conclusions: Orthopaedic surgeons have generally favorable ratings and mostly positive comments. Knowledge of which surgeon-specific attributes are important to patients may provide valuable insight to orthopaedic surgeons on how to improve patient care and patient satisfaction as well as improve online review websites as reliable sources for evaluating physicians. Level of Evidence: Level IV. (Source: Current Orthopaedic Practice)
