Hyaluronic Acid Basics and Rheology
Hyaluronic acid (HA) is the most common dermal filler in use. It improves wrinkles and volume loss not only by filling and volumizing but also by hydrating the injected area with its water affinity. It is a naturally occurring component of skin, and there is a negligible risk of immunologic or allergic reaction with injection. It is rapidly degraded by the injection of hyaluronidase, thus creating an ideal injectable material that is low risk and reversible. Its duration of effect may be longer than expected based on bioavailability of the HA product due to collagen synthesis or fibroblast stimulation. (Source: Clinics in Plastic Surgery)

Treatment of Periorbital Vascularity, Erythema, and Hyperpigmentation
Periorbital hyperpigmentation (POH) is a common aesthetic concern that impacts patients ’ emotional well-being and quality of life. POH can be difficult to manage as the etiology is often multifactorial or difficult to elucidate. An understanding of different contributing factors and ability to classify hyperpigmentation can aid in the management of POH. Classification of POH is divi ded into pigmented, vascular, structural, and mixed subtypes. A wide array of treatment options has been proposed belying the challenges inherent to improving POH. Modalities vary from topical therapies, chemical peels, dermal fillers, and l...

Mastering Midface Injections
Injectable filler is one of the most common cosmetic procedures performed annually. An aging face shows a characteristic loss of volume in the deep fat pads of the midface. The goal of midfacial rejuvenation with injectable filler is to restore lost volume, with the suborbicularis fat pad and deep medial cheek fat being the most critical areas. Filler can be instilled here with a cannula or needle with successful outcomes. However, this procedure is not without complications if proper technique and underlying anatomy are not respected. Liquid Rhinoplasty
Soft tissue filler injections have become the second most common noninvasive cosmetic procedure performed in the United States, accounting for roughly 26% of all noninvasive procedures. As experience with filler injections has increased, so too have the applications and uses throughout the face. The popularity of “liquid,” or nonsurgical, rhinoplasty has grown considerably with both patients and surgeons over the last decade. First documented in 2006, numerous descriptions of technique and application have grown in tandem with the increasing popularity of nonsurgical rhinoplasty procedures. Although nasa l injections r...

Creating Ideal Lips with Toxins and Fillers
The lips are a central and essential feature of facial appearance and esthetics. When looking at attractive faces, we spent most of our time observing the eyes and lips although the rest of the facial features fade away.1 Full lips are beautiful and youthful.2 Treatments to enhance the lips have long been pursued. Silicone injections were popular in the 1960s and collagen injections in the 1970s. Also, during the 1970s surgical lip implants with expanded polytetrafluororethylene (Gortex ®) were widely used a longer-lasting alternative to collagen injections. Minimally Invasive Techniques for Facial Rejuvenation Utilizing Polydioxanone Threads
Cosmetic procedures to combat the effects of aging are increasing in demand. Surgical interventions, such as rhytidectomy, have long been the standard method of providing a more youthful appearance. However, these procedures are costly, often require general anesthesia, and have potential risks such as scarring and prolonged recovery. A safe, effective, alternative to surgery is the nonsurgical thread-lift. Nevertheless, proper patient selection is critical for optimal outcomes and for patient and provider satisfaction. Over the past decade, these treatments have gained significant popularity for patients to achieve a more...

Microneedling-Associated Procedures to Enhance Facial Rejuvenation
Microneedling, also referred to as percutaneous collagen induction therapy, uses small needles to create mechanical injury to the skin, stimulating the wound-healing cascade and new collagen formation. Compared with other skin resurfacing techniques, microneedling preserves the epidermis and is nonablative, therefore reducing inflammation, downtime, and risk of dyspigmentation. In addition to increasing collagen production in fibroblasts, microneedling also helps normalize cell function of keratinocytes and melanocytes and can be used to increase absorption of topical medications, growth factors, or deliver radiofrequency ...

Nonsurgical Jaw Contouring
The aging face is a function of facial volume loss resulting from a combination of factors, including bony resorption, fat repositioning, and overall tissue laxity. In the lower third of the face, these changes translate to loss of definition of the jawline. A well-contoured jawline is an aesthetically pleasing feature for both men and women. Studies have demonstrated that by improving jawline contour, one can help increase perceptions of attractiveness, likeability, extroversion, and masculinity/femininity. Nonsurgical Rejuvenation of the Neck
Nonsurgical skin tightening in the neck is an area of significant growth with seemingly countless devices. Optimal treatment choice depends on the patient ’s concerns, anatomy, and lifestyle. Patients with minimal skin laxity, but dynamic platysmal bands may benefit from botulinum toxin injections. Mild to moderately lax skin can be addressed with microfocused ultrasound or radiofrequency with microneedling. Significant sun damage and laxity can bot h be addressed with fractional ablative lasers. Options for submental preplatysmal fat include percutaneous radiofrequency, radiofrequency-helium plasma, deoxycholic acid inj...

Noninvasive Hair Rejuvenation
Both nonsurgical and surgical modalities for the treatment of hair loss are being used by providers at an increasing rate worldwide. Men and woman are affected by hair loss, but the pathophysiology of the hair loss is thought to be different between sexes; therefore, gender must play a role in treatment decisions. Currently, there are 3 Food and Drug Administration –approved nonsurgical androgenetic alopecia treatments: minoxidil, finasteride, and low-light laser therapy. Platelet-rich plasma injections are showing promise as a single modality and as an adjunct to other nonsurgical and surgical treatments of androgenetic...

Injectables and Minimally Invasive Rejuvenation of the Face and Neck
Facial aging is a complex process with multifactorial influences requiring targeted approaches to restore a more refreshed and natural appearance. Optimal facial rejuvenation therefore requires a multimodal treatment approach to facial aging, including both surgical and nonsurgical therapies. These commonly include some combination of a skin care regimen, surface treatment of the skin, volume rejuvenation, soft tissue resuspension or remodeling, skin tightening, and treatment of both static and dynamic rhytids.
