Alternating bundle-branch block in acute coronary syndrome
We present a case of a patient with a known left bundle-branch block (LBBB) who developed a new right bundle-branch block (RBBB) in a setting of acute coronary syndrome. Coronary angiogram revealed subtotal proximal LAD stenosis that was resolved with stent implantation. After the percutaneous coronary intervention RBBB disappeared and LBBB reappeared. Use of OPCAB in Czechia 2010–2015: Have we learned any lessons?
ConclusionsThe prevalence of OPCAB in Czech Republic has decreased. However, it is unclear whether this is due to the recent widely respected prospective randomized clinical trials. Published papers have not shown the superiority of OPCAB in high-risk patients, yet Czech cardiac surgeons prefer this strategy, especially in patients with a history of renal insufficiency. Should we fear infarct-like myocarditis?
ConclusionInfarct-like myocarditis was the most common presentation of acute myocarditis during the observation time. No patient had an early recurrence of the disease (i.e., within 1 year) and no left ventricular (LV) systolic dysfunction was developed. Infarct-like myocarditis has a specific feature that differs from other types of myocarditis.
