Asian Upper Blepharoplasty
Asian upper blepharoplasty is one of the most popular cosmetic procedures for Asians, but less optimal result is still common. The authors ’ comprehensive approach to Asian upper blepharoplasty includes (1) to determine both the height and length of the upper skin crease; (2) to create more ideal anatomy of the upper eyelid; (3) to reconstruct the desirable anatomic structure of the upper eyelid skin crease; and (4) to add a medial epicanthoplasty if needed to enhance cosmetic results. Primary Rhinoplasty Combined with Pyriform Aperture Augmentation in Asians
To explore a pyriform aperture augmentation method that is suitable for Asian patients with mid-face concavity, we designed a novel prosthesis to perform pyriform aperture augmentation. Three different rhinoplasty schemes are designed according to the degree of mid-face concavity: (1) simple implantation around the pyriform aperture, (2) implantation around the pyriform aperture and augmentation rhinoplasty is performed concurrently, and (3) implantation around pyriform aperture combined with nasal extension technique. Comprehensive rhinoplasty with pyriform aperture augmentation can substantially improve the aesthetic rel...

Facial Beautification and Rejuvenation with Injectables
Asians are of different ethnic origins. Nevertheless, they share the common facial structural deficiencies and consequences of the aging process that create the opportunity for esthetic intervention. As a result, Injectables are widely used in young Asians to achieve an oval facial shape with increased facial height and projection. In mature Asians with lifelong structural deficiencies, the beauty treatment plan is more complex than removal of signs of aging. Therefore, apart from injectables, other treatment modalities for tissue repositioning, neocollagenesis and skin quality improvement are also required to obtain the o...

Facial Rejuvenation and Contouring with Radiofrequency-Assisted Procedures in Asians
Radiofrequency (RF)-assisted liposuction treatment is a minimally invasive skin-tightening technique for the patient population with skin laxity. The authors recommend facial liposuction combined with the RF procedure for the treatment of skin laxity. Minimal-invasive liposuction creates working channels for RF treatment and sufficiently exposes the subdermal fibrous septal network tissue so that the RF energy can directly act on the collagen of the fibrous septal network for thermal shrinkage, leading to better surgical results. In this article, the authors describe their preferred technique and experience for face rejuve...

Orthognathic Surgery to Enhance the Smile
An esthetic smile is an integral feature of beauty. Improvement of the smile can be achieved by a combination of orthognathic surgery, orthodontics, and cosmetic dentistry. Preoperative evaluation serves to address a patient ’s surgical goals; it allows a surgeon to perform a detailed facial analysis and identify patients who are contraindicated for surgery. LeFort I and bilateral sagittal split osteotomy are performed to minimize the risk of complications. Injuries to the inferior alveolar nerve are the most common c omplication after orthognathic surgery, in which 90% of patients experience transient sensory disturbanc...

Advanced Approach to Asian Lower Blepharoplasty
The lower eyelid is one of the commonest areas Asian patients present for rejuvenation. The spectrum of patients presenting for treatment ranges from the young to the more mature. Young patients present with “premature” aging due to an anatomical predisposition. These are patients we figuratively call those who “do not age well.” Even patients with strong skeletal support manifest aging, in a predictable and progressive manner. Such patients eventually benefit from quality esthetic surgery of th e lower eyelid and mid cheek. (Source: Clinics in Plastic Surgery)

Labia Minora Reduction Using Central Wedge Technique
The central wedge labiaplasty removes a “V” or a pie-slice piece of the most protuberant portion of the labium to make it smaller. The anterior and posterior edges of the labium are reapproximated to preserve the normal labial edge and anatomy with minimal scarring. The lateral or medial-lateral clitoral hood can be reduced at the sam e time. The complication rate is very low if done using meticulous technique, thus avoiding the problems with alternative labiaplasty methods. The patient satisfaction level is well over 90%. Genital Self-Image and Esthetic Genital Surgery
Genital self-image describes the perception of one ’s genital appearance as being “normal” or “abnormal,” and a disharmonious image leads to an increasing number of women seeking esthetic genital surgery each year. The concept of what constitutes “normal” is strongly influenced by the media, cultural norms, and sexual relations. In rea lity, the extent of normalcy is highly variable and overall patient education regarding extremes of size and shape should be provided to all patients considering surgery. When performed with appropriate training, expertise, and attention to detail in a properly selected patient...

Anatomical Changes of the Vulva Due to Childbirth and Aging
The female reproductive system consists of several structures. These organs are constantly undergoing changes throughout the lifetime of a woman. Most of these occur due to changes in estrogen and progesterone levels at key transitions in life such as puberty, pregnancy, and menopause. Laxity and atrophy occur during pregnancy and menopause, respectively, can cause significant symptomology for women. Although some women do have return to normal anatomy following pregnancy, many do not and develop pelvic organ prolapse that worsens over their lifetime. Symptoms are often underreported, but for those who do have symptoms, su...

The Aesthetic Genital Consultation
Consultation for female esthetic genital concerns is unique in that the topic is shrouded in secrecy. Few opportunities exist for the improvement of female sexual health and well-being when compared with male counterparts. Women concerned with the appearance of their genitalia seek help with improving their confidence and reducing mental and physical discomfort attributed to large labia minora. Validated questionnaires given to patients beforehand, a detailed sexual history, and a comprehensive vulvar examination help define the esthetic and functional issues. Providing informed consent and setting realistic expectations a...

Curvilinear Labiaplasty and Clitoral Hood Reduction Surgery
The two base techniques for labiaplasty surgery are the linear excision and the wedge excision. The modification of using radiofrequency in a curvilinear manner is exceptionally precise, safe, and can produce exceptionally beautiful results based on surgical skill and experience. We recommend using this curvilinear technique as the core skill to learn as a labial surgeon both for ease and safety. Presented are the steps to achieve safe and consistent results no matter the anatomy. The method to reduce the lateral clitoral hood to help balance the appearance of the vulvar structures is also presented. (Source: Clinics in Plastic Surgery)

Vaginal Tightening
The natural aging process and pregnancies come with certain body changes that influence overall our performance and the vagina is no different. Today more and more patients seek for new ways to enhance their sexual gratification. It is important to listen and examine the patient to look for vagina wall looseness and if a big prolapse is present it is not an ideal case for the procedure. Only tightening the vagina entrance is no enough to really impact female sexual gratification. Treating a loose vagina, loose vagina walls, really impacts sexual gratification on the woman improving their quality of life. (Source: Clinics in Plastic Surgery)

Female Aesthetic Genital Procedures
It is a great honor that I have been asked to be the guest editor for the first Female Aesthetic Genital Procedures issue of the Clinics in Plastic Surgery. Including this new controversial field among the more traditional plastic surgery topics once and for all validates its existence. In 1998, I performed my first labiaplasty on a young woman encumbered with large labia minora after reading Gary Alter ’s article on the wedge technique. The surgery went well, but what truly struck me was how happy and confident the patient was afterward. (Source: Clinics in Plastic Surgery)
