Fracture Management in Chronic Kidney Disease: Challenges and Considerations for Orthopedic Surgeons
Clin Orthop Surg. 2024 Apr;16(2):173-183. doi: 10.4055/cios23244. Epub 2024 Mar 15.ABSTRACTOrthopedic surgeons treating fractures need to consider comorbidities, including chronic kidney disease (CKD), which affects millions worldwide. CKD patients are at elevated risk of fractures due to osteoporosis, especially in advanced stages. In addition, fractures in CKD patients pose challenges due to impaired bone healing and increased post-fracture complications including surgical site infection and nonunion. In this article, we will discuss factors that must be considered when treating fractures in CKD patients. Perioperative m...

Does Preoperative Radiation Therapy Performed for Metastatic Spine Cancer at the Cervical Spine Increase Perioperative Complications of Anterior Cervical Surgery?
CONCLUSIONS: RT performed before surgery for MSC does not increase the risk of wound complication, mechanical failure, or vital structure injury during ACSS. The surgical procedural approach was not complicated by previous RT history. Therefore, surgeons can safely choose the anterior approach when the number of levels or location of MSC favors anterior surgery, and performing a posterior surgery is unnecessary due to a concern that previous RT may increase complication rates of ACSS.PMID:38562630 | PMC:PMC10973614 | DOI:10.4055/cios23322 (Source: Clinics in Orthopedic Surgery)

Cardiovascular Disease in Total Knee Arthroplasty: An Analysis of Hospital Outcomes, Complications, and Mortality
CONCLUSIONS: This study demonstrated that CHF, CAD, valvular dysfunction, and arrhythmia are prevalent among TKA patients and associated with worse hospital metrics, higher risk of perioperative complications, and increased mortality. As our use of TKA rises, a lower threshold for preoperative cardiology referral in older individuals and early preoperative counseling/intervention in those with known cardiac disease may be necessary to reduce adverse outcomes.PMID:38562631 | PMC:PMC10973625 | DOI:10.4055/cios23224 (Source: Clinics in Orthopedic Surgery)

Patella Cartilage Status Does Not Affect the Clinical Outcomes of Non-resurfaced Patella in Mobile-Bearing Total Knee Arthroplasty
CONCLUSIONS: The grade and location of the patellar cartilage lesion had no influence on clinical outcomes in mobile-bearing TKA with patellar retention at short- and long-term follow-up.PMID:38562632 | PMC:PMC10973628 | DOI:10.4055/cios22253 (Source: Clinics in Orthopedic Surgery)

Low Hand Grip Strength, Mid-Upper Arm Muscle Area, Calf Circumference, Serum Albumin Level, and Muscle Fiber Diameter as Risk Factors for Independent Walking Inability in Patients with Hip Fracture 6 Weeks after Bipolar Hemiarthroplasty Surgery
CONCLUSIONS: Low MUAMA, serum albumin levels, and muscle fiber diameter are direct risk factors for the inability to walk independently in hip fracture patients 6 weeks after bipolar hemiarthroplasty.PMID:38562634 | PMC:PMC10973617 | DOI:10.4055/cios23256 (Source: Clinics in Orthopedic Surgery)

Intertrochanteric (Reverse Oblique) Fracture Subclassifications AO/OTA 31-A3 Have No Effect on Outcomes or Postoperative Complications
CONCLUSIONS: Based on the findings of this study, no significant differences were observed between the groups, indicating that the subclassifications of AO/OTA 31-A3 fractures do not have a significant impact on surgical outcomes or the occurrence of postoperative complications.PMID:38562635 | PMC:PMC10973612 | DOI:10.4055/cios23204 (Source: Clinics in Orthopedic Surgery)

Is Surgical Treatment Necessary for the Treatment of a Jones Fracture?: A Retrospective Study
CONCLUSIONS: Surgical treatment is sometimes necessary for Jones fractures, but conservative treatment should also be considered because of the favorable outcomes. Conservative treatment can be a good option for patients who are risk-averse and place a high value on fracture healing without surgery.PMID:38562636 | PMC:PMC10973620 | DOI:10.4055/cios23333 (Source: Clinics in Orthopedic Surgery)

Epidemiology and Characteristics of Ankle and Foot Injuries in Electric Scooter-Related Accidents
CONCLUSIONS: The ankle and foot are the most common injury sites in e-scooter-related accidents. Given the high frequency and severity of e-scooter-related ankle and foot injuries, we suggest that more attention be paid to preventing these types of injuries with greater public awareness of the dangers of using e-scooters.PMID:38562638 | PMC:PMC10973611 | DOI:10.4055/cios23312 (Source: Clinics in Orthopedic Surgery)

Biomechanical Characteristics of Glenosphere Orientation Based on Tilting Angle and Overhang Changes in Reverse Shoulder Arthroplasty
CONCLUSIONS: A 10° inferior tilt of the glenosphere, without changing the position of the baseplate, had no benefit in terms of the impingement-free angle and deltoid moment arm. However, an eccentric glenosphere had a significant advantage, regardless of inferior tilt. Inferior tilt through 10° inferior reaming showed better biomechanical results than neutral tilt due to the distalization effect.PMID:38562639 | PMC:PMC10973622 | DOI:10.4055/cios23217 (Source: Clinics in Orthopedic Surgery)

Comparative Interrupted Time Series Analysis of Direct Medical Expense and Length of Stay in Elderly Patients with Femoral Neck Fractures Who Underwent Total Hip Arthroplasty and Hemiarthroplasty: A Real World Nationwide Database Study
CONCLUSIONS: When performing THA in elderly patients with femoral neck fractures, the possibility of survival for at least 2 years should be considered from the perspective of medical expense and medical utilization. Additionally, in healthy and active male femoral neck fracture patients under the age of 80 years, THA may be more recommended than HA.PMID:38562640 | PMC:PMC10973615 | DOI:10.4055/cios23282 (Source: Clinics in Orthopedic Surgery)
