Perioperative management of massive hemoptysis during flexible bronchoscopy: Case report
Publication date: Available online 8 July 2017 Source:Colombian Journal of Anesthesiology Author(s): Juan Camilo Segura-Salguero, Lorena Díaz-Bohada, Juan Ricardo Lutz-Peña, Adriana Maria Posada, Vivian Ronderos Flexible bronchoscopy is a diagnostic or therapeutic procedure with a low incidence of complications (0.08–6.8%). Bleeding after transbronchial biopsy is a rare complication (0–2.8% of the cases), usually resulting in minor bleeding that resolves with local measures. There is no clear definition of massive hemoptysis and due to the low incidence of this condition, there are no practical guidelines for ...

Monitoring of national surgical care indicators in the Peruvian health system
Publication date: Available online 4 July 2017 Source:Colombian Journal of Anesthesiology Author(s): Carlos Javier Shiraishi-Zapata Surgical care is an integral component of healthcare services in all countries, and its quality is evaluated through monitoring of national indicators of safe surgical and anaesthesia care. In Peru, monitoring is only partial because of the lack of data regarding some of the indicators. However, there is a need for comprehensive monitoring in order to gain knowledge of the progress towards the healthcare goals proposed by the World Health Organisation and The Lancet Commission Global Surger...

Caesarean section in a patient with chronic portal vein thrombosis and thrombocytopenia: Case report
We report the case of a pregnant woman at term with primary anti-phospholipid syndrome, portal vein thrombosis, massive splenomegaly, oesophageal varices and thrombocytopenia. The patient underwent an elective caesarean section under general anaesthesia to minimise the risk of spleen and variceal rupture, with a favourable outcome for both the mother and the newborn. Chronic portal vein thrombosis is a rare condition, caused by various reasons, mainly thrombotic diathesis. It leads to increased portal pressure, with development of collateral circulation, splenomegaly and thrombocytopenia. Pregnancy in these conditions is c...

Regional anesthesia guided by ultrasound in the pudendal nerve territory
Conclusions The deep and detailed knowledge of the anatomy of the pudendal nerve and its variations is essential for the realization of Regional Anesthesia techniques guided by images. These promising techniques should continue to be evaluated with clinical studies. (Source: Colombian Journal of Anesthesiology)

Curriculum for the administration of sedation outside the operating room in patients over 12 years of age
Conclusions The Curriculum collects the best available evidence with a view to determine the competencies that the professional administering sedation to patients older than 12 years old shall develop, for diagnostic or therapeutic procedures outside the OR, and hence to contribute to improved quality, reducing the variability and morbidity of patients requiring sedation outside the OR. The hidden world of drug interactions in anesthesia
This article dwells on the anesthetic drug interactions: pharmaceutical (mixing or not mixing), pharmacokinetic (alterations in distribution, metabolism, or clearance), pharmacodynamics (synergism, additive effect) and thermodynamic (affinity and intrinsic activity). The basic anesthetic triangle is a current concept and drug interactions are the cornerstone for safe anesthesia. These interactions are currently neglected and hence the anesthetist should recognize any drug interactions for a safer practice of anesthesia. (Source: Colombian Journal of Anesthesiology)

Considerations in pediatric patients with obstructive sleep apnea/hypopnea syndrome (OSAHS): From physiopathology to the perioperative period
This article addresses relevant physiopathology, clinical and paraclinical topics, perioperative treatment and care of pediatric patients with OSAHS. Materials and methods A literature search was conducted in PubMed and Embase databases using the MeSH terms: sleep apnea syndromes, obstructive sleep apnea, physiopathology, perioperative period, and perioperative care. The titles and/or the abstracts of each of the obtained results were reviewed and 60 articles were finally selected. Results and conclusion The knowledge of the physiopathology, the adequate preanesthetic assessment and the pertinent request of paraclinical ...

Lactate serial measurements and predictive validity of early mortality in trauma patients admitted to the intensive care unit
Conclusions Our findings determined a similar performance of the lactic acid as compared to other studies, especially in the lactic acid level at 24h with relevant discrimination over 2.35mEq/L at 24. Nonetheless, reverse causality can eventually occur. (Source: Colombian Journal of Anesthesiology)

Intraoperative transesophageal echocardiography to evaluate a Gerbode defect
Publication date: April–June 2017 Source:Colombian Journal of Anesthesiology, Volume 45, Issue 2 Author(s): Sergio Bustamante, Shravan Cheruku The Gerbode Defect is a rare form of intracardiac shunt from the left ventricle to the right atrium resulting from a defect in the membranous portion of the inter-ventricular septum. This defect can be congenital or acquired, with acquired causes including complications of aortic valve replacements and endocarditis. Intraoperative real time transesophageal echocardiography and 3D echocardiography are vital for diagnosis and anatomic characterization of the shunt. A thorough un...

Use of total intravenous anaesthesia in Colombia: A national survey among active anaesthetists in Colombia
Conclusions The use of TIVA has been limited by the low availability of technology, lack of experience or knowledge among anaesthetists, and a preference for the balanced anaesthesia technique. Many anaesthesiologists prefer balanced techniques for general anaesthesia. Some devices used for TIVA administration raise concerns about patient safety.
