Extracorporeal membrane oxygenation in a patient with yew and escitalopram intoxication
We present a case of a 25-year-old patient with yew and escitalopram ingestion in a suicide attempt. He presented to the emergency department of a regional hospital with cardiology center without cardiosurgery clinic, but with VA ECMO team on site. Pulseless ventricular tachycardia and fibrillation unresponsive to usual treatment developed soon after presentation and the patient had to be placed on VA ECMO during ongoing cardiopulmonary resuscitation using Lucas CPR device. Patient survived and was successfully weaned off VA ECMO after 4 days with normalization of cardiac function and without severe liver or kidney dysfunc...

MitraClip in patients with functional mitral regurgitation and advanced heart failure – Single centre experience
ConclusionOur experience shows that MitraClip implantation improves both symptoms and quality of life in selected patients with heart failure, systolic LV dysfunction and functional MiR. This method broadens the treatment options for patients who cannot undergo surgical repair because of valve defect characteristics and LV dysfunction. (Source: Cor et Vasa)

Heart rate variability in atrial septal defect both before and after operation
This article is a literature review of the data on the characteristics of HRV in patients with atrial septal defect (ASD), as well as the regularities of its dynamics after surgical and X-ray endovascular correction of the defect. The article also highlights possible pathophysiology of these vegetative abnormalities. (Source: Cor et Vasa)

Iatrogenic epinephrine-induced Takotsubo cardiomyopathy in beta-blocker poisoning: Case report
In conclusion, Takotsubo cardiomyopathy was probably triggered in the present case by the association of three etiologies: emotional stress, epinephrine infusion and beta-blocker poisoning. Physicians should be aware of possible iatrogenic triggers of this disease, especially the harmful effects of catecholamine on heart function and/or poisoning with cardiotoxic drugs. ST segment elevations caused by invasion of carcinoma of the thymus to the myocardium
Publication date: Available online 25 August 2018Source: Cor et VasaAuthor(s): Věra Müllerová, Jozef Jakabčin, Vladěna Skálová, Pavel ČervinkaAbstractPathological ST segment elevations in 12-lead ECG are in most cases caused by ischemic heart disease during acute transmural infarction. ST segment elevations in ECG can even have non-ischemic etiology during another cardiac disease and there are also rare causes of ST elevations of a non-cardiac origin. In this case report, the authors present the case of a 68-year-old male patient with carcinoma of the thymus, which was growing through to the cardiac structures and ...

Bridging anticoagulation therapy
Publication date: Available online 14 August 2018Source: Cor et VasaAuthor(s): Štefan Volovár, Radka Tancošová, Richard RokytaAbstractOral anticoagulation therapy is used in patients with various diagnoses to reduce the risk of thromboembolic events or to induce a hypocoagulation state to facilitate dissolution of a thrombus. In clinical practice we often encounter anticoagulated patients, many of whom have been diagnosed with nonvalvular atrial fibrillation. Each year a significant number of these patients undergo a medical procedure, which, in some cases, requires temporary discontinuation of anticoagulation therapy....

Single coronary artery with double coronary courses: Tips and tricks
Publication date: Available online 11 August 2018Source: Cor et VasaAuthor(s): Murat Gul, Ozcan Ozeke, Anil Ozen, Fatih Mehmet Ucar, Serkan Cay, Dursun ArasAbstractCongenital coronary artery anomalies are uncommon and the vast majority is diagnosed incidentally during coronary angiogram or necropsy. A single coronary artery is one of the most rarely seen coronary artery anomalies Determination of its course (inter-arterial, intra-septal, pre-pulmonic or retro-aortic) is very important regarding its clinical significance. Avoiding the misdiagnosis of an unsuspected anomalous coronary artery is critical to the patient and it...

Cardioembolic stroke as consequence of myocardial infarction: Case report and concise review
Publication date: Available online 11 August 2018Source: Cor et VasaAuthor(s): Ondřej Černý, Tomáš Roubíček, Jindřich Kupec, Rostislav PolášekAbstractDespite tremendous achievements in myocardial infarction therapy, thromboembolic complications remain very serious and its prognosis often fatal. Postinfarction cardioembolic stroke is the most frequent and has the highest mortality of all ischemic stroke subtypes. In this case report we describe cardioembolic stroke from postinfarction apical left ventricular aneurysm as a first symptom of myocardial infarction. In brief literature overview we discuss main risk fac...

Analysis of elective coronary angiography findings in patients with suspected angina pectoris in the cardiocenter Hradec Králové – Real-life clinical practice in light of the guidelines
ConclusionMost of the patients referred for elective CA had medium risk of IHD. The patient medical history alone was not a reliable predictor of IHD. The stress tests for detection of ischemia were performed scarcely with the dominance of stress electrocardiogram, which was not reliable in the ischemia detection in the clinical practice. The imaging stress tests were not used to a sufficient extent and the rate of negative elective CAs was high in the analyzed study population. (Source: Cor et Vasa)

Pulmonary embolism cohort in Pisek Hospital
We present retrospective unselected cohort of 204 consecutive patients treated for pulmonary embolism at Internal medicine department of Hospital Písek. We achieved low in-patient mortality (5.88%) even with a very conservative use of thrombolysis (8.82%). In total there were 39 patients with high risk PE (2009 czech nomenclature as ``massive’’), 44 with intermediate-risk PE (2009 czech nomenclature as ``submassive’’), 121 with low-risk PE (2009 czech nomenclature as ``small’’). Data were collected before 2014 ESC guidelines and PEITHO study.CTA was performed in 81.86% of patients. It is an excellent diagnosti...

Foam cell formation and cholesterol trafficking and metabolism disturbances in atherosclerosis
Publication date: Available online 25 July 2018Source: Cor et VasaAuthor(s): Alexandrina Volobueva, Dongwei Zhang, Andrey V. Grechko, Alexander N. OrekhovAbstractFoam cells are typical components of atherosclerotic plaques, where they actively participate in the intracellular cholesterol accumulation. Onset and further progression of atherosclerosis is tightly associated with foam cell formation. These cells can originate both from the circulating monocytes/macrophages and from the resident smooth muscular cells migrating to the growing lesion. Classically activated pro-inflammatory M1 macrophages play an important role in...
