Acute purulent pericarditis caused by Clostridium septicum
We describe the case of a 71-year-old man suffering from liver cirrhosis with an acute purulent pericarditis. The organism responsible for his pericarditis was Clostridium septicum; a Gram-positive obligatory pathogen, commonly associated with gastrointestinal tumours. Despite the prolonged, targeted antibiotic therapy, the infection resulted in endocarditis and the need for an urgent the aortic valve replacement. (Source: Cor et Vasa)

Surgical management of idiopathic constrictive pericarditis
Publication date: Available online 31 March 2017Source: Cor et VasaAuthor(s): Serkan Burç Deşer, Semih Murat YücelAbstractConstrictive pericarditis is defined as impaired diastolic cardiac function leading to heart failure, progressive fluid overload, systemic venous congestion and reduction of cardiac output. Both ventricles affected by the constriction; however, symptoms of right heart failure dominate. The congestive heart failure and normal left ventricular systolic functions are seen in the presence constrictive pericarditis. Pericardiectomy is considered as the mainstay treatment method for constrictive pericardit...

Anomalous left circumflex artery occlusion: A technical challenge in primary percutaneous coronary intervention?
Publication date: Available online 31 March 2017Source: Cor et VasaAuthor(s): Giuseppe Andò, Vittorio Virga, Olimpia Trio, Alessandro Di Giorgio, Francesco SaporitoAbstractAnomalies of the coronary arteries can be found in approximately 1% of patients undergoing coronary angiography. Most coronary anomalies do not result in signs, symptoms, or complications, and usually are discovered as incidental findings at the time of catheterization. Coronary anomalies may pose several challenges to the interventional cardiologist in the emergency setting, as anomalous vessels may be difficult to find or to selectively cannulate. The...

Cardiac implantable devices and Takotsubo syndrome. A rare but potential eventuality
We report the case of a female patient who suffered from Takotsubo cardiomyopathy associated to a coronary fistula after pacemaker implantation. Its occurrence should be always remembered by the implanters, avoiding it by an appropriate psychological screening and use of conscious sedation. (Source: Cor et Vasa)

Anomalous left coronary artery from the pulmonary artery: A rare case diagnosed in an adult
This article reports on a 45-year-old asymptomatic man with the ALCAPA diagnosed thanks to a detailed cardiac examination, that followed the finding of the left bundle branch block and a decreased left ventricular systolic ejection fraction. Although coronary angiography and computed tomography are considered to be the key diagnostic methods for ALCAPA, in this case the diagnosis was established based on echocardiography. Previously, the patient has been under regular review by a cardiologist with the incorrect diagnosis of multiple ventricular septal defects. In fact, this diagnosis resulted from a misinterpretation of th...

Cardiobacterium hominis and endocarditis. Rare but important clinical relevance
Publication date: Available online 25 May 2017Source: Cor et VasaAuthor(s): Annamaria Ioppolo, Claudia Morabito, Gessica D’Amico, Antonio Taormina, Emmanuele Soraci, Giuseppe DattiloAbstractInfective endocarditis is a severe disease with high morbidity rate. Cardiobacterium hominis (CH) is a member of the HACEK group of microorganisms. We show the case of a man of 56 years with infective endocarditis on a native mitral valve, sustained by C. hominis. The relevance of this case is based on the fact that infections sustained by CH, even if not so frequent (about 1–3% of all cases), are a possible cause of infective endoc...

Heart rate variability evaluation in the assessment of cardiac autonomic neuropathy in patients with type 2 diabetes
ConclusionAssessment of CAN using evaluation of HRV can optimally be performed (and simply realized in clinical practice) using SAHRV based on short ECG recordings during autonomic load imposed by orthoclinostatic test (Supine1–Standing–Supine2) and on postprocessing data analysis using complex indicator called “functional age of ANS”. In the detailed evaluation of sympathovagal balance, it complements the screening assessment with cardiovascular reflex tests (Ewing's battery). Besides the orthoclinostatic load, pronounced vagal provocation using rate-controlled, non-deepened breathing (12 cycles/min) represen...

Media sclerosis Mönckeberg affects microcirculation
Publication date: Available online 23 June 2017Source: Cor et VasaAuthor(s): P. Lanzer, C. ZouboulisAbstractMedia sclerosis Mönckeberg is known to be associated with progressive deposits of crystalline and amorphous hydroxyapatite within the media of large arteries. Here we provide evidence for calcifications within microcirculation in a 66-year-old patient with known and documented media sclerosis Mönckeberg of large peripheral arteries. (Source: Cor et Vasa)

Acute temponade due to postinfarction myocardial rupture successfully managed with urgent pericardiotomy
Publication date: Available online 8 July 2017Source: Cor et VasaAuthor(s): Shi-Min Yuan, Jin-Zao Chen, Yao-Hua Yu, Liang-Qing Lin, Xiang-Kui Fang, Jing LinAbstractPostinfarction myocardial rupture is rare and fatal. A 60-year-old female developed hemodynamic collapse after 2-h acute precordial pains. Acute tamponade and cardiogenic shock were confirmed. The patient underwent urgent pericardiotomy under the aid of intraaortic balloon pump and inotropic infusions. The differential diagnosis of acute chest pain is important. Early diagnosis and prompt surgical intervention of acute temponade are crucial for rescuing the pati...

Subclavian artery stenosis: An unusual cause of periprocedural myocardial infarction following the surgical myocardial revascularization
Publication date: Available online 19 July 2017Source: Cor et VasaAuthor(s): T. Lopuchovský, M. Moščovič, R. Novotný, A. Kolesár, Š. Lukačín, F. SabolAbstractThe introduced case report explains the atypical periprocedural myocardial infarction following the surgical myocardial revascularization. 60-year-old man has undergone the coronary bypass surgery with arterial graft of left mammary artery (LIMA) to left anterior descending artery (LAD) and venous graft to posterior interventricular branch of right coronary artery. Early in the post-surgery period a perioperative myocardial infarction (PMI) developed, with la...

Publication date: Available online 23 July 2017Source: Cor et VasaAuthor(s): Martyna Zaleska, Olga Możeńska, Mariola Laszuk, Piotr Suwalski, Dariusz Artur KosiorAbstractAndersen–Tawil syndrome (long QT syndrome 7) is a rare inherited disorder, characterized by periodic paralysis, long QT, ventricular arrhythmias and skeletal abnormalities.A 52-year-old female with a history of long QT-syndrome, ICD implantation (secondary prevention of cardiac arrest) and systemic vasculitis was admitted due to an electrical storm caused by endocarditis. She was admitted again short after discharge due to multi-organ failure, which was...
